\s=b\Bilateral choanal atresia presents as neonatal respiratory distress and suckling difficulty. Definitive repair of this anomaly by both transpalatine and transnasal approaches has been effective in young children. The transnasal procedures are preferred in infants because of the shorter operative time, low morbidity, and minimal potential for damage to developing structures. Fourteen children were treated for bilateral choanal insufficiency.
There is a 1% incidence of congenital internal carotid tortuosity, which places this vessel into apposition with the superior pharyngeal constrictor instead of its usual location 2 to 3 cm lateral to it. Two angiographically documented cases are reported and the embryologic cause of this anomaly is discussed. Palpation of the pharyngeal wall prior to tonsillectomy or adenoidectomy is recommended.
A clinically appearing, well-encapsulated lipoma was locally excised from the right posterior upper buccal gingival sulcus of a 24year-old man and diagnosed histologically as pleomorphic liposarcoma. A right maxillectomy with preservation of orbital function was followed by 5,000 rads of planned postoperative radiation therapy. Four months later the tumor recurred outside of the original site, which has been controlled by further radiation and chemotherapy to these areas.
