Giriş: Sürücü davranışları ve sürücülük becerileri, trafikte kaza riskini oluşturan ana unsurlar arasında yer almaktadır. Alkol ve madde etkisi altında araç kullanma davranışı; ölümlere, maddi hasarlara ve yaralanmalara neden olan önemli bir trafik sorunudur. Bu çalışmada; profesyonel araç sürücülerinde alkol, madde ve ilaç kullanımı sıklığının araştırılarak, bu maddeler ile sürücü davranış ve becerileri arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Yöntem: Çalışma İzmir ilinin 4 farklı ilçesinde çalışan 90 profesyonel araç sürücüsü ile gerçekleştirilmiştir. Sürücülere sosyo-demografik ve madde kullanımına ilişkin soruların yer aldığı anket formu, Fagerström Nikotin Bağımlılık Testi (FNBT), CAGE Alkol Tarama Testi, Sürücü Davranışları Ölçeği (SDÖ) ve Sürücülük Becerisi Ölçeği (SBÖ) uygulanmıştır. Veriler istatiksel olarak SPSS 18.0 programı ile analiz edilmiştir (p<0,05 anlamlılık düzeyi). Bulgular: Sürücüler 20-67 yaşları arasında olup yaş ortalaması 42,9±12,7’dir. Sürücülerin %46,7’si 20 yıldan fazla süredir profesyonel araç sürücüsüdür. Sürücülerinçoğunluğunda sigara (%57,8) ve alkol (%63,3) kullanımı vardır. Sürücülerin %55,6’sı trafikte dikkatini arttırmak ve/veya uyanık kalmak amacıyla yaşam boyu en az bir kez ağrı kesici, %12,2’si sakinleştirici ilaç, %1,1’i steroid, %20’si esrar, %3,3’ü amfetamin tipi stimülanlar (ATS), %2,2’si sentetik esrar kullandığını belirtmiştir. Alkol ve esrar kullanımı ile hatalar, kural ihlalleri ve güvenli araç kullanma becerisi puan ortalamaları arasında anlamlı ilişki bulunmuştur. Tartışma ve Sonuç: Alkol, madde ve ilaç etkisi altında araç kullanma davranışının psikolojik, psikiyatrik ve adli toksikolojik bakış açısıyla çok yönlü olarak ele alınması uygulamada fayda sağlayacaktır. Çalışmamızın sonuçları ile işyeri ve trafik ortamında adli kanıt sunan madde testlerinin önemli olduğu ve aktif olarak uygulanması gerektiği görülmüştür.
Alkol ve madde kullanımı toplumu tehdit eden önemli halk sağlığı ve toplum güvenliği sorunlarından biridir. Yüksek bir genç nüfusa sahip olan ülkemizde çocuklar ve ergenler toplumun önemli bir kısmını oluşturmaktadır ve Alkol ve Madde Kullanım Bozuklukları (AMKB) açısından ergenler üzerinde durulması gereken önemli risk gruplarındandır. Yıllar geçtikçe kullanılan maddelere yeni çeşitler eklenmekte ve bu durum özellikle erken yaşlarda madde kullanımını daha cazip hale getirmekte ve genellikle riskli madde kullanım alışkanlıklarına neden olabilmektedir. Bununla beraber madde kullanımı ile ilgili pek çok risk faktörü ve koruyucu faktörler zamanla birlikte değişmektedir. Toplum güvenliği ve halk sağlığı açısından özellikle yasadışı olan bu maddelerin kontrolü ve/veya kullanımı durumunda tanı-izlem amacıyla yapılan madde testleri somut bir veri olarak büyük önem arz etmesinin yanında adli bir delildir. Kullanılan maddelerin saptanmasına yönelik yapılan tıbbi, sosyal veya hukuki işlemlerin bütüncül ve sistematik bir yaklaşımla yapılmasıyla, çocuklarımıza ve gençlerimize adaletli ve sağlıklı bir hizmet sağlanabilmektedir. Bu derleme çalışmasında, çocuk ve ergenlerdeki madde kullanım probleminin ülkemizdeki ve dünyadaki durumu hakkında genel bir profil sunularak, madde kullanım riski ve koruyucu faktörler, çocuk ve ergenlerde madde kullanımı ile mücadele, adli toksikolojik analizler ve bu analizlerde kullanılan biyolojik materyaller ele alınmıştır.
Trafik ortamında farklı şekil ve düzeylerde, içerisinde öfke ve saldırganlık barındıran pek çok olay yaşanmaktadır. Yol kullanıcıları arasında yaşanan bu olaylar, trafik güvenliğini tehdit etmekte ve olumsuz sonuçlara yol açmaktadır. Bu araştırma ile yol kullanıcılarının başlarından geçen “yol şiddeti (road rage)” kavramıyla adlandırabileceğimiz olayların ve bu olayların içerisinde sergilenen ve/veya maruz kalınan davranışların nitel analiz yöntemleri ile incelenmesi amaçlanmıştır. Açık uçlu bir sorunun yer aldığı araştırma linki 81 katılımcı tarafından doldurulmuştur. Katılımcıların yazılı olarak aktardığı öyküler nitel analiz tekniklerinden tematik analiz ve içerik analizi ile kapsamlı bir şekilde incelenmiştir. Tematik analiz sonuçları; olayın nedeni, en yoğun hissedilen duygular ve verilen tepkiler olmak üzere 3 ana tema olduğunu göstermiştir. Her bir ana temanın altında birbirleriyle ilişkili olduğu düşünülen 20 alt tema bulunmuştur. İçerik analizi sonuçları, yol kullanıcılarının en fazla sıklıkta ısrarlı takip deneyimlediklerini göstermiştir. Bulgular, ayrıntılı bir şekilde literatürdeki bilgiler çerçevesinde tartışılmıştır. Son olarak da yol şiddeti olaylarının azaltılması için yapılabilecekler üzerinde durulmuştur.
Sıklıkla sürücüler arasında yol vermeme gibi nedenlerden dolayı trafikte yaşanan sözlü veya fiziksel şiddet, trafik güvenliğini tehdit ederek ciddi boyutlara ulaşmaktadır. Bu durum “road rage (yol şiddeti)” kavramıyla alanyazında karşımıza çıkmaktadır. Yol şiddeti hakkında yapılan bazı araştırmalar, aracında silah bulunduran sürücülerin saldırgan davranışlar sergileme eğilimi açısından daha riskli olabileceğini göstermiştir. Bu davranışlar üzerinde sosyal psikolojide “silah etkisi (weapons effect)” diye anılan kavramın etkili olabileceği öne sürülmüştür. Bu çalışma ile, trafik ortamında sergilenen saldırgan davranışları etkilediği düşünülen faktörlerden silah etkisi üzerinde durularak, bu etki ile yol şiddeti arasındaki ilişkiye dair alanyazında yapılan araştırma bulguları üzerinden bir değerlendirme yapmak amaçlanmıştır. Konu, sosyal psikoloji ve trafik psikolojisi perspektifiyle trafik güvenliği kapsamında ele alınmıştır.
