AimWe investigated the effect of age on post-cardiac arrest treatment outcomes in an elderly population, based on a local database and a systemic review of the literature.MethodsData were collected retrospectively from medical charts and reports. Sixty-one comatose patients, cooled to 32–34 °C for 24 h, were categorized into three groups: younger group (≤65 years), older group (66–75 years), and very old group (>75 years). Circumstances of cardiopulmonary resuscitation (CPR), patients' characteristics, post-resuscitation treatment, hemodynamic monitoring, neurologic outcome and survival were compared across age groups. Kruskal-Wallis test, Chi-square test and binary logistic regression (BLR) were applied. In addition, a literature search of PubMed/Medline database was performed to provide a background.ResultsAge was significantly associated with having a cardiac arrest on a monitor and a history of hypertension. No association was found between age and survival or neurologic outcome. Age did not affect hemodynamic parameter changes during target temperature management (TTM), except mean arterial pressure (MAP). Need of catecholamine administration was the highest among very old patients. During the literature review, seven papers were identified. Most studies had a retrospective design and investigated interventions and outcome, but lacked unified age categorization. All studies reported worse survival in the elderly, although old survivors showed a favorable neurologic outcome in most of the cases.ConclusionThere is no evidence to support the limitation of post-cardiac arrest therapy in the aging population. Furthermore, additional prospective studies are needed to investigate the characteristics and outcome of post-cardiac arrest therapy in this patient group.
Proper hemodynamic management is necessary among post-cardiac arrest patients to improve survival. We aimed to investigate the effects of PiCCO™-guided (pulse index contour cardiac output) hemodynamic management on mortality in post-resuscitation therapy. In this longitudinal analysis of 63 comatose patients after successful cardiopulmonary resuscitation cooled to 32–34 °C, 33 patients received PiCCO™, and 30 were not monitored with PiCCO™. Primary and secondary outcomes were 30 day and 1 year mortality. Kaplan–Meier curves and log-rank tests were used to assess differences in mortality among the groups. Interaction effects to disentangle the relationship between patient’s condition, PiCCO™ application, and mortality were assessed by means of Chi-square tests and logistic regression models. A 30 day mortality was significantly higher among PiCCO™ patients, while 1 year mortality was marginally higher. More severe patient condition per se was not the cause of higher mortality rate in the PiCCO™ group. Patients in better health conditions (without ST-elevation myocardial infarction, without cardiogenic shock, without intra-aortic balloon pump device, or without stroke in prior history) had worse outcomes with PiCCO™-guided therapy. Catecholamine administration worsened both 30 day and 1 year mortality among all patients. Our analysis showed that there was a complex interaction relationship between PiCCO™-guided therapy, patients’ condition, and 30 day mortality for most conditions.
