Цель. Изучить особенности клинических проявлений и диагностики сочетания туберкулеза органов дыхания (ТОД) с оппортунистическими инфек-циями легких (ОИЛ) у больных на поздних стадиях ВИЧ-инфекции с иммунодефицитом. Материалы и методы. В исследование включены 338 впервые выявленных больных ТОД с выделением Mycobacterium tuberculosis, в возрасте 25–60 лет, со стадией 4В и 5 ВИЧ-инфекции в фазе прогрессирования и без антиретровирусной терапии, разделенных на 2 группы: 169 пациентов с сочетанием ТОД с ОИЛ (основная группа) и 169 – с ТОД (группа сравнения). Всем больным проводилось клинико-лабораторное, лучевое и брон-хологическое исследование, а для выявления ОИЛ – микробиологическое, иммунологическое, молекулярно-генетическое, цитологическое и гисто-логическое исследование диагностического материала респираторного тракта, спинномозговой и плевральной жидкости, крови, мочи и кала. Статистическая обработка данных проводилась с помощью программы Microsoft Office Excel 2010 с вычислением среднего показателя в группе и стандартной ошибки среднего, доверительного интервала. Результаты. Установлено, что туберкулез у больных основной группы и группы сравнения имел генерализованный характер с множественными поражениями, внелегочными проявлениями, что характеризовалось выраженным синдромом интоксикации, бронхолегочными проявлениями и симптомами поражения других органов и систем. Это не позволяет дифференцировать сочетание ТОД с ОИЛ и с ТОД, и их диагностика возможна только при выявлении M. tuberculosis и этиологии возбудителей других ОИЛ. На компьютерной томограмме органов грудной клетки при одновре-менном сочетании нескольких болезней легких визуализируется синдром диссеминации, синдром аденопатии и синдром патологии легочного ри-сунка, представленный уплотнением интерстициальной ткани по типу «матового стекла». В этих случаях не последнюю роль играло одновременное сочетание ТОД с ОИЛ, а также с наслоением поражений легких непосредственно ВИЧ, что не представляло возможным дифференцировать эти патологии. Различие было возможным только при выявлении M. tuberculosis и этиологии возбудителей других ОИЛ. Заключение. На основании проведенного исследования предложен алгоритм последовательной этиологической диагностики сочетания ТОД с ОИЛ у больных на поздних стадиях ВИЧ-инфекции с иммунодефицитом. The aim of this study was to analyze the course of the disease and efficacy of treatment of pulmonary multidrugresistant (MDR) cavitary tubercu losis in antituberculosis settings of Russian penitentiary system. Drug resistant was studied using the method of absolute concentrations. Among 190 males with MBTpositive cavitary pulmonary tuberculosis followedup in antituberculosis hospitals of Russian penitentiary system in Voronezh in 2003–2006, 79 patients (41.6 ± 3.4 %) were yielded MDR strains resistant to the firstline antimucobacterial drugs and 111 patients (58,4 ± 3,4 %) had strains resistant both to the firstline and the secondline drugs (isoniazid and rifampicin). Moreover, MBT strains resistant to cycloserine and quinolones were found in patients who have not been previously treated with these drugs. Both newly diagnosed patients with pulmonary tubercu losis and those with recurrent cavitary pulmonary tuberculosis had a high rate of primary and secondary MDR to the firstline and the secondline antimicobacterial drugs. Administration of I and IIA chemotherapeutic regimens in these patients resulted in occurrence of additional resistance to a wide spectrum of the firstline and the secondline antibiotics. Patients with chronic pulmonary tuberculosis are at high risk of MDR to combi nations of the firstline and the secondline drugs and need other treatment strategies including surgical interventions. Tuberculosis patients with MDR to combinations of the firstline and the secondline drugs were characterized by more severe chronic course of the disease with extended lesions and large cavities in the lungs. Adverse events to the secondline drugs were seen in 42.6 % of the patients with cavitary tuberculosis. Of them, serious adverse events requiring cancellation of the drug and change in chemotherapeutic regimen were noted in 17.9 % of cases.
