RESUMO:A incidência de doenças cardiovasculares vem aumentando a cada ano e, na maioria das vezes, a cirurgia de revascularização do miocárdio se faz necessária, sendo a infecção de sítio cirúrgico, uma das principais complicações. Este estudo teve por objetivo identificar sinais e sintomas de infecção de sítio cirúrgico no pós-alta hospitalar de cirurgia cardíaca reconstrutora. Trata-se de um estudo quantitativo descritivo de caráter prospectivo. A coleta de dados foi realizada na residência de 20 pacientes maiores de 18 anos, submetidos à cirurgia cardíaca reconstrutora, em um hospital filantrópico de nível quaternário, situado em São José do Rio Preto-SP, Brasil. Quatro pacientes apresentavam exsudato seroso ou purulento e hiperemia na incisão cirúrgica. Evidenciou-se uma taxa de 20% de infecção de sítio cirúrgico no pós-alta hospitalar. Ressalta-se a necessidade de implementar um método de vigilância no período cirúrgico focalizado no pós-alta hospitalar que possibilite uma real notificação das infecções de sítio cirúrgico, já que estas podem ser subnotificadas ocasionando uma alta taxa de morbi-mortalidade. SURGICAL SITE INFECTION SURVEILLANCE IN POST-HOSPITAL DISCHARGE AFTER CARDIAC RECONSTRUCTIVE SURGERYABSTRACT: The incidence of cardiovascular disease has been increasing annually in recent years, and in the majority of cases Myocardial Revascularization surgery becomes necessary. One of the principle complications of this procedure is post-surgical infection of the surgical site. This study aimed to identify signs and symptoms of infection of surgical site in the post-hospital discharge following cardiac reconstructive surgery. This is a descriptive quantitative study of a prospective character. Data was collected in the respective houses of 20 patients older than 18 years old who were submitted to cardiac reconstructive surgery in a quaternary philanthropic hospital located in São José do Rio Preto, SP, Brazil. Four patients presented serous exudate or festering and hyperemia in the surgical incision. An infection rate of the surgical site of 20% was found in post-hospital discharges studied. We highlight the need to implement a surveillance method in the period following post-hospital discharge to enable a real notification of surgical site infections, as these may be underreported and cause a high mortality rate. Controle glicêmico no perioperatório: evidências para a prevenção de infecção de sítio cirúrgico ABSTRACTThe objective of the present study was to analyze the evidence available in literature regarding blood glucose control to prevent surgical site infection in the perioperative period. The integrative literature review was the chosen review method.The primary studies were searched on the following databases: Medline, CINAHL, EMBASE and LILACS. Of the 26 primary studies included in the review, 12 were conduced with diabetes patients and 14 were performed with patients with and without diabetes; results showed a reduction in surgical site infection rates, mortality, and length of stay, when blood glucose is practiced. Nevertheless, due to the different outlines of the studied included, the investigated sample, type of surgery, blood glucose control parameters and methods, it is concluded that evidence is still insufficient to delimit the best level of target blood glucose and the ideal diet for blood glucose in the perioperative period.Descriptors: Perioperative Nursing; Surgical Wound Infection; Glycemic Index. RESUMENSe objetivó analizar las evidencias disponibles en la literatura sobre control glucémico para prevención de infección del sitio quirúrgico en el perioperatorio. Se adoptó el método de revisión integrativa de literatura. Para localización de los estudios primarios, se seleccionaron las siguientes bases de datos: Medline, CINAHL, EMBASE y LILACS, De los 26 estudios primarios incluidos en la revisión, 12 fueron realizados con muestra de pacientes diabéticos y 14 con pacientes con y sin diabetes; los resultados expresaron reducción en la tasa de infección del sitio quirúrgico, mortalidad y tiempo de internación hospitalaria, cuando se realizó control glucémico. Mientras tanto, en razón de diferencias en el diseño de los estudios incluidos, muestra investigada, tipo de cirugía, parámetros y métodos de control glucémico, se concluye que aún no existen evidencias suficientes como para delimitar el mejor nivel de glucosa-blanco y el régimen ideal para el control glucémico en el perioperatorio.
Objective:to identify socio-demographic, clinical and psychological predictors of well-being and quality of life in men who underwent radical prostatectomy, in a 360-day follow-up. Method:longitudinal study with 120 men who underwent radical prostatectomy. Questionnaires were used for characterization and clinical evaluation of the participant, as well as the instruments Visual Analog Scale for Pain, The Ways of Coping Questionnaire, Hospital Depression and Anxiety Scale, Satisfaction with Social Support Scale, Marital Satisfaction Scale, Subjective Well-Being Scale and Expanded Prostate Cancer Index. For data analysis, the linear mixed-effects model was used. Results:the socio-demographic factors age and race were not predictors of the dependent variables; time of surgery, problem-focused coping, and anxiety were predictors of subjective well-being; pain, anxiety and depression were negative predictors of quality of life; emotion-focused coping was a positive predictor. Marital dissatisfaction was a predictor of both variables. Conclusion:predictor variables found were different from the literature: desire for changes in marital relationship presented a positive association with quality of life and well-being; emotion-focused coping was a predictor of quality of life; and anxiety was a predictor of subjective well-being.