Összefoglaló. Bevezetés: A cardiovascularis halálokok közül világszerte nagy jelentőségű a hirtelen szívhalál. Annak ellenére, hogy a cardiopulmonalis resuscitatio és a postresuscitatiós intenzív osztályos kezelés is komoly metodikai és technikai fejlődésen ment keresztül az elmúlt időszakban, kevés az olyan validált pontrendszer, amely jól becsülné a beteg intenzív osztályra kerülésekor a mortalitási rizikót. Célkitűzés: A sikeres újraélesztést követő intenzív osztályos kezelés kezdetekor felmért, a cardiogen shock rizikóstratifikációjára alkalmazott CardShock Risk Score (CSRS) és az általunk hozzáadott, specifikus súlyozófaktorokkal (iniciális ritmus, inotropigény) módosított CardShock Risk Score (mCSRS) összevetése a mortalitás előrejelzésében post-cardiac arrest szindrómás betegeknél. Módszerek: Retrospektív vizsgálatunk során 172, kórházon kívül sikeresen újraélesztett és klinikánkon ellátott consecutiv betegből a CSRS- és mCSRS-pontrendszerek segítségével végül 123 beteg adatait elemeztük. A CSRS- és mCSRS-változók és a korai/késői mortalitás közötti összefüggést Cox-regressziós analízissel vizsgáltuk. A pontszámok alapján 3 csoportba (1–3, 4–6, 7+) soroltuk a betegeket. Az összevont csoportok túlélését log-rank teszttel hasonlítottuk össze. Eredmények: A betegpopuláció átlagéletkora 63,6 év volt (69% férfi), és a hirtelen szívhalál hátterében 80%-ban akut coronaria szindróma állt. A korai/késői mortalitást leginkább a felvétel utáni neurológiai állapot, a szérumlaktátszint, a vesefunkció, az iniciális ritmus és a beteg katecholaminigénye határozta meg. A mCSRS alkalmazását követően mind az „1–3” és a „4–6” (p≤0,001), mind a „4–6” és a „7+” (p = 0,006) csoportok között szignifikáns különbséget találtunk a túlélésben. Orv Hetil. 2021; 162(2): 52–60. Summary. Introduction: Sudden cardiac death is one of the most significant cardiovascular causes of death worldwide. Although there have been immense methodological and technical advances in the field of cardiopulmonary resuscitation and following intensive care in the last decade, currently there are only a few validated risk-stratification scoring systems for the quick and reliable estimation of the mortality risk of these patients at the time of admission to the intensive care unit. Objective: Our aim was to correlate the mortality prediction risk points calculated by CardShock Risk Score (CSRS) and modified (m) CSRS based on the admission data of the post-cardiac arrest syndrome (PCAS) patients. Methods: The medical records of 172 out-of-hospital resuscitated cardiac arrest patients, who were admitted at the Heart and Vascular Centre of Semmelweis University, were screened retrospectively. Out of the 172 selected patients, 123 were eligible for inclusion to calculate CSRS and mCSRS. Based on CSRS score, we generated three different groups of patients, with scores 1 to 3, 4 to 6, and 7+, respectively. Mortality data of the groups were compared by log-rank test. Results: Mean age of the patients was 63.6 years (69% male), the cause of sudden cardiac death was acut coronary syndrome in 80% of the cases. The early and late mortality was predicted by neurological status, serum lactate level, renal function, initial rhythm, and the need of catecholamines. Using mCSRS, a significant survival difference was proven in between the groups “1–3” vs “4–6” (p≤0.001), “4–6” vs “7+” (p = 0.006). Conclusion: Compared to the CSRS, the mCSRS expanded with the 2 additional weighting points differentiates more specifically the low-moderate and high survival groups in the PCAS patient population treated in our institute. Összefoglaló. A koronavírus-betegség (COVID–19) okozta közvetlen mortalitáson túl, a járvány közvetett úton is hatással lehet a hirtelen szívhalálra. Egyre növekvő számú közlemény foglalkozik a járványnak a hirtelen szívhalálra kifejtett közvetett hatásával. A kijárási korlátozások és az egészségügyi rendszerek átszervezése hozzájárulhatott ahhoz, hogy a járvány alatt mind a kórházon kívüli, mind a kórházon belüli szívhalál előfordulása megemelkedett. Közegészségügyi intézkedések, mint a korlátozások