Introducción: la presencia de enterobacterales productores de betalactamasas en los hospitales es cada vez más frecuente, constituyendo un problema de salud pública cada vez más grave a nivel mundial por su drogoresistencia. Objetivo: identificar enterobacterales productores de betalactamasas aislados con mayor frecuencia en muestras clínicas de pacientes hospitalizados en una institución de atención secundaria de la habana - Cuba, durante 2021. Material y Métodos: Estudio descriptivo - retrospectivo de corte transversal, que incluyeron 540 aislamientos de enterobacterales obtenidos de muestras clínicas procedentes de diferentes productos patológicos en 2021. Se determinó la resistencia bacteriana a 13 antibióticos por el método de Bauer - Kirby / difusión por discos (antibiograma interpretado). El mecanismo de resistencia por producción de BLEE y AmpC, se detectó mediante prueba de sinergia de doble disco y la prueba de aproximación de disco, respectivamente. Resultados: Escherichia coli (41%), Serratia marcescens (21%) y Klebsiella pneumoniae (18%) en orden descendente, fueron los mayores aislamientos durante el período de estudio; y la secreción traqueobronquial (35%) la muestra más significativa, a expensas de la Unidad de Cuidados Intensivos. Klebsiella pneumoniae (22%) y Escherichia coli (11%) fueron los de mayor producción de BLEE, destacándose altas cifras de resistencia para la cefotaxima, ceftazidima y ceftriaxona (100%), seguido de amoxicilina/ácido clavulánico (95%); se encontró 100 % de resistencia a la cefoxitina, cefotaxima, ceftazidima, ceftriaxona y amikacina en la única cepa bacteriana aislada productora de AmpC - Proteus mirabilis. Conclusiones: Klebsiella pneumoniae y Escherichia coli fueron los aislamientos de mayor producción de BLEE; AmpC se encontró solamente en Proteus mirabilis. Se encontraron altos porcentajes de resistencia a los antimicrobianos ensayados para todos los enterobacterales productores de betalactamasas.
Hay evidencias de que la pandemia por COVID-19 afecta también a los trabajadores del Sector Salud, existiendo reportes de su vulnerabilidad para infectarse durante su jornada laboral. Por esta razón se imponen retos y estrategias de control de infecciones por parte de las instituciones donde se brinde asistencia médica, contribuyendo decisivamente a la disminución de la propagación del virus. La implementación de medidas administrativas, ambientales y de protección respiratoria - fundamentalmente - requiere de una interrelación mantenida que abarque desde los directivos de una Unidad hasta el personal que se encuentre en la atención directa al paciente infectado; es por ello que el trabajo en conjunto es determinante para el éxito de estas estrategias. Por las razones anteriores nos propusimos abordar y profundizar sobre el tema, planteando medidas concretas para disminuir la incidencia del riesgo de contagio en el manejo de esta patología por parte del personal sanitario.
Introducción: Acinetobacter spp. constituye un patógeno relevante causante de Infecciones Asociadas a la Asistencia Sanitaria, lo que representa un desafío terapéutico por su multidrogorresistencia. El complejo Acinetobacter baumannii - calcoaceticus es la principal especie causante de infecciones graves en nuestro país. Objetivo: caracterizar los aislamientos de especies de Acinetobacter en pacientes ingresados en el Hospital Ortopédico Universitario Fructuoso Rodríguez, durante el período junio 2020 - junio 2021. Material y métodos: se realizó un estudio descriptivo - experimental de corte transversal, que incluyó 264 aislamientos obtenidos de este microorganismo procedentes de diferentes productos patológicos. La identificación de especies se realizó mediante pruebas bioquímicas según esquemas de diagnóstico estandarizados en nuestro medio y se determinó la susceptibilidad a 18 antibióticos por el método de Bauer - Kirby / difusión por discos (antibiograma interpretado) con excepción de la colistina cuya determinación se realizó por el E-test. Resultados: predominó el complejo Acinetobacter baumannii - calcoaceticus, fundamentalmente en muestras de secreción endotraqueal y sangre; a expensas de la Unidad de Cuidados Intensivos. Se detectaron porcentajes elevados de resistencia para los betalactámicos, aminoglucósidos y fluoroquinolonas. La tetraciclina, doxiciclina y colistina resultaron los antimicrobianos más activos. El 73.5% de los aislamientos fueron multidrogorresistentes, el 26.1% extremodrogorresistentes y un aislamiento resultó pandrogorresistente - 0.4%. Conclusiones: los hallazgos de Acinetobacter spp. se caracterizaron por una elevada resistencia a los antibióticos disponibles. Las infecciones provocadas por este Género constituyen un serio problema de salud, lo que revela la necesidad del monitoreo continuo de la susceptibilidad a los antimicrobianos y el reforzamiento de las medidas de control, fundamentalmente en áreas de alto riesgo.
