Introducción: El tratamiento del queratoquiste, previamente conocido como tumor odontogénico queratoquístico, sigue siendo controversial. El queratoquiste es una lesión odontogénica que probablemente deriva de la lámina dental, de manera frecuente se localiza en la parte posterior del hueso mandibular y, seguramente, representa a la lesión antes denominada quiste primordial. El queratoquiste se considera una lesión benigna de comportamiento biológico agresivo. Una amplia variedad de técnicas se han reportado para su tratamiento, tales como la enucleación, la marsupialización y la resección en bloque, así como la combinación de varios métodos. En general, la resección quirúrgica se considera como la última opción para su tratamiento. Objetivo: Presentar el reporte de un caso de una paciente de 36 años con diagnóstico de queratoquiste odontogénico asociado a un proceso inflamatorio crónico. Material y métodos: El tratamiento de elección fue una descompresión de la lesión usando un tubo tipo Penrose, seguida de irrigación con clorhexidina al 2%. Resultados: Durante el seguimiento se observó una reducción de aproximadamente 80% del tamaño original del quiste. De igual manera, la cavidad aceptaba menos cantidad de irrigación; por último, se realizó una biopsia excisional junto con osteotomía periférica. La biopsia reveló un queratoquiste odontogénico con metaplasia escamosa. Discusión: En el presente caso el tratamiento conservador utilizado resultó exitoso; sin embargo, para este tipo de pacientes el seguimiento a largo plazo es recomendable. Además, la asociación con el síndrome del carcinoma basocelular debe considerarse en pacientes con lesiones extensas o múltiples. Conclusiones: El abordaje quirúrgico inicial dirigido a pacientes con tumores odontogénicos agresivos puede tener efectos importantes en la recuperación de los pacientes afectados y en su calidad de vida. Por lo tanto, un Revista Odontológica Mexicana 2020; 24 (2): 00-133 adecuado plan de tratamiento requiere de la valoración multidisciplinaria de cada caso.
El neuroma traumático no es una verdadera neoplasia sino una proliferación reactiva del tejido neural después de la transección u otro daño de un haz de nervios. Pueden desarrollarse en cualquier lugar, pero son más comunes en el área del foramen mentoniano, la lengua y el labio inferior. La formación de neuromas traumáticos es una causa importante de dolor neuropático, que sigue siendo un problema difícil al que se enfrentan los cirujanos. El mecanismo exacto del dolor asociado al neuroma aún no se comprende completamente y la prevención de su formación es primordial para evitarlo. Hasta la fecha el tratamiento más efectivo ha sido la extirpación quirúrgica. Se reporta el caso de un paciente de sexo femenino de 23 años de edad que presenta un aumento de volumen en el fondo de surco vestibular a nivel de los dientes 44 y 45, así como dolor espontáneo de tipo ardoroso con una evolución de aproximadamente cinco meses. Se decide realizar la resección quirúrgica de la lesión y el resultado histopatológico fue de un neuroma traumático. A los 7 días de seguimiento, se observa un ligero edema en la región bucal y mentoniana; así como anestesia a nivel de la piel del mentón y labio inferior. A 10 meses de seguimiento persistió la anestesia y no se observaron datos de recidiva.

Introducción: La mucormicosis es una infección producida por hongos aerobios saprofitos oportunistas de distintos géneros, pertenecientes al orden de los Mucorales, patógenos que, bajo condiciones de inmunocompromiso como diabetes y SIDA, producen enfermedades. Existen varias formas de presentación clínica: rinocerebral, pulmonar, mucocutańea, gastrointestinal y diseminada. La presentación rinocerebral es frecuentemente fatal, con una morbimortalidad del 85-100 %, a pesar del uso de la anfotericina B, terapia hiperbárica y resección quirúrgica agresiva. Es primordial el diagnóstico temprano y el soporte sistémico del paciente. La disponibilidad limitada de tratamientos contemporáneos constituye una barrera para los pacientes en entornos de países subdesarrollados. Materiales y métodos:El objetivo es realizar una breve revisión de la literatura acerca de la mucormicosis rino-órbito-cerebral, su incidencia y mortalidad en un hospital de tercer nivel de México.Resultados: En un periodo de 3 años (julio de 2018-julio de 2021) se trataron tres pacientes con diagnóstico de mucormicosis rino-órbito-cerebral. Los tres casos revisados presentaron diagnóstico de diabetes mellitus tipo 2 con un mal control de la enfermedad. Dos de los tres pacientes atendidos fallecieron durante su episodio de hospitalización a pesar del manejo multidisciplinario que incluyó soporte multiorgánico y tratamiento quirúrgico radical en todos los casos.Conclusiones: La mucormicosis sigue presentando una alta mortalidad en los países en desarrollo, a pesar del avance en los tratamientos médicos y quirúrgicos. El factor pronóstico más importante sigue siendo la detección temprana de la enfermedad y el adecuado tratamiento de las enfermedades asociadas.