és a kórházak átszervezése, megváltoztathatják az egészségügyi szolgáltatásokhoz való hozzáférést, ezért hozzájárulhattak az elmúlt évben tapasztalt emelkedett számú szívmegálláshoz. Közleményünk célja a SARS-CoV-2-járvány hirtelen szívhalálra kifejtett hatására vonatkozó, a nemzetközi irodalomban jelenleg megtalálható tanulmányok összefoglalása, melyek a kórházon kívüli szívmegállás előfordulásának háromszoros emelkedéséről számoltak be a járványt megelőző évhez képest. Általánosságban elmondható, hogy a kórházon kívüli szívmegállás a járvány ideje alatt nagyobb gyakorisággal járt nem sokkolandó ritmussal, hosszabb idő telt el a mentők kiérkezéséig, alacsonyabb volt a szemtanú által megkezdett újraélesztés, a spontán keringés visszatérésének, valamint a kórházi elbocsátásnak a gyakorisága. A járványnak a kórházon belüli szívmegállásra kifejtett hatása kevésbé vizsgált az irodalomban. Míg a hirtelen szívhalált követő mortalitás néhány kutatásban jelentős emelkedést mutatott, addig máshol nem volt különbség a járványt megelőző időszakhoz képest. A COVID–19-pandémia ideje alatt jelentősen megnövekedett kórházon kívüli és belüli szívmegállás hátterében a járványnak közvetett úton is szerepe lehet, a fertőzés közvetlen hatása mellett. A túlélési lánc megbomlását számos helyen tapasztalták, ami hozzájárulhatott a kedvezőtlen kimenetelhez. Mind a prehospitális, mind pedig a hospitális ellátás gyakorlatában bekövetkező jelentős változások magyarázhatják a világ különböző pontjain megfigyelt eltéréseket. Orv Hetil. 2021; 162(46): 1831–1841. Summary. The direct effect of COVID-19 on mortality through acute respiratory failure is well-established. However, there are a growing number of publications suggesting that the prevalence and outcome of sudden cardiac death may also be indirectly affected by the pandemic. Public health measures, such as lockdowns and reorganisation of hospitals, can alter the access to healthcare services and therefore might have contributed to the excess number of cardiac arrests which were seen over the last year. Our aim was to review the currently available publications regarding the impact of the COVID-19 pandemic on out-of-hospital and in-hospital cardiac arrests. A recent study reported a 3-fold growth in the incidence of out-of-hospital cardiac arrests during the 2020 COVID-19 period compared to the year before. In general, the number of non-shockable rhythms increased, bystander-witnessed cases and bystander-initiated cardiopulmonary resuscitation were reduced and ambulance response times were significantly delayed during the pandemic. Return of spontaneous circulation and survival to discharge substantially decreased compared to the time before the pandemic. The difference between the rate of mortality following in-hospital cardiac arrest during and before the pandemic is controversial according to published data. The incidence of out-of-hospital and in-hospital cardiac arrests significantly increased during the pandemic compared to previous years suggesting direct effects of COVID-19 infection and indirect effects from new public health measures. The disruption of the chain of survival could have contributed to the increased mortality following out-of-hospital cardiac arrest. Orv Hetil. 2021; 162(46): 1831–1841.
A koronavírus-betegség megjelenése óta világszerte nyomon követik a fertőzésekhez kapcsolódó közvetlen halálozást, és naponta számolják a vírus okozta halálesetek számát. A koronavírus okozta világjárvány nemcsak mindennapi életünket, de a teljes egészségügyi rendszer működését átrendezte. A kórházi felvételre való fokozott igény kapcsán a különböző országok vezetői számos, az egészségügyi ellátást befolyásoló rendkívüli intézkedést vezettek be. Az átrendeződés mind direkt, mind indirekt módon negatív hatással volt a hirtelen szívhalál epidemiológiai mutatóira, a laikus elsősegélynyújtók cardiopulmonalis resuscitatióra való hajlandóságára és a félautomata defibrillátor használatára, ezek a negatív hatások azonban széles határok között mozognak a különböző kontinenseken és országokban. A laikusok és az egészségügyi