Mutations of ASXL1 are early events in acute myeloid leukemia (AML) leukemogenesis and have been associated with unfavorable prognosis. In this study, we investigated the type and frequency of ASXL1 mutations in a large cohort of patients with de novo or secondary AML (s-AML) and looked for correlations with cytogenetic findings and disease features. ASXL1 mutations were associated with older age, s-AML and higher peripheral leukocytosis. We observed more frequent co-occurrence of ASXL1 mutations with trisomy 8 and chromosome 11 aberrations but a negative correlation with myelodysplastic syndromes (MDS)-related cytogenetic abnormalities, especially -5/del(5q) and -7/del(7q). ASXL1 mutations were also found in other genetically defined AML subgroups such as those with t(9;22), inv(3)/t(3;3), t(8;21) or t(15;17); however, none of our inv(16) cases carried ASXL1 mutations. We detected two previously unreported ASXL1 mutations, p.IIe593Val and p.Cys688Tyr. Our findings suggest that ASXL1 mutations tend to cluster with specific clinical and cytogenetic profiles of AML patients.
Background/Aim: PON1 gene has an executive role in antioxidant defense, protecting cells from genotoxic factors. Q192R and L55M PON1 polymorphisms reduce catalytic activity of the encoded protein. These polymorphisms were studied in 300 chronic lymphocytic leukemia (CLL) patients and 106 healthy donors. They were also associated with patients' cytogenetic findings, to investigate their possible implication in CLL pathogenesis. Materials and Methods: SNP genotyping was performed using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) assay. Karyotypic analysis was also performed by chromosome G-banding analysis and fluorescence in situ hybridization. Results: Genotypic and allelic distribution of Q192R polymorphism showed a statistically significant higher frequency of mutant genotypes and mutant alleles in patients compared to controls. The same observation was noted in patients with abnormal karyotypes and those carrying abn14q32 and del(6q). A statistically increased frequency for the mutant allele was also revealed in patients with del(11q). On the contrary, L55M polymorphism showed a similar distribution between patients and controls. Conclusion: Q192R polymorphism plays a role in CLL predisposition and the formation of specific chromosomal aberrations.Chronic lymphocytic leukemia (CLL), a clonal neoplasia of B-lymphocytes and the most common type of adult leukemia in the Western world is a disease of the elderly. Deletion of 13q appears to be the most frequent cytogenetic abnormality, while deletion or rearrangement of 17p, a less frequent abnormality resulting in loss or mutation of TP53 gene confers the worst survival in CLL (4-7).Although CLL pathogenesis has not been yet fully elucidated, epidemiological studies support an important role of genetics, immune function, infections