Résumé. - L'auteur étudie le rôle joué par la départementalisation (1946) dans l'évolution sanitaire et sociale de l'île de la Réunion. Il part de la situation initiale, très mauvaise, qui s'expliquait par l'environnement tropical et le passé colonial. Il montre ensuite les effets positifs de la départementalisation sur l'équipement de l'île, notamment sanitaire, et ses lacunes. La deuxième partie est consacrée à l'état sanitaire actuel, et à l'analyse de l'évolution de la courbe de la mortalité. Les causes de mortalité (en particulier de mortalité infantile), sont passées de celles d'un milieu tropical sous développé à celles d'un pays évolué, au prix d'une aide financière de la métropole très importante.
