Hypertension is highly prevalent; in Mexico, the 2012 National Health and Nutrition Survey reported a prevalence of hypertension of 31.5% in the adult population. Pharmacological treatment is the commonest intervention and has been shown to reduce cardiovascular mortality and morbidity, and total mortality. Accordingly, the type and number of antihypertensives used and the outcome — in terms of blood pressure (BP) control — are important. Therefore, our purpose is to learn the pattern of antihypertensive drug prescription and explore the determinants of BP control in an urban population in Mexico. A retrospective cross‐sectional drug utilization study was conducted. Medical records from a community health centre were searched to identify those corresponding to patients diagnosed with hypertension; information upon antihypertensives used and control of the disease was carefully retrieved. A logistic regression model was built to know the main determinants of BP control. A sample of 345 clinical records of interest was identified. Most patients received antihypertensives (86.4%); the leading medications used were angiotensin‐converting enzyme inhibitors, 63.8%; beta‐blockers (26.5%), diuretics (19.8%), angiotensin‐receptor blockers (15.8%) and calcium‐channel blockers (6.4%). Only the age (≥55 years) and BMI (>30) of the patients, and the age of the doctors (≥55 years), had an important influence on BP control. Obesity is a particular and important determinant of uncontrolled hypertension; it is worth to act on body weight, on an individual basis. As lack of control has been also tied to elderly doctors, an education programme could be envisaged.
www.conamed.gob.mx/revistaconamed.html www.medigraphic.org.mx Resumen Introducción: La OMS identificó a la automedicación como un componente del autocuidado en la salud; 80% de la población mexicana usa medicamentos por iniciativa propia sin indicación médica y sin receta. Objetivo: Identificar las principales causas de automedicación en estudiantes del área de la salud. Material y métodos: Se realizó un estudio transversal donde el universo del estudio fueron estudiantes del área de la salud de nivel licenciatura y postgrado. Se utilizó un instrumento exprofeso de automedicación. Resultados: La muestra fue de 305 estudiantes, 72% correspondió al género femenino, 71% tiene entre 17 a 30 años. La prevalencia de automedicación fue del 88%. Las razones principales por la que se automedican son por falta de tiempo 42.1%, fácil acceso a los medicamentos 37.4% y 14.4% por economía. Y el principal motivo de salud por que se automedican es por algias 73.8%, gripe 49.7%, fiebre 33.8%, gastroenteritis 27.8% y edema 26.4%. Conclusiones: La automedicación es frecuente en estudiantes del área de la salud y las principales causas de automedicación son falta de tiempo, fácil acceso y los motivos son: algias, gripe y fiebre. El profesional de salud debe establecer estrategias de prevención para evitar el enmascaramiento y agravar el proceso de la enfermedad, resistencia a medicamentos, intoxicación e inclusive la muerte. Asimismo, debe fomentar que la automedicación es un componente del autocuidado.
Introducción: La hipertensión arterial es un factor de riesgo para las enfermedades cardiovasculares; es un problema de salud que se incrementa cuando el paciente no cuenta con apoyo familiar. Objetivo: Conocer cómo influye el apoyo familiar en el cumplimiento o adherencia del tratamiento de hipertensión arterial. Metodología: Se realizó un estudio transversal en pacientes hipertensos que acudieron al Instituto Nacional de Cardiología (INCICh) de la ciudad de México, la recolección de la información fue a través de un cuestionario ex profeso que contempló dos secciones una para los pacientes y otro para los familiares de los pacientes. Resultados: Se estudiaron 234 pacientes hipertensos del INCICh de más 20 años de edad, el 64% son casados, el 47.7% cohabita con su cónyuge y el 33% con su hija(o); el 88% tiene una familia nuclear; 131 (56%) son pacientes descontrolados (47% del género femenino y 57% masculino) 103 (44%) controlados (54% del género femenino y 46% masculino). Con respeto al apoyo familiar el 84% vive con un familiar y sólo el 79% está en contacto diario con el paciente, en cuanto al apego farmacológico, el 56% le recuerda que debe tomar el medicamento, el 82% de la administración del medicamento la realiza el paciente mismo y el 76% le ayuda a su familiar a entender las indicaciones del médico. Las variables de apoyo familiar en las que se encontró una asociación estadísticamente significativa fue que el hipertenso debe medirse la tensión arterial en la casa y debe llevar su dieta y cuando el enfermo no la sigue lo amenaza de los riesgos que implica. Discusión: El apego al tratamiento se vincula de forma directa con el apoyo que los familiares le otorguen al enfermo. Conclusiones: El éxito del tratamiento depende fundamentalmente del apego, sin embargo el apoyo familiar juega un papel importante.
Introducción: En su devenir en la historia, la profesión de enfermería ha adoptado y construido símbolos que le representan socialmente. Los más comunes son el uniforme blanco, la capa, la lampara y la cofia. Objetivo: Conocer las causas de resistencia en el uso de la cofia en estudiantes de la licenciatura de enfermería y en enfermería obstétrica. Material y Métodos: Se realizó un estudio descriptivo, la unidad de análisis fueron estudiantes de las licenciaturas en enfermería. La muestra fue de 80 estudiantes del sexo femenino Se aplico un cuestionario, que consta de los siguientes apartados: datos generales y uso y resistencia de la cofia. Resultados: Las estudiantes refieren que el 71% usa la cofia como parte del uniforme, 61% por imposición y solo el 15% la usa como símbolo. El 95% considera que no es útil su uso para el rol de enfermería. El 96% de los estudiantes considera que el uso de la cofia es un factor de riesgo porque puede ser un fomite. Conclusiones: La principal causa de resistencia del uso de la cofia es que no le ven utilidad y la consideran un factor de riesgo para la Infecciones asociadas en la atención a la salud. Sin embargo, no debemos de perder de vista los factores sociohistóricos y las necesidades de cada época y contexto geográfico.
Introducción: La Encuesta Nacional sobre Dinámica de las Relaciones en los Hogares (ENDIREH) 2011, reporto que el 46.1% de las mujeres de más de 15 años han sufrido violencia de parte de sus parejas. Objetivo: Conocer y adentrarse en el estudio de violencia en las relaciones de pareja de los estudiantes de la ENEO-UNAM. Metodología: Se realizó un estudio descriptivo, la unidad de observación fueron alumnos de las carreras: Lic. en Enfermería y Lic. en Enfermería y Obstetricia, el muestreo fue no probabilístico. Se aplicó un cuestionario exprofeso “Detección de violencia en pareja”, de 15 reactivos. Resultados: Se estudiaron 112 alumnos, el 84.8% correspondió al género femenino, el 51% tienen menos 20 años, 37% tiene entre 20-23 y el 12% más de 23 años de edad. El 85.7% de las (os) alumnos, informo que tiene violencia emocional (controla su tiempo, celos y la acusa de ser le infiel), El 38.3% está en proceso de gestación de dependencia, el 15.8% tiene violencia económica, el 1.8% dijo tener violencia física, el 8.9% reportó violencia psicológica y solo se reportó un caso de violación por parte de su pareja. Conclusiones: Se confirma que existe violencia en las relaciones de pareja de tipo: emocional, dependencia, física, económica y sexual en los alumnos de enfermería, por lo que se debe de implementar y aplicar una intervención para disminuir esta. Es necesario fomentar que toda pareja llegue a acuerdos, donde se establezcan límites y distancias, sobre todo fomentar el mutuo respeto.