and variable environmental factors in CLL pathogenesis (8,9). Indeed, CLL appears to show a relatively common familial aggregation supporting genetic susceptibility and marks heritability of the disease as a significant risk factor (10-13).Recently, it has been postulated that CLL results from a combination of genotoxic exposure influenced by genetic susceptibility. Since CLL is a leukemia of advanced age, it is unlikely that genetic susceptibility to CLL relies mainly on mutations in crucial genes such as tumor suppressor genes which would result in the onset of the disease in younger ages. Chronic lymphocytic leukemia (CLL) is the most common type of leukemia in adults and is characterized by the presence of specific cytogenetic abnormalities. CLL research has been focused on epigenetic processes like gene promoter methylation of CpG islands. In the present study, the methylation status of the RAD21 gene is studied and associated with cytogenetic findings in CLL patients in order to investigate its possible implication in CLL pathogenesis and the formation of CLL chromosomal abnormalities.
H Χρόνια Λεμφοκυτταρική Λευχαιμία (ΧΛΛ), μία από τις πιο συχνές μορφές αιματολογικών κακοηθειών, χαρακτηρίζεται από in vivo συσσώρευση μονοκλωνικών Β-λεμφοκυττάρων στο περιφερικό αίμα και στο μυελό των οστών, τα οποία διηθούν τους λεμφικούς ιστούς και τον σπλήνα προκαλώντας συχνά διόγκωση των λεμφαδένων και δυσλειτουργία του ανοσοποιητικού συστήματος. Η νόσος παρουσιάζει μεγάλη κλινική ετερογένεια η οποία είναι συνυφασμένη με την ετερογένεια που παρατηρείται σε γενετικό και μοριακό επίπεδο. Κάποιοι ασθενείς επιβιώνουν για πολλά χρόνια χωρίς θεραπεία, ενώ άλλοι παρουσιάζουν επιθετική νόσο και μικρή επιβίωση. Η μοριακή και γενετική βάση της ΧΛΛ δεν έχει πλήρως αποσαφηνιστεί ως σήμερα, ωστόσο στην παθογενετική πορεία της νόσου και στην μεγάλη κλινική ετερογένεια που παρουσιάζει, φαίνεται να εμπλέκονται γενετικοί και επιγενετικοί μηχανισμοί. Τα κυτταρογενετικά ευρήματα συμβάλλουν καθοριστικά στη διάγνωση, πρόγνωση και επιλογή του κατάλληλου θεραπευτικού πρωτοκόλλου των ασθενών με ΧΛΛ. Σήμερα, στην καλλιέργεια του μυελού των οστών των ασθενών με ΧΛΛ γίνεται προσθήκη ενός συνδυασμού διεγερτών των Β κυττάρων, όπως είναι ο συνδυασμός DSP-30 και ιντερλευκίνης-2 (IL-2), αυξάνοντας έτσι σημαντικά το ποσοστό ανίχνευσης των κλωνικών χρωμοσωμικών ανωμαλιών, εξατομικεύοντας την πρόγνωση των ασθενών. Η μοριακή ανάλυση έχει αναδείξει αλλαγές σε γονίδια τα προϊόντα των οποίων ρυθμίζουν σημαντικές λειτουργίες των κυττάρων, όπως η αύξηση, η διαίρεση και ο πολλαπλασιασμός τους. Οι αλλαγές αυτές αφορούν τόσο μεταλλάξεις γονιδίων, όσο και επιγενετικές αλλαγές. Η μεθυλίωση των κυτοσινών στις νησίδες CpG υποκινητών γονιδίων είναι μια από τις πιο μελετημένες επιγενετικές τροποποιήσεις του DNA στον άνθρωπο και επηρεάζει τη δομή της χρωματίνης και τη μεταγραφική ενεργότητα των γονιδίων. Αν