Лебедева, д. 6) Лечение пациентов с тяжелой термической травмой включает терапию сопутствующих заболеваний и осложнений, среди которых особую роль играют поражения центральной нервной системы. Их разви тие в значительной степени обусловливает утяжеление состояния, ухудшает прогноз заболевания, а так же отдаляет сроки выздоровления. Цель -определить основные патогенетические механизмы невроло гических осложнений и их коррекцию у обожженных. В генезе неврологических нарушений у обожженных основную роль составляют гемодинамические нарушения, электролитные расстройства, нарушение гематоэнцефалического барьера на фоне интоксикации. Развивается сложный комплекс нарушений, результатом которого могут быть вторичный гнойный менингит и менингоэнцефалит. Проведение ком плексного лечения, включающего хирургическое восстановление кожного покрова, инфузионную тера пию, антибиотики, препараты нейропротекторного ряда, позволяет избежать ряда грозных осложнений. Своевременное патогенетически обоснованное лечение неврологических осложнений у обожженных, проводимое в рамках комплексной терапии ожоговой болезни, является крайне необходимым, посколь ку именно такой подход способствует формированию наилучших исходов.Ключевые слова: травма, ожог, ожоговая болезнь, ожоговая энцефалопатия, эндотоксикоз, гипоксия.Лечение тяжелообожженных сопряжено с трудностями, которые возникают вслед ствие часто встречающегося при ожоговой болезни комбинированного поражения раз личных органов и систем. Среди пациентов отделения анестезиологии, реанимации и интенсивной терапии клиники термических поражений значительное количество состав ляют пострадавшие с формированием цереб ральных неврологических осложнений. Их развитие в значительной степени обуслав ливает утяжеление состояния, ухудшает про гноз заболевания, а также отдаляет сроки вы здоровления [5,6,28].Патогенез неврологических нарушений у обожженных определяется особенностями протекания термической травмы и ожоговой болезни в частности [34]. Ведущее значение имеет ожоговая интоксикация, которая нарас тает с увеличением площади и глубины ожога. Ее возникновение, а также степень выражен ности определяются как резорбцией токси ческих продуктов распада тканей из области ожога, так и каскадным нарастанием в крови и интактных тканях токсических метаболитов. Кроме того, существенное значение играет внедрение бактериальных агентов, оказыва ющих дополнительное инфекционное токси ческое воздействие на организм пострадав шего. Если концентрация эндотоксинов не превышает обезвреживающие возможности естественных детоксикационных систем ор ганизма, то клиническая манифестация син дрома эндотоксикоза отсутствует.
