Those most likely to undergo LEA among Costa Rican diabetic patients were men with 10 or more years of diabetes and average HbA1c > 8% who used insulin and had diabetic retinopathy. Patients on insulin therapy were at greatest risk, especially those with a previous amputation. Diabetic patients with the above-mentioned profile should be considered to be at very high risk of LEA and followed closely by the health care system.
Official figures show that life expectancy in Costa Rica is longer than in the United States (US), in spite of the fact that per capita health expenditure is only one-tenth that of the US. To check whether this is for real and to explore some of its determinants, 900 Costa Ricans aged 60+ were followed from 1984 to 2001. Follow-up household visits were made, deaths were tracked in the national death registry, and survival status in the voting registry was double-checked. In addition, the survivors were contacted in 2002. Two-thirds of the panel had died by December 2001. Kaplan-Meier curves, life tables and Cox regression were used to analyse the panel's survival. Mortality in the panel was slightly higher than the Costa Rican average and similar to that in the US, confirming the exceptional longevity of Costa Ricans. Survival was substantially lower among unmarried men and individuals with limited autonomy at the beginning of the study. The effect of socioeconomic status is weak. Insurance effects seem to be confounded by selection biases.
Introducción: La hormona llamada Leptina ha sido asociada con varios factores del Síndrome metabólico, incluyendo hipertensión arterial, obesidad, resistencia insulínica y lipoproteína de alta densidad no solo en adultos, también en adolescentes. También en el modelo animal se ha relacionado con la diabetes mellitus. En Costa Rica no se conocen sus niveles plasmáticos, ni su relación con la obesidad y lípidos en adolescentes.Objetivo: Investigar los niveles de leptinemia en escolares de sexto grado de un área urbano-marginal, y su asociación con lípidos y sobr epeso.Material y métodos: Estudio transversal realizado año 2001, con muestra aleatoria de escolares de sexto grado de 12 escuelas del Área de Desamparados 3 (n = 220). Previo consentimiento escrito de padres y escolares se midió peso y talla; se obtuvo suero en ayunas de 12 horas para cuantificar leptina (IRMA), colesterol total,HDLcolesterol y triglicéridos; el LDL-col se calculó.Resultados: Los niveles de leptinemia mostraron una distribución no normal, por lo cual su análisis se realizó con métodos no paramétricos. Su rango varió desde una mínima de 1.0 ng/ml, a un valor máximo de 42.8 ng/ml en el sexo masculino, y de 3.9 ng/ml a 65.2ng/ml en el sexo femenino. La comparación de cuartiles según sexo fue altamente significativa (p < 0.0003) siendo mayor en las mujeres. Introducción: Actualmente en Costa Rica debería considerarse la Diabetes Mellitus tipo 2, un problema de salud pública, por el impacto de sus complicaciones crónicas tanto en el ámbito individual y familiar, como en lo económico, en la escala nacional. Su detección y tratamiento temprano ha demostrado que reduce la carga de sus complicaciones, por lo que su tamizaje podría ser oportuno en personas que tengan factores de riesgo asociados. El objetivo de este estudio fue valorar los resultados de la implementación de un tamizaje de la diabetes tipo 2 a nivel de atención primaria, en personas en riesgo de desarrollarla.Metodología: El tamizaje se efectuó durante el año 2000, en la población adulta con al menos un factor de riesgo asociado, en 6 Equipos Básicos de Atención Integral en Salud (EBAIS), del Área de Salud 3 de Desamparados. Este se incorporó en el quehacer diario del EBAIS, sin alterar su programación. Se utilizaron cuatro estrategias para la realización del tamizaje: la oportunista, la selectiva, las programadas a grupos cautivos u organizacionales, y las jornadas de atención comunal o nacional.Resultados: Durante 2000 se lograron captar 174 diabéticos, diagnosticados por primera vez, o que ya lo eran, pero que no estaban registrados al carecer de control en el EBAIS, aunque vivían en el área. La estrategia predominante fue la oportunista, cuya responsabilidad