Sarcoidosis is a multisystemic granulomatous disease of unknown etiology that primarily affects adults between the ages of 20 and 40 years old. It is characterized by the activation of Th1 lymphocytes resulting in the production of inflammatory cytokines and the formation of noncaseating epithelioid cell granulomas in affected tissues. The lungs and lymphatic system are the ones most frequently affected. The disease usually presents spontaneous remission in the first two years and, in a few patients, the disease progresses to pulmonary fibrosis or other fatal complications depending on the affected organ. The pathogenesis of sarcoidosis is still not clearly defined, and is considered an interaction between the environment and risk alleles in many genes.The present case control study consisted of 146 Greek patients with sarcoidosis and 90 healthy volunteers from the same ethnic group. The coding and neighboring intronic regions of the BTNL2 gene were sequenced and risk alleles were compared amongst the two groups. Thirty-seven different variants were detected from which 12 were synonymous substitutions and 25 non-synonymous. With the help of in silico tools (SIFT, PolyPhen, PROVEAN, PMut and EX_SKIP), 13 variants were classified as possible pathological risk variants including 4 novel ones. The most common risk variants contributing to phenotypic modulation of sarcoidosis were p.S360G and p.S334L, with the latter contributing to a more severe disease stage with extra-pulmonary manifestations such as skin granulomas and relapses being more common.
Η σαρκοείδωση είναι μία πολυσυστηματική κοκκιωματώδης νόσος αγνώστου αιτιολογίας που προσβάλλει κυρίως άτομα ηλικίας 20 έως 40 ετών. Χαρακτηρίζεται από την ενεργοποίηση των Th1 λεμφοκυττάρων με αποτέλεσμα την παραγωγή φλεγμονωδών κυτταροκινών και τον σχηματισμό μη τυροειδοποιημένου κοκκιώματος στους προσβεβλημένους ιστούς.Συχνότερα προσβάλλεται ο πνεύμονας και το λεμφικό σύστημα αλλά και οποιοδήποτε άλλο όργανο και παρουσιάζει αυτόματη ύφεση τα δύο πρώτα έτη σε ποσοστό που διαφέρει ανάλογα με το ακτινολογικό στάδιο της νόσου ενώ σε κάποιους ασθενείς εξελίσσεται σε πνευμονική ίνωση ή σε άλλες θανατηφόρες επιπλοκές ανάλογα με το προσβεβλημένο όργανο.Η διάγνωση βασίζεται στα κλινικοακτινολογικά ευρήματα και στην ιστολογική επιβεβαίωση με την ανεύρεση του μη τυροειδοποιημένου επιθηλιοειδούς κοκκιώματος σε προσβεβλημένο όργανo, καθώς και στον αποκλεισμό άλλων κοκκιωματωδών νοσημάτων.Η παθογένεια της νόσου δεν είναι σαφώς καθορισμένη, αλλά σύμφωνα με τις τελευταίες βιβλιογραφικές αναφορές, είναι αποτέλεσμα έκθεσης γενετικά ευπαθών ατόμων σε ειδικούς περιβαλλοντικούς παράγοντες. Πολλά γονίδια έχουν μελετηθεί για την συσχέτισή τους με την εκδήλωση της σαρκοείδωση. Στην παρούσα μελέτη διερευνήθηκε η συσχέτιση των γονιδίων IL-12Bp40 στην θέση +1602A/C (rs3212227), IL-18 στη θέση -137G/C (rs187238) και BTNL2 με την παθογένεια της σαρκοείδωσης. Η ομάδα των ασθενών αποτελούνταν από 146 Έλληνες ασθενείς που έπασχαν από σαρκοείδωση και προήρθαν από την Α' Πανεπιστημιακή Πνευμονολογική κλινική του Πανεπιστημίου Αθηνών και από την 8η Πνευμονολογική κλινική του Ν.Ν.Θ.Α. «Η Σωτηρία». Η ομάδα ελέγχου αποτελούνταν από 90 υγιή άτομα εθελοντές από το γενικό πληθυσμό.Από την μελέτη των γονιδίων διαπιστώθηκε ότι ο πολυμορφισμός rs3212227 της IL-12Βp40 δεν σχετίζεται με τη παθογένεια της σαρκοείδωσης στον Ελληνικό πληθυσμό, ενώ για τον πολυμορφισμό -137G/C (rs187238) της IL-18, διαπιστώθηκε ότι το αλλήλιο C αποτελεί πιθανό παράγοντα κινδύνου εκδήλωσης της σαρκοείδωσης στον Ελληνικό πληθυσμό. Αντίθετα το αλλήλιο G ανιχνεύθηκε σε αυξημένη συχνότητα στην ομάδα των μαρτύρων καθιστώντας αυτό το αλλήλιο πιθανό προστατευτικό παράγοντα στην εκδήλωση της σαρκοείδωσης.Από την μελέτη της αλληλουχίας του γονιδίου BTNL2 ανιχνεύθηκαν 24 διαφορετικές μεταλλάξεις σε 102 ασθενείς (69,9%) με σαρκοείδωση και 15 διαφορετικές μεταλλάξεις σε 56 μάρτυρες (62,2%) και στην πλειοψηφία τους στα άτομα και από τις δύο ομάδες μελέτης ανιχνεύθηκαν περισσότερες από μία μεταλλάξεις. Από τις 24 μεταλλάξεις οι 9 μεταλλάξεις p.D118N, p.A142, p.G143D, p.T165I, p.E242G, p.S334L, p.G412C, p.A430D, p.G454C διαπιστώθηκαν, από την in silico ανάλυση, ότι ήταν πιθανόν παθολογικές αλλά η συχνότητα εμφάνισή τους μεταξύ των δύο ομάδων μελέτης δεν ήταν στατιστικά σημαντική. Η μετάλλαξη p.W94R (rs28362682) διαπιστώθηκε ότι σχετίζεται με την εκδήλωση ήπιας πνευμονικής νόσου ενώ δεν διαπιστώθηκε ανάλογη συσχέτιση των υπόλοιπων μεταλλάξεων ούτε και του πολυμορφισμού rs2076530 οποίος έχει αποδειχθεί ότι σχετίζεται με την εκδήλωση της σαρκοείδωσης σε άλλους Ευρωπαϊκούς πληθυσμούς.
