This is the first study to report the range of normal volume for Caucasian lacrimal glands measured on CT scans. The volume of the lacrimal gland decreases with age, and there is no gender or laterality difference.
Imaging is critical for the care of emergency department (ED) patients. However, much of the imaging performed for acute care today is overutilization, creating substantial cost without significant benefit. Further, the value of imaging is not easily defined, as imaging only affects outcomes indirectly, through interaction with treatment. Improving the quality, including appropriateness, of emergency imaging requires understanding of how imaging contributes to patient care. The six-tier efficacy hierarchy of Fryback and Thornbury enables understanding of the value of imaging on multiple levels, ranging from technical efficacy to medical decision-making and higher-level patient and societal outcomes. The imaging efficacy hierarchy also allows definition of imaging quality through the Institute of Medicine (IOM)'s quality domains of safety, effectiveness, patient-centeredness, timeliness, efficiency, and equitability and provides a foundation for quality improvement. Resumo: A transformação do estatuto dos diferentes espaços urbanos e metropolitanos e das práticas sociais que neles se inscrevem implicaram uma renovação das abordagens aos espaços públicos e um alargamento do debate sobre o seu papel integrador. Este enfoque na função de edificar e reforçar os laços sociais dos espaços públicos, ganha pertinência quando se assiste a um duplo processo na extensão da metropolização caracterizado, simultaneamen te, por uma recuperação e uma redução das relações comunitárias. Partindo-se de uma aná lise dos fenómenos presentes na coexistência socioespacial, procurou-se compreender al guns dos aspectos que tendem para o esvaziamento daquela função dos espaços públicos mais tradicionais e para emergência de novos espaços. Outcomes of Covid-19 organizing pneumonia in critically ill patients
Dear editor,Efficacy of corticosteroids in COVID-19 pneumonia has been reported in numerous studies. 1 However, Yang et al. concluded that corticosteroids have a negative impact, suggesting that not all patients benefit from the therapy. 2 Features of organizing pneumonia (OP) have been observed in radiological and histopathological studies from these patients. [3][4][5] OP has usually a good response to corticosteroids. The possible correlation between the radiological pattern and clinical evolution has important implications. Only patients needing advanced noninvasive respiratory support (NIRS) with high flow nasal cannula (HFNC), continuous positive airway pressure (CPAP) and/or noninvasive ventilation (NIV) were included. All patients had a "do intubate" (DI) or "do not intubate" (DNI) order defined at admission. Information regarding demographics, comorbidities, blood test results and mode and usage of NIRS were recorded. Resumo: Este artigo pretende dar um contributo à reflexão e à avaliação sobre a persistên cia da situação habitacional precária dos portugueses ciganos ao longo dos tempos. Neste sentido, serão analisados os principais factores exógenos à população cigana que poderão justificar a perpetuação das situações de exclusão habitacional. Partindo-se de um enquadramento sobre a dimensão deste fenómeno explicitam-se aqueles factores, através de três eixos: l) os discursos técnica e cientificamente pouco fundamentados para justificar a precariedade habitacional e a inacção política, onde o nomadismo aparece como denomina dor comum, seja enfatizando-se a presença de ciganos itinerantes, seja admitindo-se a sua inexistência; 2) as estratégias públicas de âmbito local para a não fixação e para a mobilida de forçada; 3) a inexistência de tradução dos problemas e necessidades sentidos localmente nos sucessivos Planos Nacionais de Acção para a Inclusão e em medidas de política de habitação que pudessem colmatar alguns dos problemas identificados.
