In recent years body-orientated concepts have gained more and more importance in the therapy of mental disorders. But there is still a widespread skepticism about the effectiveness of movement-therapeutic measures. In the present study the effectiveness of an unspecific versus a disorder-orientated movement-therapy was tested in the clinical setting including a 6-month catamnesis with depressive patients (n=103) with a BDI >18. The results show, at every data point, the effectiveness of the measures for all investigated parameters. Both forms of movement-orientated interventions differ only slightly with regard to the therapeutic success. The catamnesis shows a big gap between resolution and actually realized activities. On the basis of the results it can be assumed that mainly general unspecific determinants play a more important role for the therapeutic success than assumed so far.
In zwei Untersuchungen [24,25] ist es in jüngster Zeit im Rahmen von Laborexperimenten gelungen, gezielt Flow-Erfahrungen über die Beanspruchungsintensität zu induzieren. Das Konzept dieser Studien wurde in der vorliegenden Untersuchung bei klinisch depressiven Patienten (n = 31) eingesetzt. Die Probanden sollten in diesem Zusammenhang 40 Minuten innerhalb eines vorher bestimmten individuellen Trainingsbereiches auf einem Fahrradergometer fahren. Mithilfe einer beanspruchungsorientierten Widerstandssteuerung des Ergometers gelang es, die Teilnehmer während der gesamten Zeit innerhalb des gewählten Bereiches zu halten. Das Flow-Erleben wurde während der Aktivität mit der Flow-Kurzskala von Rheinberg (2003) gemessen [27]. Um die kurzfristigen Auswirkungen auf Variablen der aktuellen Befindlichkeit zu untersuchen, kam die Kurzform der Befindlichkeitsskala von Abele-Brehm und Brehm [3] vor und nach der Intervention zum Einsatz. Als Fazit lässt sich feststellen, dass die Probanden über ein tiefes und im Verlauf stabiles Flow-Erleben berichten. Die Veränderungen der Befindlichkeit lassen sich klassisch im Sinne eines Eisbergprofils darstellen. Dem Verband Deutscher Rentenversicherungsträger zufolge waren Depressionen 2003 die häufigste Ursache für Berentung [30]. Die World Health Organisation (WHO) kommt im Rahmen der Global-Burden-of-Disease-Studie zu dem Ergeb-nis, dass unipolare Depressionen weltweit in allen Altersgruppen den verbreitetsten Grund für mit Behinderung gelebte Lebensjahre darstellen [21]. Depressionen zählen somit zu den bedeutendsten Krankheiten unserer Zeit, mit beträchtlichem Einfluss auf Gesellschaft und Volkswirtschaft [28]. Vor diesem Hintergrund stellt sich die Frage nach der Qualität und Effektivität von Therapieformen.
