ABSTRAK Wilayah Indonesia yang terletak diantara tiga lempeng tektonik yaitu lempeng eurasia, lempeng Pasifik, dan lempeng hindia - australia. Kondisi indonesia rentan terhadap gempa bumi, tsunami letusan gunung api dan jenis- jenis bencana geologi lain. Kecamatan Meuraxa merupakan salah satu kecamatan di Kota Banda Aceh yang berada di ujung pulau sumatera. Wilayah ini telah terdampak langsung kejadian gempa dan tsunami tahun 2004 silam. Kecamatan tersebut sering mengalami kejadian angin puting beliung, badai dan abrasi yang merusak beberapa bangunan infrastruktur dan mengancam jiwa masyarakat setempat dan juga kejadian wabah penyakit seperti Covid 19, demam berdarah dan beberapa penyakit lainnya. Tujuan kegiatan pengabdian kepada masyarakat ini terbentuknya kader kesehatan dengan peningkatan pengetahuan dalam menghadapi krisis kesehatan serta bermanfaat bagi pemerintah daerah, masyarakat dan institusi dalam Pengurangan Risiko Bencana (PRB). Metode pengabdian ini menggunakan 3 (tiga) metode yaitu (1) ceramah dan tanya jawab, (2) demonstrasi dan (3) praktik (simulasi langsung). dimana dilakukan pengukuran pengetahuan terhadap pelatihan yang di berikan melalui pre dan post test. Hasil pengabdian masyarakat di Kecamatan Meuraxa Kota Banda Aceh di peroleh hasil pengetahuan sebelum dan sesudah pelatihan kepada kader kesehatan siaga bencana. Sebelumnya memiliki nilai rata-rata 8.09 menjadi 9.88. Perbedaan nilai rata-rata saat pre dan post test bermakna secara statistik, dimana nilai p value < α (0,000<0.05). Selain itu juga kegiatan pengabdian masyarakat ini juga menghasilkan sebuah komunitas baru yaitu terbentuknya 32 orang kader kesehatan siaga bencana di Kecamatan Meuraxa Kota Banda Aceh. Masukan dan saran kepada pihak mitra agar kegiatan ini tetap berkelanjutan minimal adanya pelatihan setiap 3 bulan sekali dengan berkoordinasi dengan mitra kesehatan dan penangulangan bencana. Indonesia is vulnerable to earthquakes, tsunamis, volcanic eruptions and other types of geological disasters. Meuraxa District is one of the sub-districts in Banda Aceh City which is on the tip of the island of Sumatra. This area was directly affected by the 2004 earthquake and tsunami. The sub-district often experiences tornadoes, storms and abrasions which damage several infrastructure buildings and threaten the lives of local people as well as disease outbreaks such as Covid 19, dengue fever and several other diseases. The purpose of this community service activity is to form health cadres with increased knowledge in dealing with health crises and to benefit local governments, communities and institutions in Disaster Risk Reduction (DRR). This service method uses 3 (three) methods, namely (1) lectures and questions and answers, (2) demonstrations and (3) practice (direct simulation). where the measurement of knowledge of the training provided through pre and post tests is carried out. The results of community service in Meuraxa District, Banda Aceh City, resulted in knowledge obtained before and after training for disaster preparedness health cadres. Previously it had an average value of 8.09 to 9.88. The difference in mean scores during the pre and post tests is statistically significant, where the p value <α (0.000 <0.05). In addition, this community service activity also resulted in a new community, namely the formation of 32 disaster preparedness health cadres in Meuraxa District, Banda Aceh City. Input and suggestions to partners so that this activity remains sustainable at least with training every 3 months in coordination with health and disaster management partners. Inguinal hernia is defined as a lump of an organ through the inguinal cleft in the abdominal wall. According to the World Health Organization (WHO), in 2010-2015 hernia patients reached nearly 20 million cases and there were 20 million cases of inguinal hernia surgery every year. In Indonesia, it is estimated that there are 300 thousand cases. Based on data obtained from the HCU Room at RSUD Dr. Zainoel Abidin Banda Aceh, from January 2020 to April 2021, there were 14 cases. The purpose of this study was to determine the implementation of nursing care for Mrs. T with postoperative inguinal hernia in the HCU Room of Dr. Zainoel Abidin Hospital Banda Aceh in 2021. In compiling this case study the author uses a descriptive method with a nursing process approach which includes assessment, nursing diagnoses, nursing planning, implementation and evaluation. The results of nursing care were obtained after 3x24 hours the client's pain complaints decreased from a scale of 6 to 4, respiratory status was 18 x/m, daily activities were still assisted. Based on the results of the final evaluation, it was concluded that Mrs. T's problem was partially resolved for the diagnosis of acute pain and activity intolerance. As for the ineffective breathing pattern, the problem is resolved. The suggestions given are expected to improve communication and obtain more accurate data, so that nursing care can be carried out appropriately.
