The low index of suspicion of periprosthetic tubercular infection could delay a correct diagnosis with risk of permanent damage due to a late treatment. During any surgical revision of prosthetic joints with suspect infection culture for tuberculosis should be taken into consideration.

INTRODUZIONELe infezioni delle vie urinarie (IVU), pur essendo associate a una letalità piuttosto bassa, rappresentano a causa dell'elevata frequenza e morbilità un rilevante problema clinico. Vi è consenso sul fatto che, in assenza di fattori di rischio concomitanti, quali per esempio l'età avanzata e il diabete, il microrganismo più frequentemente in causa sia rappresentato da Escherichia coli, con incidenze che oscillano, nei diversi studi e nelle differenti situazioni geografiche, dal 70% al 90% (1, 2, 7, 10, 14, 21, 23). Questo dato è stato recentemente confermato anche in Italia da differenti studi che hanno evidenziato, per le IVU gestite a livello ambulatoriale, una netta prevalenza di E. coli (5,6,22). L'eziologia delle IVU non complicate comunitarie è inoltre frequentemente attribuibile ad altri Gram-negativi, come Proteus spp., Klebsiella spp. e Enterobacter spp., più raramente a Gram-positivi come Enterococcus spp. e Staphylococcus spp. (12,14). L'eziologia delle IVU nosocomiali, che sono pressoché tutte complicate, differisce notevolmente da quella delle infezioni comunitarie. L'importanza relativa di E.
Introduction. Staphylococcus aureus is a major opportunistic pathogen especially in nosocomial settings. An increasing proportion of strains has intrinsic resistance to methicillin (MRSA) and recent reports documented the emergence of MRSA also in the community. The aim of this study was to evaluate methicillin-resistance in Liguria. Methods. 767 isolates of Staphylococcus spp have been collected from 10 laboratories, including 508 S. aureus: 292 (57.5%) hospital-acquired, 188 (37%) community-acquired, and 28 (5.5%) from healthcare settings; 259 coagulase-negative staphylococci (CNS): (193 (74.5%) hospital-acquired, 56 (21.6%) community-acquired, and 10 (3.9%) healthcare settings. Susceptibility tests were carried out by the disk diffusion method (CLSI, 2006) using cefoxitin (CFX) (Oxoid, Milan). MRSA has been also evaluated by PBP2' latex agglutination test (Oxoid, Milan) on 52 randomly selected S. aureus. Results. The data obtained indicated an incidence of MRSA in nosocomial settings up to 45.2%, while among the community-acquired isolates the percentage was 6.4%. With respect to CNS the figures were 61,8% and 10,6% for the nosocomial-and community-acquired organisms respectively. A 99% agreement between data of the coordinating centre and the various laboratories was found. Methicillin-resistance was confirmed in 52 S. aureus by PBP2' latex agglutination test. Conclusions. Present findings indicate that MRSA in community in the Ligurian area exists and this phenomenon requires future surveillance.
Introduzione. Lo streptococco di gruppo B (GBS) è causa frequente di sepsi e infezioni gravi nel neonato e nella donna gravida. La penicillina rimane il farmaco di scelta per il trattamento e la profilassi delle infezioni da S. agalactiae. La totale sensibilità di GBS ai β-lattamici, non modificatasi nel tempo, potrebbe evolvere negativamente. Fallimenti terapeutici attribuiti al fenomeno della tolleranza e la considerazione di quanto avviene nelle specie batteriche correlate allarmano la comunità scientifica che, come per Streptococcus pyogenes, invita alla continua sorveglianza della sensibilità reale alla penicillina e altri βlattamici nello streptococco di gruppoB. In questo studio è stata valutata la sensibilità alla penicillina di 754 ceppi di streptococco di gruppo B isolati in Liguria nel 2005 da campioni clinici. Materiali e metodi. Ceppi analizzati: sono stati valutati 699 ceppi di GBS provenienti da laboratori liguri, isolati da tamponi vaginali, tamponi rettali, urine. L'identificazione è stata eseguita con metodi routinari e confermata con agglutinazione al lattice. La sensibilità a penicillina, eritromicina e clindamicina è stata determinata come suggerito da CLSI mediante metodo della diluizione in agar. Risultati. nella Tabella sono riportati i valori della sensibilità agli antibiotici valutati durante lo studio. Sul totale degli isolati sono stati individuati ceppi con sensibilità al limite e 1 ceppo totalmente resistente alla penicillina.