και η διερεύνηση των μοριακών μηχανισμών λευχαιμογένεσης βρίσκεται σήμερα στο επίκεντρο του ερευνητικού ενδιαφέροντος, η μελέτη αυτών των μηχανισμών στη ΧΛΛ είναι ακόμα σε πρώιμο στάδιο. Πολύ πρόσφατες μελέτες υποδεικνύουν ότι η μεθυλίωση του DNA είναι μια σημαντική επιγενετική τροποποίηση η οποία σχετίζεται με τη ΧΛΛ. Στην παρούσα διατριβή έγινε μελέτη της μεθυλίωσης του γονιδίου RAD21 το οποίο εδράζεται στα μακρά σκέλη του χρωμοσώματος 8, στη χρωμοσωμική περιοχή 8q24.11. Η παραγόμενη πρωτεΐνη RAD21 αποτελεί μία από τις βασικές υπομονάδες του πρωτεϊνικού συμπλέγματος cohesin που εμπλέκεται στον μηχανισμό που ελέγχει το σωστό διαχωρισμό των αδελφών χρωματίδων και συμμετέχει στους μηχανισμούς επιδιόρθωσης του DNA. Η μεθυλίωση του υποκινητή του γονιδίου RAD21 οδηγεί σε μεταγραφική απενεργοποίηση και κατ’ επέκταση γονιδιακή αποσιώπηση. Η μελέτη της μεθυλίωσης του RAD21 και η συσχέτιση των ευρημάτων με την ΧΛΛ και τις ειδικές κυτταρογενετικές αλλοιώσεις της μπορεί να συμβάλει στην καλύτερη κατανόηση της παθογενετικής πορείας της νόσου αλλά και στο σχεδιασμό νέων θεραπευτικών σχημάτων απομεθυλιωτικών παραγόντων που ήδη χρησιμοποιούνται στη θεραπεία διαφόρων άλλων αιματολογικών κακοηθειών. Επιπλέον, μελέτες ασθενών με ΧΛΛ έχουν αναδείξει την συμμετοχή γενετικών προδιαθεσικών παραγόντων στην εμφάνιση της νόσου. Οι παραοξονάσες (PONs) είναι ένζυμα που προστατεύουν τα κύτταρα από γονοτοξικούς παράγοντες όπως τα οργανοφωσφορικά και οι ελεύθερες ρίζες οξυγόνου, παράγοντες που προκαλούν μεταβολικές αλλοιώσεις και βλάβες στο DNA συμβάλλοντας στην καρκινογένεση. Το γονίδιο PON1 (παραοξονάση 1, υπομονάδα του γονιδιακού συμπλέγματος των παραοξωνασών) που εδράζεται στην χρωμοσωματική περιοχή 7q21.3, παρουσιάζει δύο πολυμορφισμούς τύπου SNP στην κωδικοποιούσα περιοχή του, στις θέσεις 55 και 192, που έχουν σαν αποτέλεσμα την αντικατάσταση ενός αμινοξέος και επηρεάζουν τη συγκέντρωση, τη σταθερότητα και την ενεργότητα του ενζύμου. Πιο συγκεκριμένα ο πολυμορφισμός L55M (Leu55Met [rs854560]) αφορά αντικατάσταση λευκίνης (L) από μεθειονίνη (Μ) και ο πολυμορφισμός Q192R (Gln192Arg [rs662]) αντικατάσταση γλουταμίνης (Q) από αργινίνη (R). Στην παρούσα διατριβή μελετήσαμε τους δύο πολυμορφισμούς του γονιδίου PON1, προκειμένου να διερευνηθεί η πιθανή εμπλοκή τους στην γενετική προδιάθεση της ΧΛΛ και στη δημιουργία των χαρακτηριστικών χρωμοσωμικών αλλοιώσεων της. Σκοπός της παρούσας διδακτορικής διατριβής είναι η κυτταρογενετική, μοριακή και επιγενετική διερεύνηση ασθενών με Χρόνια Λεμφοκυτταρική Λευχαιμία. Πιο συγκεκριμένα η παρούσα διατριβή επικεντρώνεται στη διερεύνηση του προτύπου μεθυλίωσης του υποκινητή του γονιδίου RAD21 και τη μελέτη των πολυμορφισμών L55M και Q192R του γονιδίου PON1 που σε συνδιασμό με τα κυτταρογενετικά δεδομένα μπορεί να συμβάλλουν στη διαλεύκανση των μηχανισμών παθογένεσης και προδιάθεσης της νόσου. Για τη συγκεκριμένη διατριβή χρησιμοποιήθηκαν 