Differences in quantitative expression of IGF-1 mRNA isoforms in HCV-infected livers, as compared to the control, suggest that HCV may induce alteration of IGF-1 splicing profile.
The objective of our study was to evaluate the in vitro activity of levofloxacin in comparison with other antibiotics (coamoxiclav, ceftriaxone, ceftazidime, meropenem, aztreonam, ciprofloxacin and netilmicin) against about 500 Gram-negative nosocomial microorganisms, isolated during 2003. The bactericidal activities (minimal bactericidal activity and killing curve) of levofloxacin and the other antibiotics were also evaluated. The results confirm the excellent in vitro activity of levofloxacin against multiresistant nosocomial Gram-negative pathogens, including the 73 extended spectrum β-lactamase producer strains (90% of Escherichia coli, Enterobacter cloacae and Klebsiella pneumoniae were inhibited at 0.5 mg/l). Of particular interest is its susceptibility and bactericidal activity against Stenotrophomonas maltophilia strains. In conclusion, due to the favorable pharmacokinetics and pharmacodynamics of the molecule, associated with its safety, levofloxacin may represent a valid therapeutic option in the treatment of severe Gram-negative nosocomial infections.
INTRODUZIONEIn natura le cellule batteriche esistono come cellule singole (plancton) o come biofilm (5). Il biofilm rappresenta un sistema biologico con un alto livello di organizzazione, dove i batteri sono adesi a una superficie e strutturati in comunità funzionali (4). Le cellule del biofilm, adese a una superficie e contenute in una matrice extracellulare composta essenzialmente da polisaccaridi e proteine, differiscono dalla controparte planctonica per un diverso pattern di espressione genica e per una maggiore resistenza agli antibiotici (1). In ambienti ospedalieri i biofilm possano essere causa di infezioni persistenti, favorite dallo stato di immunocompromissione del paziente e da una precedente infezione o da esposizione all'antibiotico (2). Scopo del nostro lavoro è stato quello di: 1) valutare la presenza di microrganismi in pazienti geriatrici istituzionalizzati portatori di catetere vescicale permanente, 2) valutare la capacità dei microrganismi Gram negativi di produrre biofilm. METODIIl metodo utilizzato è stato quello di esaminare 150 campioni di urine da pazienti portatori di catetere vescicale permanente, ricoverati presso l'A.S.P Pio Albergo Trivulzio. Le urine sono state seminate con ansa calibrata da 1 microlitro su piastre di CLED agar e Mac Conkey agar e incubate a 35°C per una notte in termostato ad aria. Per i saggi di adesione sono state utilizzate micropiastre con fondo a "U", terreno LB diluito 1:4 e colorazione con cristalvioletto (3). La micropiastra inoculata è stata incubata per una notte alla temperatura di 37°C in termostato ad aria. Prima della colorazione con cristalvioletto si sono effettuati lavaggi con acqua distillata sterile. Per la rilevazione di enzimi di tipo ESBL, MBL e AmpC sono state utilizzate strip E-test (bioMérieux). La Figura I rappresenta una micropiastra con test di adesione sia di alcuni microrganismi isolati, sia dei controlli positivo e negativo. RISULTATISono risultati positivi 96 (64%) campioni, negativi 30 (20%), contaminati (campioni con più di due microrganismi isolati) 24 (16%). 85 (88%) degli isolati erano costituiti da microrganismi Gram negativi: E. coli (48%), P. mirabilis (16%), P. aeruginosa (13%), K. pneumoniae (5%), P. stuartii (2%), C.