dolgozók védelme, illetve a pandémia terjedésének megakadályozása érdekében a korábbi alapszintű és emelt szintű újraélesztésre vonatkozó, az Európai Újraélesztési Tanács által megfogalmazott ajánlások számos változtatáson estek át. Bevezetés:A hirtelen szívhalál (HSZH) vezető halálozási okként szerepel a fejlődő ipari országokban, amelynek hátterében leggyakrabban malignus ritmuszavarok állnak. Ezeket sokkolandó (kamrafi brilláció, kamrai tachycardia), illetve nem sokkolandó (asystolia, elektromechanikus disszociáció) formákra osztjuk. Célkitűzés: Vizsgálatunk célja volt összehasonlítani a hosszú távú túlélést HSZH esetén, sokkolandó, illetve nem sokkolandó ritmusú betegek esetében. Anyag és módszer: Retrospektív tanulmányunkban a SE VSZÉK-án 2008 és 2011 között kezelt, 116 hirtelen szívhalált elszenvedett, konszekutív beteg adatait vetettük össze. Hirtelen szívhalált követően az iniciálisan észlelt ritmus szerint két csoportba soroltuk őket, és összehasonlítottuk a túlélési arányokat kórhazi távozáskor, egy hónapot, illetve három hónapot követően. Statisztika analízist t-próba, c 2 -próba és nem parametrikus (Mann-Whitney-Wilcoxon) próba segítségével végeztünk. Az iniciálisan tisztázatlan ritmussal rendelkező betegek kizárásra kerültek. Eredmények: A vizsgált populáció 67%-a rendelkezett iniciálisan sokkolandó ritmussal. Szignifi kánsan magasabb számban távoztak a kórházból a sokkolandó ritmusú csoport tagjai (54% vs. 16%, p=0,0002). Az első és a harmadik hónapot túlélt betegek aránya sziginifi kánsan magasabb volt a sokkolandó ritmusú csoportban mindkét vizsgált időpontban 60% vs. 24%, p=0,0001, illetve 49% vs. 16%, p=0,0004. Következtetés: Az álta-lunk vizsgált esetekben, hirtelen szívhalált követően lényegesen jobb prognózissal rendelkeztek az iniciálisan sokkolandó ritmusú betegek, mint a nem sokkolandó ritmusú társaik, amely megegyezést mutat az irodalommal.Relationship between survival and initial rhythm after cardiac arrest Background: The sudden cardiac death (SCD) is one of the main purposes of death in the developing countries. The most common causes of SCD are malignant arrhythmias. That could be devided into two major groups by sort: shockable (ventricular fi brillation, ventricular tachycardia) and non-shockable rhythms (asystole, pulseless electrical activity). Aim and Methods: The aim of this study was to analize the long-term survival after SCD due to initial rhythm. The data were collected retrospectively from Semmelweis University Heart and Vascular Center between 2008 and 2011. We analized the data of 116 consecutive patient. Patients were devided to initial shockable (SR) and non-shockable rhythm (NSR) groups, and we compared the survival rates at hospital discharge, and after 1 and 3 months. Patients detected with unclear data of initial rhythm were excluded. Statistical analysis was performed with non-parametric tests, c 2 test and t test. Results: 67% of patients have got initially shockable rhythm. Patients with SR had a higher hospital discharge rate than those with NSR (54% vs. 16%, p=0.0002). The 1-month and 3-months survival rates were significantly higher in the SR group, than in the NSR in both cases, 60% vs. 24%, p=0.0001, and 49% vs. 16%, p=0.0004. Proper hemodynamic monitoring is necessary among post-cardiac arrest patients. Moreover, patients in better health conditions (without ST-elevation myocardial infarction, without cardiogenic shock, without intra-aortic balloon pump device or without stroke in prior history) had worse outcomes in PiCCO™-guided therapy. Furthermore, catecholamine administration worsened both 30-day and one-year mortality among all patients. Conclusions Our analysis showed that there was a complex interaction relationship between PiCCO™-guided therapy, patients’ condition, and 30-day mortality for most conditions. Furthermore, in terms of one-year mortality, there was either no benefit whatsoever of PiCCO™ monitoring on survival or there was worse survival among patients who had no underlying conditions but received PiCCO™ (such as among patients who had no STEMI or had no stroke).
