Patients with atrial fibrillation are faced with an increased risk of thromboembolic events, myocardial infarction, chronic heart failure and death. For some patients with atrial fibrillation, direct current cardioversion (DCCV) is a strategy that can be used to reacquire sinus rhythm. Our aim was to analyse the most commonly used medications after an electrical cardioversion, the reasons for not using them, the effects of pharmacotherapy on recurrence rates, and compare results with data from studies in 2014. The prospective study includes patients with electrocardiographically confirmed atrial fibrillation who underwent direct current cardioversion, hospitalised at Pauls Stradiņš Clinical University Hospital (Rīga, Latvia). The average age was 64.6 years. 50% of the patients were female. During the six-month study period, 14.3% patients were using amiodarone, 8.3% patients were on etacizine, 7.1% received propafenone, and 57.1% used beta blockers in monotherapy or in combination. Warfarin was used in 28.0% patients, direct oral anticoagulants (DOAC’s) in 29.9%, 21,4% of patients received aspirin and 16.7% did not use any antithrombotic therapy. Comparing the recurrence rate in patients using different antiarrhythmic drugs, amiodarone showed a statistically significant superiority compared to etacizine and propafenone (p = 0.02). The obtained data showed that over four years, the use of anticoagulants increased by 11.6%.
Fibrilace síní (FS) je celosvětově nejčastější formou arytmie, která postihuje přibližně 33 milionů jedinců. Antikoagulanciem volby v prevenci cévní mozkové příhody u jedinců s FS je již dlouho warfarin. Několik velkých studií prokázalo, že nová (přímá) perorální antikoagulancia (non-vitamin K antagonist oral anticoagulant, NOAC) mají z hlediska bezpečnosti a účinnosti řadu předností, jako například fi xní dávku a předvídatelnější farmakokinetiku. Metody: Naše studie, provedená v lotyšském kardiocentru Fakultní nemocnice Pauls Stradins (Pauls Stradins Clinical University Hospital, Latvian Centre of Cardiology), probíhala od října 2015 do června 2017. Do studie bylo zařazeno celkem 356 pacientů, u nichž byla provedena elektrická kardioverze (EKV). Výsledky: Jeden rok po EKV užívalo 27,5 % pacientů warfarin, 33,7 % pacientů NOAC, zatímco 38,8 % jich neužívalo žádná perorální antikoagulancia. Devět (2,5 %) pacientů, kteří se studie zúčastnili, v následujícím roce zemřelo. Celkem u osmi pacientů (2,2 %) došlo k významnému krvácení a tři pacienti (0,8 %) prodělali nefatální ischemickou cévní mozkovou příhodu. Incidence nefatální ischemické cévní mozkové příhody u pacientů užívajících warfarin byla 2 %, přičemž u pacientů užívajících NOAC nebyla žádná taková příhoda zaznamenána (p = 0,20). Incidence významného krvácení byla 5,1 % versus 2,5 % ve skupinách s warfarinem, resp. NOAC (p = 0,12). Závěry: Ve srovnání s podobnými studiemi prokázala naše studie nízkou incidenci ischemických cévních mozkových příhod a významného krvácení i nízkou celkovou mortalitu. Jeden rok po EKV se užívání warfarinu snížilo o 2,8 % (p = 0,63) a v případě rivaroxabanu a dabigatranu o 17,5 % (p = 0,001), resp. 17,4 % (p = 0,001). Naše studie tak prokázala, že NOAC představují bezpečnou a účinnou alternativu k warfarinu.
A reproducible model of platelet rich thrombi was produced in canine arteries by a previously described technique of intravascular insertion of metal tubing. The effect of platelet suppressants including sulfinpyrazone and ticrynafen, a clofibrate analog, on the rate and nature of thrombus formation was studied. ADP and epinephrine induced platelet aggregation of the animal's platelet rich plasma was monitored during the experiment. Platelet aggregation was impaired after the intravenous administration of either sulfinpyrazone (30 mg/kg body weight) or ticrynafen (100 mg/kg body weight). The rate of thrombus formation was not significantly altered by sulfinpyrazone when compared to the control, but histochemically, the thrombus studied showed a greatly diminished platelet content. In contrast, ticrynafen was able to prevent a complete occlusion of the involved vascular segment and a weak thrombus composed mostly of fibrin was produced instead. This effect of ticrynafen was dose related with minimal changes observed at a dose of 75 mg/kg. The results suggest that this arterial thrombosis model is of value in evaluating antithrombotic agents. They also indicate a potential value of ticrynafen as a therapeutic agent which should be further studied.