300 δείγματα μυελού των οστών από ασθενείς με ΧΛΛ που παραλήφθησαν στο Εργαστήριο του ΕΚΕΦΕ «Δημόκριτος» προκειμένου να πραγματοποιηθεί κυτταρογενετική τους ανάλυση. Τα κυττάρα μυελού των οστών καλλιεργήθηκαν, συλλέχθηκαν και μονιμοποίηθηκαν. Μετά από ζωνοποίηση των χρωμοσωμάτων, με τη μέθοδο GTG banding, έγινε μικροσκοπική και καρυοτυπική ανάλυση και τέλος αξιολόγηση των αποτελεσμάτων. Οι καρυότυποι περιγράφηκαν σύμφωνα με το διεθνές σύστημα ονοματολογίας Κυτταρογενετικής ISCN 2016. Σε ασθενείς που κρίθηκε απαραίτητο έγινε εφαρμογή της μεθόδου του Φθορίζοντος in situ Υβριδισμού (FISH) σε μεσοφασικά και μεταφασικά κύτταρα (interphase και metaphase FISH). Από όλα τα δείγματα των ασθενών με ΧΛΛ καθώς και από 106 δείγματα περιφερικού αίματος υγιών δοτών απομόνωθηκε ολικό γενωμικό DNA, προκειμένου στη συνέχεια να πραγματοποιηθεί η μοριακή ανάλυση. Στη μελέτη της μεθυλίωσης του υποκινητή του γονιδίου RAD21 συμπεριλήφθηκαν 105 ασθενείς με ΧΛΛ, επιλεγμένοι με βάση τα κυτταρογενετικά τους ευρήματα, και 17 υγιείς δότες. Με PCR πραγματικού χρόνου (Real-Time PCR) και τη χρήση του EpiTect Methyl II PCR Assay μελετήθηκε το πρότυπο μεθυλίωσης του γονιδίου RAD21 και ακολούθως τα αποτελέσματα συσχετίστηκαν με τα κυτταρογενετικά ευρήματα των ασθενών ώστε να διερευνηθεί πιθανή εμπλοκή της εν λόγω μεθυλίωσης στο σχηματισμό συγκεκριμένων χρωμοσωμικών αλλοιώσεων της ΧΛΛ. Για την μελέτη των πολυμορφισμών Q192R και L55Mτου γονιδίου PON1, συμμετείχαν 300 ασθενείς με ΧΛΛ και 106 υγιείς δότες. Αφού έγινε προτυποποίηση των μεθόδων γονοτυπικής ανάλυσης για τους δύο πολυμορφισμούς του γονιδίου PON1, η κατανομή των γονοτύπων και των αλληλομόρφων των πολυμορφισμών έγινε μέσω της αλυσιδωτής αντίδρασης πολυμεράσης (Polymerase Chain Reaction, PCR) και πέψης με περιοριστικές ενδονουκλεάσες για τον προσδιορισμό φυσιολογικών και μεταλλαγμένων γονοτύπων. Η ανάλυση των αποτελεσμάτων πραγματοποιήθηκε με την μέθοδο PCR-RFLPs και με ηλεκτροφόρηση σε πήκτωμα αγαρόζης. Μετά την κατανομή των γονοτύπων και των αλληλομόρφων των πολυμορφισμών έγινε συσχέτιση με τα κυτταρογενετικά ευρήματα των ασθενών. Οι συσχετίσεις όλων των αποτελεσμάτων, εξετάστηκαν και συγκρίθηκαν με το στατιστικό µη-παραµετρικό τεστ χ2 (Chi-Square test). Η στατιστική ανάλυση πραγματοποιήθηκε χρησιμοποιώντας λογισμικό SPSS (Statistical Package for Social Sciences) έκδοση 20. Ο έλεγχος έγινε βάσει 95% διαστήματος εμπιστοσύνης (95% confidence interval) και επίπεδο σημαντικότητας 5%. Στις περιπτώσεις που ήταν απαραίτητο χρησιμοποιήθηκε και το Fisher’s Exact Test. Κατά την κυτταρογενετική ανάλυση των 300 ασθενών με ΧΛΛ, 39,3% (118/300) των ασθενών είχαν φυσιολογικό καρυότυπο (επιβεβαιωμένο και με FISH) και 60,7% (182/300) παθολογικό καρυότυπο. Μεταξύ των 182 ασθενών με παθολογικό καρυότυπο το 33,5% (61/182) έφεραν del(13q), 30,8% (56/182) τρισωμία 12, 21,9% (40/182) del(11q), 13,2% (24/182) del(17p), 11,5% (21/182) των ασθενών έφεραν