… Perché i discorsi se li porta via il vento… (detto livornese)L'argomento, di fatto il cuore della attività cardiologica quotidiana valutativa, è estremamente complesso e tutt'altro che definito: a fronte delle numerose, limitate esperienze locali, vi è di fatto una modesta evidenza scientifica a cui ricondursi. Qualche anno fa il Registro Internazionale Multicentrico ROSETTA [1], al fine di stabilire quale valutazione funzionale e a che distanza fosse effettuata dopo una rivascolarizzazione coronarica, aveva raccolto l'esperienza di 13 centri in 5 paesi occidentali relativa a 800 pazienti trattati con angioplastica, in oltre il 50% dei quali effettuata a seguito di una sindrome coronarica acuta (SCA). Il registro aveva evidenziato che solo nel 60% dei pazienti veniva effettuata una valutazione funzionale, prevalentemente di tipo ergometrico; il timing delle valutazioni funzionali era estremamente disperso nei primi 6 mesi, sebbene la maggior parte dei test valutativi fosse stata effettuata precocemente (<1 mese) rispetto all'evento iniziale.Altra importante "non evidenza" documentata nel Registro ROSETTA era l'assenza di relazione tra modalità operative e domanda clinica: in altri termini, i test erano effettuati indipendentemente dai presupposti e dal quesito nel singolo paziente.Cosa suggeriscono le Linee Guida? A complicare ulteriormente l'argomento concorre sicuramente la mancanza di Linee Guida chiare e concordi nelle indicazioni. Per esempio, le Linee Guida AHA/ACC sul test ergometrico [2] danno una indicazione di Classe I nel caso l'esame venga effettuato per la valutazione di pazienti con sintomatologia ischemica ricorrente, e solo di Classe IIB per l'individuazione della restenosi tardiva, limitatamente ad una popolazione ad alto rischio: indicazioni quindi molto precise, selettive e assai pragmatiche. Gli orientamenti della Società Americana di Ecocardiografia [3] ci sembrano invece decisamente lontani da un simile atteggiamento pragmatico: c'è un'indicazione di Classe I per l'eco-stress effettuato ai fini di identificare la restenosi in pazienti con sintomatologia ricorrente atipica, l'indicazione è invece di Classe IIA per la valutazione con eco-stress della restenosi in pazienti con sintomatologia tipica. All'esatto opposto si collocano le indicazioni della Società Americana di Cardiologia Nucleare [4]: in modo ultra-pragmatico, viene riconosciuto il valore clinico (indicazione di Classe IIA) dello stress imaging scintigrafico solo se effettuato tardivamente (>6 mesi) dopo angioplastica in una popolazione ad alto rischio. Le Linee Guida 2005 della Società Europea di Cardiologia dedicate alla angioplastica coronarica [5] non dedicano neppure una riga all'argomento "valutazione funzionale post-PCI". Le opinioni allora ci sembrano molte, discordanti, talora anche abbastanza confuse (o, meno eufemisticamente, tutt'altro che confuse e ben orientate da interessi di bottega).
