Leprosy is a chronic infectious disease caused by Mycobacterium leprae. According to official reports from 121 countries across five WHO regions, there were 213 899 newly diagnosed cases in 2014. Although leprosy affects the skin and peripheral nerves, it can present across a spectrum of clinical and histopathological forms that are strongly influenced by the immune response of the infected individuals. These forms comprise the extremes of tuberculoid leprosy (TT), with a M. leprae-specific Th1, but also a Th17, response that limits M. leprae multiplication, through to lepromatous leprosy (LL), with M. leprae-specific Th2 and T regulatory responses that do not control M. leprae replication but rather allow bacterial dissemination. The interpolar borderline clinical forms present with similar, but less extreme, immune biases. Acute inflammatory episodes, known as leprosy reactions, are complications that may occur before, during or after treatment, and cause further neurological damages that can cause irreversible chronic disabilities. Congenital Zika syndrome (CZS) is a cluster of malformations induced by Zika virus (ZIKV) infection and the underline mechanisms involved in its occurrence are yet not fully understood. Along with epidemiological and environmental factors, the genetic host factors are suggested as important to the CZS occurrence and development, however, few studies have evaluated this. This study enrolled a total of 245 individuals in a case–control association study compound a cohort of high specific interest constituted by 75 mothers who had delivered CZS infants, their 76 infants, and 47 mothers that had delivered healthy infants, and their 47 infants. Sixteen single-nucleotide polymorphisms on TREM1, CXCL10, IL4, CXCL8, TLR3, TLR7, IFNR1, CXCR1, IL10, CCR2 and CCR5 genes were genotyped to investigate their association as risk factors to CZS. The results show an association between C allele at TREM1 rs2234246 and C allele at IL4 rs224325 in mothers infected with ZIKV during pregnancy, with the increased susceptibility to CZS occurrence in their infants and the SNP CXCL8 rs4073 and the G allele at CXCL10 rs4508917 with presence of CZS microcephaly in the infants. Furthermore, the T allele at CXCL8 rs4073 and TRL7 rs179008 SNPs were associated with the severity of microcephaly in children with CZS. These results suggest that these polymorphisms in genes of innate immune responses addressed here are associated to increased risk of occurrence and severity of CZS in pregnant mothers infected with ZIKV and their CZS infants.
Objetivos: determinar o perfil clínico-epidemiológico do trauma mecânico pediátrico em um hospital público terciário do município de Aracaju-SE; descrever as características do trauma como o mecanismo, o tipo e gravidade; e analisar o desfecho do trauma. Metodologia: trata-se de um estudo observacional, prospectivo, de base hospitalar pautado na análise documental de dados obtidos a partir dos prontuários com registros de crianças internadas vítimas de trauma mecânico. Resultados: com base nos dados obtidos, de um total de 62 participantes da pesquisa, o sexo masculino foi o mais acometido, com (62,9%) do total, sendo a faixa etária mais acometida foi de 5 a 9 anos, com (40,3% dos casos); 64,5% dos pacientes chegaram por meios próprios ao hospital, tendo como principal mecanismo de trauma a queda, 75,8% dos casos. O politraumatismo esteve presente em 19,4% dos pacientes. Conclusão: a maior parte dos casos de trauma evoluiu para alta hospitalar e não houve necessidade de admissão em UTI. O trauma não é apenas de uma doença grave, mas um relevante problema de Saúde Pública, pelo fato de ser a principal causa de mortalidade pediátrica nos países desenvolvidos, além de determinar a presença de sequelas transitórias ou permanente que, consequentemente, podem trazer prejuízos ao desenvolvimento físico, social e psicológico do paciente.
Este estudo teve como objetivo avaliar o incentivo ao aleitamento materno no pré-natal e no intra-hospitalar em Hospital pediátrico filantrópico de Aracaju-SE, entre Fevereiro a Abril de 2019. Trata-se de uma pesquisa quantitativa, prospectiva, transversal de caráter exploratório realizada através de questionários aplicados a nutrizes de recém-nascido e lactentes até um ano levados a urgência pediátrica e ambulatório do Hospital em questão. Os dados foram colocados em tabelas através de distribuição proporcional, sumarizadas como frequência simples e seus intervalos de confiança para 95%, bicaudal e p ≤ a 0,05. As variáveis mais relevantes foram comparadas através do teste qui-quadrado, teste exato de Fisher e Teste t-Student. O programa estatístico foi o SPSS (Statistical Package for Social Sciencies) 21. Observou-se na amostra que a faixa etária predominante entre as nutrizes variou entre 25 a 31 anos, apresentavam baixa escolaridade, eram desempregadas ou realizavam atividades diversas. Além disso eram naturais de Aracaju, raça parda e casadas. Entre o total, 63,6%(n=82) tiveram orientações sobre o aleitamento materno no pré-natal e 62% (n=80) no intra-hospitalar, a média de consultas foi de 6, sendo maior o número de consultas entre as pacientes que obtiveram orientações sobre o aleitamento materno. A prevalência do contato pele a pele foi de 59,8% (n=79), sendo que 65,1% (n=71), 34,9% (n=38) nos partos normais e cesarianos respectivamente. Conclui-se, portanto, que o papel do profissional de saúde no incentivo ao aleitamento materno precisa melhorar, pois a frequência do aleitamento materno permanece aquém do preconizado pelo Ministério da Saúde.