Клиническое излечение впервые выявленных боль ных деструктивным туберкулезом легких является актуальной проблемой фтизиатрии в виду тяжелого течения заболевания, высокого риска развития мно жественной лекарственной устойчивости (МЛУ) ми кобактерий туберкулеза (МБТ), возможности фор мирования хронического фиброзно кавернозного туберкулеза с высокой эпидемиологической опас ностью для здорового населения [1][2][3][4].Для лечения впервые выявленных больных де структивным туберкулезом легких в соответствии с приказом МЗ РФ № 109 от 21.03.03 [5] и Нацио нальным руководством по фтизиатрии 2007 г. [3] определен I режим химиотерапии (I РХТ), состоя щий из основных противотуберкулезных препаратов: изониазида, рифампицина, пиразинамида, стрепто мицина (этамбутола). При этом клиническое излече ние пациентов, зарегистрированных в 2009 г., соста вило всего 31,5 %, а фиброзно кавернозный туберку лез у них сформировался в 45,5 % случаев [6][7][8].Столь низкая эффективность лечения связана, в первую очередь, с повсеместным использованием оказавшегося неадекватным в РФ I РХТ [2,3,9], применение которого только за 7 лет привело к зна чительному увеличению первичной и вторичной МЛУ МБТ. SummaryA randomized multi center clinical study of chemotherapy efficacy, clinical cure and early recurrence rate involved 240 newly diagnosed patients with cavitary pulmonary tuberculosis treated in municipal and penitentiary medical facilities. Given the primary multiple drug resistance (MDR) of 22.5-33.3 %, intensive chemotherapy with isoniazid, rifampicin, pyrazinamide, ethambutol, kanamycin (or amikacin) and ofloxacin (or lev ofloxacin) allowed to achieve sputum conversion in 100 % of smear positive patients using sputum microscopy and in 89.2 % using sputum culture, cavity closure in 71.7 %, and clinical cure in 94.2 % with early recurrence rate of 2.6 % in comparison to 54.2, 45.8, 45.8, 65 and 20.5 % respectively in patients treated with isoniazid, rifampicin, pyrazinamide and ethambutol. Lung fibrocavitary lesions with high level of MDR developed in 55 % of patients and were associated with high cost of treatment due to the need in expensive salvage chemotherapy regimens and high tech surgical inter ventions. РезюмеБыло проведено рандомизированное многоцентровое клиническое исследование эффективности химиотерапии, клинического излече ния и частоты ранних рецидивов у 240 впервые выявленных больных деструктивным туберкулезом легких в гражданских и пенитенци арных противотуберкулезных учреждениях. Aim. To study the features of clinical and radiological manifestations and diagnosis of COVID-19, respiratory tuberculosis and opportunistic lung infections (OIL) coinfection in patients with late stages of HIV-infection with immunodeficiency. Materials and methods. The study included 29 patients with COVID-19 coinfection, respiratory tuberculosis and opportunistic lung infections in the late stages of HIV-infection with immunodeficiency (group 1) and 29 patients similar in all parameters without COVID-19 (group 2). All patients were underwent clinical and laboratory, radiation and bronchological examination, and microbiological, immunological, molecular genetic, cytological and histological examination of diagnostic material of the respiratory tract, cerebrospinal and pleural fluid, blood, urine and feces to identify pathogens of coinfection. Statistical data processing was carried out using the Microsoft Office Excel 2010 program with the calculation of the average in the group and the standard error of the average, confidence interval. Results. It has been established that co-infection with COVID-19, respiratory tuberculosis and opportunistic lung infections in patients with late stages of HIV-infection with immunodeficiency is manifested by a pronounced intoxication syndrome, bronchopulmonary manifestations and symptoms of damage to other organs and systems, which is due to the generalization of tuberculosis with extrapulmonary lesions and the development of opportunistic lung infections, as in patients without COVID-19. A computed tomogram of the chest organs with this coinfection visualizes the syndrome of dissemination, the syndrome of adenopathy and the syndrome of pathology of the pulmonary pattern, represented by the compaction of interstitial tissue in the frosted glass type, which is associated with the simultaneous layering of various pathologies, which complicates their differential diagnosis. This determines the similarity of clinical and radiological manifestations of COVID-19 coinfection, respiratory tuberculosis and opportunistic lung infections in patients with late-stage HIV-infection with immunodeficiency, as in patients without COVID-19. This requires complex microbiological and molecular genetic research methods to identify specific pathogens for the appointment of timely treatment. Conclusion. Patients with COVID-19 coinfection, respiratory tuberculosis and opportunistic lung infections in the late stages of HIV-infection with immunodeficiency pose a high risk of infection in a healthy population, taking into account their social maladaptation and non-adherence to examination and treatment. This requires an active diagnosis of COVID-19 in all patients with respiratory tuberculosis and HIV-infection who are registered in the office of anti-tuberculosis care for HIV-infected in an anti-tuberculosis dispensary, for emergency isolation and treatment.
