Prevalensi wanita dengan diagnosis Diabetes Mellitus (DM) meningkat sepanjang tahunnya termasuk dalam kehamilan. Kami laporkan kasus seorang wanita 36 tahun dengan persalinan anak pertama yang dikonsultasikan dengan peningkatan gula darah sesudah persalinan. Dari hasil anamnesa dan pemeriksaan fisik tidak didapatkan riwayat DM pada pasien dan keluarga yang dapat menjadi penyebab keterlambatan diagnosis DM pada kehamilan. Anamnesis, observasi dan hasil pemeriksaan laboratorium mengindikasikan potensi DM yang dimulai sebelum kehamilan namun tidak dikonfirmasi melalui skrining pemeriksaan gula darah pada kehamilan. Bayi yang dilahirkan mempunyai berat badan normal namun sempat mengalami hipoglikemia. Pasien diberikan terapi injeksi insulin basal dimulai pada dosis 6 iu dan dinaikkan bertahap menjadi 8 iu dan 10 iu, sesuai dengan hasil evaluasi hingga tujuan terapi tercapai. Kasus ini menunjukkan pentingnya penggalian riwayat dengan teliti dan skrining DM dalam kehamilan pada mereka yang berisiko untuk mencegah komplikasi pada janin maupun ibu. Osteoporosis is an important health problem, and the number of patients who suffered from it is increasing. Breast cancer is a condition that has a high risk of osteoporosis. Cancer induced bone disease results from the primary disease, metastatic process, or from therapies against the prime condition, causing bone fragility. Therefore, there are still different opinions regarding the therapeutic effect of breast cancer on bone mineral density. Aim: to determine the effect of chemotherapy and hormonal therapy on bone mineral density in patients with breast cancer in Saiful Anwar Hospital Malang. Method: this is a case-control study conducted in 30 patients with breast cancer who had undergone chemotherapy and hormonal therapy are compared to 30 patients without breast cancer as controls. Bone Mineral Density (BMD) was measured by DEXA. Result: there are 53% of breast carcinoma patients having osteopenia and 13% having osteoporosis, with one patient has osteoporosis located in the femoral neck and three patients in lumbar (L1-L4). We obtain a significant T-score (p=0.02) in the great trochanter. Odds Ratio (OR) 0.233 with p = 0.008 showed a protective effect of chemotherapy and hormonal therapy for breast cancer to the bone density. Conclusion: the incidence of osteoporosis in the study was 13%, with the most location in the lumbar spine. Chemotherapy and hormonal therapy had a protective effect on BMD in patients with breast cancer in Saiful Anwar Hospital Malang.
ABSTRAKPseudotumor adalah kista berisi darah pada jaringan lunak atau tulang. Satu sampai dua persen hemofilia berat menyebabkan pseudotumor, yang bisa berasal dari jaringan lunak atau pada subperiosteal atau area intraosseus. Pseudotumor jarang terjadi namun merupakan komplikasi hemofilia yang berbahaya. Kami laporkan kasus pseudotumor hemofilia pada seorang laki-laki 27 tahun dengan keluhan bengkak pada paha kanan yang muncul tiba-tiba, merata dari pangkal paha sampai lutut. Keluhan hilang timbul, bergantian pada kaki kanan kiri sejak 2002. Riwayat perdarahan yang sulit berhenti 15 tahun sebelumnya. Pemeriksaan MRI (magnetic resonansi imaging) menunjukkan massa intra muskular regio femur dekstra 1/3 proksimal hingga 1/3 tengah dan massa intra muskular regio femur sinistra 1/3 proksimal melibatkan kompartemen anterior dan bone marrow replacement. Didapatkan Hemofilia A sedang dengan APTT memanjang (faktor VIII 7%) pada pemeriksaan darah. Pasien membaik dengan terapi konservatif, yaitu pemberian faktor VIII.
