Prawo do informacji o własnym stanie zdrowia, w tym o niekorzystnym rokowaniu, wynika z zapisów Ustawy o prawach pacjenta i Rzeczniku Praw Pacjenta oraz Kodeksu Etyki Lekarskiej. Odpowiednie narzędzia, takie jak protokół SPIKES, mogą wspierać lekarzy w przekazywaniu niekorzystnego rokowania. Celem niniejszej pracy było zbadanie sposobu, w jaki lekarze pracujący w podstawowej opiece zdrowotnej (POZ) komunikują swoim pacjentom i ich bliskim informacje o niekorzystnym rokowaniu. Spośród 744 zbadanych lekarzy podstawowej opieki zdrowotnej 72,2% deklaruje, iż porusza kwestię śmierci lub niekorzystnego rokowania w rozmowie z pacjentem na jego bezpośrednią prośbę, samodzielnie zaś informacje przekazuje 63,3%. Jednocześnie zwraca uwagę fakt, iż 15,1% lekarzy czyni to bez zgody chorego pod jego nieobecność, a 40,2% badanych wskazało, iż przekazuje informacje o niekorzystnym rokowaniu rodzinie za "domniemaną zgodą chorego". W badaniu podkreśla się, iż w wyodrębnionej grupie lekarzy POZ mających jednocześnie specjalizację z medycyny paliatywnej samodzielność w zakresie podejmowania rozmowy o niekorzystnym rokowaniu deklaruje 100% badanych. Aby wspomóc lekarzy POZ w realizowaniu prawa pacjenta do informacji o stanie zdrowia, stworzono narzędza kompetencyjne w zakresie przekazywania niekorzystnego rokowania. Do komunikowania w sposób otwarty niekorzystnego rokowania można wykorzystać wypracowaną relację z pacjentem i jego bliskimi. Pomocne może być ustawiczne szkolenie, w tym w zakresie kompetencji psychologicznych, z jednoczesnym psychologicznym wsparciem lekarzy POZ oraz wdrażanie odpowiednich protokołów pracy.