αλλοιώσεις της περιοχής 14q32 [abn(14q32)], 6,6% (12/182) αλλοιώσεις του χρωμοσώματος 8 [abn(8)], 4,9% (9/182) των ασθενών del(6q) και 12,1% (22/182) των ασθενών έφεραν άλλες χρωμοσωμικές αλλοιώσεις. Όσον αφορά τη μεθυλίωση του γονιδίου RAD21 η μελέτη αυτή ήταν επιτυχής στο 96% των ασθενών και στο 100% των υγειών δοτών. Στατιστικά σημαντική υψηλότερη συχνότητα μεθυλιωμένων δειγμάτων παρατηρήθηκε στους ασθενείς (25,75%), σε σύγκριση με τους υγιείς δότες (0%) (p=0,022). Η συσχέτιση της μεθυλίωσης με τα κυτταρογενετικά ευρήματα έδειξε παρόμοια αποτελέσματα. Συγκεκριμένα, μεθυλίωση παρατηρήθηκε στο 28.57% των ασθενών με φυσιολογικό καρυότυπο και στο 25% των ασθενών με παθολογικό καρυότυπο υποδικνύοντας ότι η μεθυλίωση του συγκεκριμένου γονιδίου δεν σχετίζεται με τη δημιουργία χρωμοσωμικών αλλοιώσεων. Η παρούσα διατριβή αποτελεί την πρώτη μελέτη μεθυλίωσης του γονιδίου RAD21 σε ασθενείς με ΧΛΛ.Κατά την μελέτη των δύο πολυμορφισμών του γονιδίου PON1, η μελέτη του πολυμορφισμού Q192R ήταν 100% επιτυχής στους ασθενείς με ΧΛΛ και στο 98,1% για τούς υγιείς δότες. Η κατανομή των γονοτύπων και των αλληλομόρφων του πολυμορφισμού αυτού έδειξε στατιστικά σημαντική υψηλότερη συχνότητατων μεταλλαγμένων γονοτύπων (ετεροζυγοτών QR και ομόζυγων RR) (p=0.028) και του μεταλλαγμένου R αλληλομόρφου στους ασθενείς σε σύγκριση με τους υγιείς δότες (p=0.012). Συγκρίνοντας την κάθε καρυοτυπική υποομάδα ασθενών μαζί με τα αποτελέσματα των υγιών δοτών στατιστικά ψηλότερη συχνότητα εμφάνισης των μεταλλαγμένων γονοτύπων και αλληλομόρφων παρατηρείται στην ομάδα των ασθενών με παθολογικό καρυότυπο και συγκεκριμένα αυτών που έφεραν abn14q32 και del(6q) (p=0.030 και p=0.021 αντίστοιχα). Στατιστικά αυξημένη συχνότητα για το μεταλλαγμένο αλληλόμορφο αποκαλύφθηκε επίσης στους ασθενείς με την αλλοίωση del(11q) στον καρυότυπο τους (p=0.035). Η μελέτη του πολυμορφισμού L55M ήταν 100% επιτυχής στους ασθενείς με ΧΛΛ και στο 99,1% για τούς υγιείς δότες. Αντίθετα όμως με τον πολυμορφισμό Q192R, ο πολυμορφισμός L55M έδειξε στατιστικά παρόμοιες κατανομές μεταξύ ασθενών και μαρτύρων υποδεικνύοντας ότι ο πολυμορφισμός L55M σε αντίθεση με τον πολυμορφισμό Q192R, δεν φαίνεται να προδιαθέτει στην εμφάνιση της ΧΛΛ. Συμπερασματικά μπορούμε να πούμε ότι, στη συγκεκριμένη διατριβή έγινε η πρώτη μελέτη διερεύνησης της κατάστασης μεθυλίωσης του γονιδίου RAD21 σε ασθενείς με ΧΛΛ, και έδειξε αυξημένη παρουσία μεθυλίωσης στους ασθενείς με ΧΛΛ σε σχέση με τους υγιείς δότες, υποδεικνύοντας πιθανή συμετοχή της μεθυλίωσης του γονιδίου RAD21 στην παθογένεση της ΧΛΛ πιθανόν μέσω αυτοανανέωσης των κυττάρων της ΧΛΛ και όχι μέσω του σχηματισμού χρωμοσωμικών αλλοιώσεων. Τα αποτελέσματά μας από την μελέτη των δύο πολυμορφισμών του γονιδίου PON1, ενισχύουν τον πιθανό ρόλο του πολυμορφισμού Q192R στην προδιάθεση της ΧΛΛ και στη δημιουργία συγκεκριμένων χρωμοσωμικών αλλοιώσεων της νόσου, σε αντίθεση με τον πολυμορφισμό L55M.
