Introducción: La maloclusión clase III se considera el tercer problema de salud bucal después de las caries y enfermedad periodontal, según la OMS. La clase III dental se relaciona con el prognatismo mandibular, la cual se caracteriza por una mandíbula con crecimiento vertical. Por esto es necesario hacer un diagnóstico precoz de la erupción de los primeros molares y caninos permanentes, que determinan la llave de la oclusión. Los tratamientos pueden ser interceptivos utilizando ortopedia funcional y correctivos con tratamiento ortodóntico-quirúrgico. Objetivo: Determinar los cambios cefalométricos en pacientes con prognatismo y pseudo prognatismo mandibular pre y postquirúrgicos. Materiales y Métodos: El presente trabajo de investigación tiene un enfoque cualitativo, el tipo de investigación es retrospectivo porque se obtuvo información ya publicada en años anteriores; y transversal porque se recolectaron los datos en un solo periodo haciendo corte en el tiempo. El diseño de investigación es descriptivo-observacional, NO-experimental, porque se realiza sin manipular deliberadamente variables. Es una revisión sistemática en la que se utilizó metabuscadores tales como: Google Scholar, Pubmed, Scielo, Medline, por criterios de inclusión y exclusión quedaron 40 artículos relacionados. Resultados: El pseudoprognatismo mandibular es producto de alteraciones multifactoriales que influyen en el desarrollo dentoesqueletal, que deben corregirse precozmente con ortopedia funcional y orto-quirúrgicamente después de calcificación de centros de crecimiento óseo. El diagnóstico y control postratamiento deben ser clínico, y radiográficos. Conclusión: La desarmonía pseudoclase III tiende a empeorar con el crecimiento, por lo que una intervención ortodóntica temprana es necesaria para evitar un prognatismo verdadero antes que concluya el crecimiento de las suturas esqueletales craneofaciales.