recayó principalmente en el médico. La estrategia programada se realizó por medio de “jornadas” durante las ferias de salud en cada EBAIS y en el Día Mundial de la Diabetes, como parte de la jornada nacional; la cautiva se llevó a cabo en el Hogar Diurno de Ancianos. La selectiva, mediante visita domiciliar, fue mínima, por falta de glucómetros. Estas estrategias permitieron que la cobertura de captación aumentara de un 50% a finales de 1999 a un 70.2% a finales de 2000, considerando la cobertura como el porcentaje dado por el número de pacientes diabéticos captados o registrados en cada EBAIS, entre el número de personas diabéticas estimadas para cada EBAIS (considerando su prevalencia igual al 5% de la población de 20 y más años).Conclusión: Costa Rica puede lograr la detección temprana y tratamiento oportuno de la Diabetes Mellitus tipo 2, por medio de un tamizaje en subgrupos poblacionales de alto riesgo. El esfuerzo ya se está haciendo con la estrategia oportunista. Este estudio demuestra que esta no solo se puede implementar efectivamente, sino que, además, puede realizarse con muy poco esfuerzo. Ahora bien, la organización del Sistema de Salud con base en la Atención Primaria, permitiría desarrollar otra de las estrategias de tamizaje para obtener una mayor cobertura sin alto costo, es decir, el tamizaje domiciliar. Logísticamente existen las condiciones para realizarlo; lo que se necesita es llevar a cabo un estudio comunitario controlado para ver su costobeneficio.Si el resultado de este estudio es favorable, se podría implementar en toda Costa Rica como parte de la atención integral de la Diabetes Mellitus tipo 2, convirtiendo al país en pionero en el mundo en esta estrategia.
Justificación: La Diabetes Mellitus tipo 2 es una enfermedad de alta prevalencia en el país; alrededor de 130,000 costarricenses la padecen. Sus complicaciones tienen gran impacto en la autonomía de estas personas. Una de sus complicaciones microvasculares es la retinopatía diabética, que si no se diagnostica tempranamente y se le interviene lleva a la pérdida de la visión. Su tratamiento es eficaz, logrando disminuir hasta en dos tercios el riesgo de ceguera, si se siguen dos recomendaciones: hacer un diagnóstico temprano y proporcionando tratamiento con foto-coagulación cuando sea indicado. En Costa Rica no se conoce la prevalencia de esta complicación y tampoco cual es la alternativa de solución de su atención en nuestro sistema de salud.Objetivo: Describir cómo se ha enfrentado la búsqueda y atención de la retinopatía diabética en un área de salud del Á rea Metropolitana, considerando los diferentes niveles de atención; así como analizar la prevalencia de la retinopatía diabética, después de implementado un tamizaje para su detección.Método: Se revisaron los expedientes de 572 diabéticos tipo 2 registrados al 31 de diciembre del 2000 en 6 EBAIS del Á rea de Salud 3 de Desamparados, anotándose el resultado del fondo de ojo que no tuviera más de dos años de antigüedad. A los pacientes que carecían de este examen se les contactó y se les realizó el fondo de ojo por personal experimentado.Resultados: El 93% de los diabéticos no tenían fondo de ojo en los últimos 2 años independientemente del nivel de atención donde llevara su control. Más aún, alrededor del 90 % no tenía documentado un fondo de ojo en ningún momento en el expediente. El tamizaje realizado logró aumentar la cobertura de fondo de ojo a un 60 %. Se obtuvieron datos de fondo de ojo de 345 pacientes (60.3%). De éstos, 27.5% presentaron algún tipo de retinopatía diabética; 16.5% no proliferativa, 8.7% no proliferativa severa y un 2.3% con retinopatía proliferativa. Fueron diagnosticados con el tamizaje 30 pacientes con retinopatía no proliferativa severa que requerían tratamiento con foto-coagulación de inmediato. Tres de esos paciente con edema macular requerían fotocoagulación urgente y se encontró dificultad para referirlos al III nivel de su área de atracción por no haber citas prontas.Conclusión: El presente estudio en comunidad demostró que no se está detectando la retinopatía diabética por cuanto no se hace fondo de ojo a los pacientes y hay dificultad para su intervención en el III nivel.
