SummaryAim– To develop predictive models to allocate patients into frequent and low service users groups within the Italian Community-based Mental Health Services (CMHSs). To allocate frequent users to different packages of care, identifing the costs of these packages.Methods– Socio-demographic and clinical data and GAF scores at baseline were collected for 1250 users attending five CMHSs. All psychiatric contacts made by these patients during six months were recorded. A logistic regression identified frequent service users predictive variables. Multinomial logistic regression identified variables able to predict the most appropriate package of care. A cost function was utilised to estimate costs.Results– Frequent service users were 49%, using nearly 90% of all contacts. The model classified correctly 80% of users in the frequent and low users groups. Three packages of care were identified: Basic Community Treatment (4,133 Euro per six months); Intensive Community Treatment (6,180 Euro) and Rehabilitative Community Treatment (11,984 Euro) for 83%, 6% and 11% of frequent service users respectively. The model was found to be accurate for 85% of users.Conclusion– It is possible to develop predictive models to identify frequent service users and to assign them to pre-defined packages of care, and to use these models to inform the funding of psychiatric care.
SummaryAims – To obtain a new, well-balanced mental health funding system, through the creation of i) a list of psychiatric interventions provided by Italian Community-based Psychiatric Services (CPS), and associated costs; ii) a new prospective funding system for patients with a high use of resources, based on packages of care. Methods – Five Italian Community-based Psychiatric Services collected data from 1250 patients during October 2002. Socio-demographical and clinical characteristics and GAF scores were collected at baseline. All psychiatric contacts during the following six months were registered and categorised into 24 service contact types. Using elasticity equation and contact characteristics, we estimate the costs of care. Cluster analysis techniques identified packages of care. Logistic regression defined predictive variables of high use patients. Multinomial Logistic Model assigned each patient to a package of care. Results – The sample's socio-demographic characteristics are similar, but variations exist between the different CPS. Patients were then divided into two groups, and the group with the highest use of resources was divided into three smaller groups, based on number and type of services provided. Conclusions – Our findings show how is possible to develop a cost predictive model to assign patients with a high use of resources to a group that can provide the right level of care. For these patients it might be possible to apply a prospective per-capita funding system based on packages of care.Declaration of Interest: None
SUMMARYAims— To investigate the prevalence of psychotherapy intervention in five Italian Centres for Mental Health. Analysing sociodemographic characteristics, geographical differences, frequencies of psychotherapy interventions, and the costs of these interventions.Methods— Five Italian Community—based Psychiatric Services collected data from 1250 patients during October 2002. Socio—demographic and clinical characteristics and GAF scores were collected at baseline. All psychiatric contacts during the following six months were recorded and categorised into 24 service contact categories. A comparison between patients who received or did not receive psychotherapy was made.Results— The socio—demographic characteristics of patients are very similar to those found in previous studies; there is a great difference in the number of psychotherapy treatments between the five areas. Psychotherapy patients use different services compared to the other patients, and their care cost less.Conclusions— This study provides a description of the provision of psychotherapy by Italian Mental Health Services. RIASSUNTOScopo – Mettere a confronto la prevalenza di stati di disagio psichico in quattro gruppi di giovani di diversa condizione sociale (operai metalmeccanici esposti e non esposti a rischi occupazionali per la salute mentale, disoccupati o in cerca di prima occupazione e studenti a tempo pieno); lo studio inoltre valuta tra gli stessi gruppi, a distanza di sei anni, i cambiamenti di stato e le variazioni del disagio psichico. Disegno – I soggetti, selezionati attraverso accurate procedure di randomizzazione, sono maschi, in età compresa tra 20 e 29 anni, residenti da almeno 10 anni nella stessa area geografica e ancora conviventi con il nucleo famigliare originario. Sono stati reclutati 157 giovani, suddivisi nei seguenti gruppi: metalmeccanici esposti a rischi occupazionali specifici per la salute mentale (n° = 30); metalmeccanici non esposti (n° = 57); disoccupati (n° = 40); studenti a tempo pieno (n° = 30).La prevalenza degli stati di disagio psichico è stata confrontata nei quattro gruppi; a distanza di sei anni, sono stati valutati, nei 116 soggetti che hanno partecipato allo studio di follow-up, i cambiamenti di status e i livelli di disagio psichico.Setting – L'indagine è stata condotta a Cento (Ferrara), una zona economicamente ricca e con tassi di disoccupazione molto bassi. Le interviste sono state effettuate nel 1987 e nel 1993; i giovani, dopo essere stati contattati telefonicamente, sono stati intervistati nella propria abitazione, sempre dallo stesso psicologo. Principali misure utilizzate – Nel 1987 a ciascun soggetto sono stati somministrati una scheda «Posizione sociale e salute» (PSS) e un questionario, precedentemente validato, per la determinazione dell'indice di disagio psichico (IDP).Inoltre gli operai metalmeccanici, mediante il «Questionario di rischio lavorativo» (QRL), sono stati suddivisi nei due gruppi «a rischio» e «non a rischio» occupazionale per la salute mentale.Nel 1993, nello studio di follow-up, è stato nuovamente somministrato il questionario per la rilevazione dell'IDP ed è stata fatta compilare una scheda (Scheda per la posizione sociale e occupazionale) per raccogliere dettagliate informazioni sui cambiamenti lavorativi o di stato sociale intervenuti nei sei anni precedenti.Risultati – Nel 1987 si sono riscontrate situazioni di disagio psichico significativamente più frequenti e di maggior rilevanza negli operai metalmeccanici esposti a rischi occupazionali per la salute mentale (mentre nulla di significativo è emerso negli altri gruppi, in quello dei disoccupati in particolare). Dallo studio di follow-up si sono potuti rilevare miglioramenti significativi, per quanto riguarda il disagio psichico, negli operai metalmeccanici che sono passati dal lavoro manuale ad un altro tipo di impiego; si è inoltre riscontrata la tendenza, nei giovani disoccupati che hanno trovato, nei corso dei sei anni, un lavoro fisso, ad un peggioramento del grado di disagio psichico.Conclusioni – L'aspetto piu interessante della nostra ricerca sembra essere rappresentato dall'analisi delle relazioni tra prospettive occupazionali e salute mentale di giovani che vivono in un'area economicamente forte. Ulteriori ricerche dovranno tuttavia approfondire le problematiche specifiche connesse con le possibili prospettive occupazionali di studenti e giovani disoccupati e/o in cerca di prima occupazione.
The results of our study allow to demonstrate that there are cost differences between the diagnosis (direct costs are highest for schizophrenic patients and lowest for those with neurotic disorders) and that the costs evaluation can be used to ensure appropriate provisions to Mental Health Department for support and treatment of a wide range of psychiatric disorders.
