Intubation is one of the key procedures in emergency situations in Pediatrics. Rapid sequence intubation, which comprises the simultaneous or sequential administration of inducing agents (sedatives and analgesics) and neuromuscular relaxants, is not a common practice among pediatricians, despite many studies showing that it greatly improves intubating conditions and even reduces complications in the procedure. This article presents an update of the main available drugs as well as the indicated uses in airway management in the ICC.
Diseases of the respiratory system are some of the leading causes of child care in emergency services, with severity ranging from mild and self-limiting to life-threatening. Undetected acute respiratory failure is the main cause of cardiac arrest in children. The ability to recognize, produce an early diagnosis, and properly manage respiratory impairments in children are essential skills for doctors working in emergency services. The purpose of this article is to present an approach to systematic evaluation so as to enable early diagnosis and initial management of acute respiratory failure in children in emergency services.
A realização de intervenções diagnósticas e terapêuticas dolorosas ou desagradáveis tem mais chance de sucesso e é mais segura quando a dor e a ansiedade são controladas efetivamente. Este artigo tem como objetivo elaborar recomendações sobre sedação para procedimentos em crianças e adolescentes, por não anestesiologistas, a partir da melhor evidência disponível. Foi realizada busca na literatura, que incluiu os principais sumários e diretrizes sobre o tema, e o sistema GRADE foi utilizado para hierarquizar o conjunto de evidências que sustenta cada recomendação. A sedação para procedimentos consiste em cinco etapas: preparo, monitorização, intervenções não farmacológicas, intervenções farmacológicas e alta. O preparo envolve avaliação clínica, preparo da equipe e do material. A monitorização do paciente deve ser rigorosa, de acordo com o nível de sedação. A capnografia não reduziu a taxa de eventos adversos significativos, sendo recomendada sua utilização quando disponível, porem não obrigatória. Entre as intervenções não farmacológicas existe evidência de moderada qualidade para sustentar o uso de técnicas de distração e hipnose. As evidências são insuficientes para recomendar o uso de um medicamento em detrimento a outro, sendo que a escolha deve ser realizada de acordo com o tipo de procedimento, estado clínico basal/comorbidade de cada paciente e prevendo eventos adversos associados a cada medicamento. Para garantir a liberação segura, o paciente deve preencher critérios específicos, retornando à sua condição funcional inicial.
