Resumen: Introducción. Las heridas por armas de fuego son poco frecuentes en nuestro medio, de ahí que la experiencia en el manejo de las mismas sea limitada. En este artículo mostramos la experiencia de nuestro Servicio en el tratamiento de estas lesiones y realizamos una revisión del tema para intentar protocolizar la actuación inicial ante este tipo de pacientes. Material y Método. Presentamos seis casos tratados en nuestro Servicio durante el año 2002. Estudiamos las características demográficas, etiología, patrón de lesiones y tratamiento recibido. Resultados. Cinco hombres y una mujer presentaron heridas por arma de fuego durante este período, con una edad media de 38 años (Rango 13-74). La etiología más frecuente fue la agresión, seguido del intento de autolisis. Sólo un paciente requirió estabilización urgente de vía aérea mediante traqueotomía. No existieron complicaciones postoperatorias y sólo un enfermo requirió intervenciones secundarias por secuelas. Discusión. El aspecto más controvertido en el tratamiento de estas lesiones es el manejo quirúrgico de pacientes con lesiones extensas de la cara que involucran partes blandas y hueso, ya que pueden tratarse con técnicas reconstructivas complejas en el primer acto operatorio o, por el contrario, esta reconstrucción se puede realizar de forma diferida tras una primera fase de tratamiento quirúrgico de desbridamiento, estabilización de fracturas y cierre de partes blandas con técnicas simples. Pensamos que la elección del tipo de tratamiento debe ser individualizada en cada caso, aunque solemos optar por una reconstrucción diferida de los defectos tan pronto como sea posible. IntroducciónLos traumatismos faciales provocados por armas de fuego en nuestro medio son raros, probablemente debido a la legislación sobre posesión de armas de fuego vigente en nuestro país. Esta falta de experiencia unido a las características especiales de estas lesiones, que requieren en muchas ocasiones el empleo de técnicas reconstructivas microquirúrgicas para su tratamiento definitivo, hacen que este tipo de pacientes constituyan un reto para el cirujano oral y maxilofacial.La severidad de las lesiones por arma de fuego depende de dos variables fundamentalmente. La primera variable se debe al calibre del proyectil y a la velocidad con la que se dispara, que son dependientes del arma utilizada. La segunda variable relacionada es la distancia a la que se realiza el disparo, que fue clasificada por Sherman y Parrish en tres tipos: tipo I, aquellos realizados más allá de 6,6 metros; tipo II entre 6,6 y 2,74 metros; y tipo III a menos de 2,74 metros. 1 Como veremos posteriormente, estas lesiones se pueden clasificar de forma básica en dos grandes grupos: las de alto y las de bajo grado de energía. 2 Además, hay que tener en cuenta la región donde se produce, ya que las lesiones en el territorio maxilofacial pueden tener importantes secuelas estéticas y funcionales, o lo que es más grave, provocar la muerte del paciente. Resumen: Introducción. La cirugía reconstructiva tras la exéresis de tumores malignos que afectan a la lengua y al suelo de boca, continúa siendo uno de los grandes retos de la cirugía oral y maxilofacial. La mayor recuperación del habla y de la deglución, van a ser los objetivos finales de toda la reconstrucción de la cavidad oral. Presentamos un nuevo diseño del colgajo libre radial para la cirugía reconstructiva del carcinoma lengua y suelo de boca. Material y método. Nuestra serie consta de tres pacientes diagnosticados de carcinoma epidermoide de lengua y suelo de boca. Tras la cirugía oncológica ablativa, se reconstruyó el defecto oral con un colgajo libre radial de doble paleta cutánea con zona intermedia desepidermizada microvascularizado. Resultados. La movilidad de la lengua de nuestros pacientes fue suficiente y adecuada, consiguiéndose una buena calidad de vida. No hubo pérdida de la comida u otros fluidos a través del tracto respiratorio. Conclusiones. Con el colgajo radial de doble paleta cutánea microvascularizado, se consigue una buena recuperación funcional tanto en el habla como en la deglución en los pacientes, por lo que creemos que debería estandarizarse su uso en la cirugía reconstructiva oral tras la exé-resis de tumores malignos de lengua y suelo de boca. IntroducciónLa cirugía reconstructiva tras la resección de tumores de cabeza y cuello ha experimentado un gran avance y perfeccionamiento en los últimos años, cambiándose los objetivos de dicha cirugía, que van desde el cierre directo en un comienzo, hasta las formas más complejas de restauración en la actualidad, y que permiten restituir la estructura anatómica y funciones de las partes resecadas. 1 El principal objetivo de toda reconstrucción es conseguir la máxima recuperación funcional del paciente, siendo la reconstrucción de la lengua el factor más importante en dicha recuperación. Sin embargo, la complejidad en la disposición de las fibras musculares linguales y el extraordinario rango de movimientos de las mismas aún no se ha podido alcanzar con las técnicas reconstructivas actuales. Lo que sí se sabe es que, tras la glosectomía parcial, es primordial mantener la movilidad de la lengua residual mientras se restaura el revestimiento mucoso oral. Los colgajos libres inervados se han propuesto como la otra forma de garantizar un alto grado de recuperación de la deglución, habla y protección de la vía aérea en la reconstrucción de la lengua.Dentro de los colgajos libres utilizados en la reconstrucción lingual, el colgajo libre radial microvascularizado, popularizado por Soutar, 2 se ha convertido en el de elección, no sólo para la reconstrucción lingual, sino, en general, para los tejidos blandos de la cavidad oral y faringe. 3,-6 . Una mujer de 60 años, sin antecedentes médicos de interés, edéntula parcial, fue remitida a nuestro servicio por su odontólogo tras el hallazgo casual en una radiografía panorámica de una lesión radiolúcida, intraósea y redondeada, de un tamaño aproximado de 3 x 2 cm, con bordes bien definidos, en el ángulo mandibular derecho y en relación con la corona de un tercer molar inferior incluido (Fig 1A). La lesión respetaba y desplazaba el canal del nervio dentario inferior (Fig 1B).Clínicamente la paciente se encontraba asintomática. A la exploración se detectó un abombamiento de ambas corticales óseas, con una mucosa de aspecto normal. La sensibilidad del nervio dentario inferior no estaba alterada. El diagnóstico diferencial incluyó en primer lugar el quiste folicular o dentíge-ro, por su relación al tercer molar inferior incluido; y en segundo lugar, queratoquiste, quiste lateral periodontal, quiste botrioide odontogénico y ameloblastoma como lesión agresiva intraósea que cause osteolisis.
Paciente mujer de 51 años de edad que acude a urgencias por un cuadro de seis meses de evolución de obstrucción nasal. No presenta ningún antecedente personal de interés exceptuando un cuadro de algias faciales de repetición, compatible con cuadro de sinusitis, de carácter esporádico y que ceden con la analgesia habitual. Se le realiza de forma rutinaria una radiografía simple de cara en proyección anteroposterior y lateral (Fig. 1) donde se objetiva una lesión centrofacial de densidad osteocondensante. Tras el hallazgo se cita en consulta para proseguir con las pruebas complementarias que lleven al diagnóstico de la misma. Se le realiza una tomografía axial computerizada (Fig. 2) así como una resonancia magnética con cortes axiales y coronales (Fig. 3) observándose esta tumoración a nivel etmoidal derecho que invade fosas nasales y oblitera el ostium de drenaje del seno maxilar derecho y que se acompaña de otra lesión de consistencia blanda en el interior de dicho seno. El estudio previo a la intervención quirúrgica se completó con una rinoscopia. ¿Cuál es su diagnóstico y abordaje?
