SummaryBackgroundIn this prospective study, we looked for correlations between anatomic variants of paranasal sinuses and chronic or recurrent sinusitis.Material/MethodsTwo hundred and forty (240) patients with clinical features of chronic rhinosinusitis were examined; patients with first-onset or allergic sinusitis and pregnant females were excluded. Routine multi-slice CT of the paranasal sinuses was performed to look for mucosal disease of the paranasal sinuses, drainage pathways, and presence of anatomical variations and their relation to known sinus drainage pathways.ResultsAnatomic variations were very frequent, and we classified them into four easily recognized groups: nasal septum variations, middle turbinate variations, uncinate process variations, and ethmoidal variations. Deviated nasal septum was the most frequent variation in patients with chronic or recurrent sinusitis, and it was detected in 48.8% of cases. Agger nasi cells and concha bullosa were equally frequent (30.6%), and Haller cells were detected in 11.2%. Uncinate process variations were detected in 18.1%, and the large ethmoid bulla was detected in 10%.ConclusionsThe importance of anatomic variations is that they can compromise drainage pathway of the related sinus, which results in inflammatory sinus disease. Anatomical variations are not diseases on their own and may be present as incidental findings in patient with chronic sinusitis.

Background: Non-traumatic gastrointestinal emergencies in the children and neonatal patients and the presenting characteristics ultrasound features on the longitudinal and axial axes are a dilemma for the radiologist in the emergency room. The most frequent emergencies are appendicitis, intussusceptions, hypertrophic pyloric stenosis, and volvulus due to intestinal malrotation. Objectives: this work aimed to assess the role of ultrasound in the assessment of pediatric non-traumatic gastrointestinal emergencies. Patients and Methods: A retrospective analysis of 205 ultrasound examinations performed for neonatal and children patients with fever, abdominal pain, vomiting and diarrhea were evaluated. Results: Of 205 cases, 19 cases of intussusceptions, 49 cases of appendicitis, 2 cases associated with abscess, 1 gangrenous appendicitis with absence a color Doppler, 30 cases of perforated appendicitis and 3 cases of volvulus were found. Conclusions: Acute abdominal pain is one of the most common complaints in childhood, and one that frequently requires rapid diagnosis and treatment in the emergency department. Although acute abdominal pain is typically self-limiting and benign, there are potentially life-threatening conditions that require urgent management, such as appendicitis, intussusception, or bowel obstruction. Meticulous history taking and repeated physical examinations are essential to determine the cause of acute abdominal pain and to identify children with surgical conditions.

Background: Lower extremity venous thrombosis (LEVT) is the most frequent form of vascular involvement in Behcet's disease. Up to 17% of the mortality in Behcet's disease is reported to be associated with venous involvement such as pulmonary embolism or Budd-Chiari syndrome (BCS). Early recognition and appropriate management of vascular involvement in BD is essential to reduce associated morbidity and mortality. Homocysteine (hcy) may play a role in BD patient with venous affection. Objectives: To find out the role of homocysteine and venous Doppler ultrasonography in detection of venous insufficiency (VI) in BD patients. Subjects and Methods: A case control study, 50 patients with Behcet's diseaseand 50 age and sex matched healthy controls were included. Serum homocysteine (by ng/l) was determined by ELISA. Also measurement of VI by venous Doppler ultrasonography in lower limbs of Behcet's patients and controls. Results: In this study there was statistically significant difference between serum homocysteine level among BD patients and controls (p < 0.05). There was a significant difference between serum homocystiene levels in BD patients with venous versus non venous affections, p<0.05. There was a significal positive correlation between serum hcy levels and VI grades by Doppler US. Conclusions: Behcet's disease patients with hyperhomocysteinaemia and VI diagnosed by Power Doppler US should be considered as a strong indicator of pathological venous involvement.
