HuR mediated post-transcriptional upregulation of COX-2 and HO-1 expression after GEM treatment in pancreatic cancer cells. HuR silencing significantly increased the effectiveness of GEM treatment in vitro.
Decreased activity of CUGBP2 could be associated with high chemoresistance and early dissemination of pancreatic cancer through the HO-1- and COX-2-mediated cytoprotective and carcinogenesis pathways. Curcumin significantly increased the effectiveness of GEM treatment in vitro via the CUGBP2-mediated post-transcriptional regulation pathway.
Background: It is supposed that a prolonged lifetime will be associated with increased incidence of PDAC among the elderly. Some studies show a tendency toward decreased survival in the elderly patients following pancreatoduodenectomy for PDAC. The aim of this study was to evaluate factors, influencing survival following pancreatoduodenectomy for PDAC in different age groups. Methods: Data of 251 patients after pancreatoduodenectomy for PDAC between 1999 and 2012 were analyzed. The Kaplan-Meier method and log-rank test were used to calculate survival and to compare differences between groups. The Cox proportional hazard model was applied to indentify independent prognosticators. Results: The overall median survival was 14.9 months. Postoperative morbidity was 25.5% with a 5.1% mortality rate. No significant differences in the overall morbidity (22.4 vs. 29.6%) or mortality (2.8 vs. 8.3%) rates were observed between different patients' age groups (<70 years and >70 years). Multivariate analysis revealed R1 resection (HR 1.76) and poor tumor differentiation (G3-G4) (HR 1.48) were independent negative factors for survival in patients <70 years. Lymph-node metastases (N1) - HR 4.89 and perineural invasion - HR 2.73 were independent prognosticators in the elderly. Conclusions: Our study highlighted different factors influencing long-term survival after pancreatoduodenectomy: R1 resection and poor tumor differentiation (G3-G4) were independent negative factors for survival in patients <70 years, while perineural invasion and lymph-node metastases result in worse survival among the elderly.

Vienas svarbiausių baltymų apoptozės mechanizme yra apoptozės proteazes aktyvinantis veiksnys APAF-1. Duomenų apie APAF-1 geno raišką ir jos reikšmę kasos vėžio atveju nėra. Šio darbo tikslas nustatyti APAF-1 geno raišką kasos vėžiniame audinyje ir jį supančiame sveikame audinyje bei susieti su klinikinėmis ir morfologinėmis ligos ypatybėmis. APAF-1 geno raiškos tyrimas atliktas realaus laiko polimerazės grandininės reakcijos metodu iRNR lygmenyje. Tyrime dalyvavo 29 pacientai. Nustatyta, kad APAF-1 geno raiška navikiniame audinyje sumažėjusi 82,8 proc. (n=24) atvejų. APAF-1 geno raiška reikšmingai (p= 0,02) skyrėsi tarp navikinio ir sveiko audinio. Sąsajų su klinikinėmis ir morfologinėmis kasos vėžio ypatybėmis nenustatyta. Šiuo tyrimu mes parodėme, kad proapoptozinio geno APAF-1 raiška kinta kasos vėžio atveju ir gali būti susijusi su šios ligos patogeneze. ĮvadasKasos vėžys -blogiausią prognozę turintis virškinimo organų piktybinis navikas. Jis užima 7-ą vietą tarp vyrų ir moterų mirčių pasaulyje, susijusių su piktybiniais navikais [1]. Penkerių metų išgyvenamumas po radikalaus chirurginio gydymo siekia vos 17-20 proc., tuo tarpu 5-erių metų išgy venamumas esant pažengusiai ligos formai yra apie 2 proc. 2008 metų GLOBOCAN duomenimis, bendras penkerių metų išgyvenamumas, sergant kasos vėžiu, pasaulyje nesiekia 1 proc. Mirtingumas nuo šios ligos taip pat išlieka vienas didžiausių tarp virškinimo organų navikų [2]. Asme nų, sergančių kasos vėžiu, bendras vienerių metų išgyvenamumas siekia 23,7 proc., tačiau dramatiškai sumažėja per sekančius ketverius metus ir tesiekia 6,3 proc.Trumpas išgyvenamumas dažniausiai siejamas su ligos iš-plitimo laipsniu diagnozės nustatymo metu, nes apie 80 proc. atvejų kasos vėžys būna išplitęs lokaliai arba jau yra atokiosios metastazės. Trumpas kasos vėžiu sergančių pacientų išgyvenamumas skatina ieškoti naujų efektyvių diagnostikos ir gydymo metodų.Apoptozė arba programuota ląstelių mirtis yra fiziologinis genetiškai reguliuojamas ląstelių žūties mechanizmas, būtinas audinių homeostazei palaikyti. Vienas svarbiausių baltymų apoptozės mechanizme yra apoptozės proteazes aktyvinantis veiksnys APAF-1, kuris dalyvauja reguliuojant ląstelių žūtį per vidinį apoptozės mechanizmą, susijusį su mitochondrijomis [3]. APAF-1 baltymas -neaktyvus ir uždaroje konformacijoje būna tol, kol neatsiranda apoptozės signalas, t.y. ląstelės stresiniai veiksniai (DNR pažaida, onkogenų aktyvacija ir kt.) [4]. Atsipalaidavęs citochromas C iš mitochondrijų ir dalyvaujant dATP/ ATP sukelia APAF-1 molekulių oligomerinio komplekso susidarymą. Septynios APAF-1 molekulės suformuoja apoptosomą, kuri jungiasi su prokaspaze-9 ir ją aktyvuoja, paleidžiant tolimesnę kaspazių aktyvavimo kaskadą, kuri pasibaigia ląstelės apoptoze [5].
