AimsTo assess the association of social determinants on the performance of health systems around the world.MethodsA transnational ecological study was conducted with an observation level focused on the country. In order to research on the strength of the association between the annual maternal and child mortality in 154 countries and social determinants: corruption, democratization, income inequality and cultural fragmentation, we used a mixed linear regression model for repeated measures with random intercepts and a conglomerate-based geographical analysis, between 2000 and 2010.ResultsHealth determinants with a significant association on child mortality(<1year): higher access to water (βa Quartile 4(Q4) vs Quartile 1(Q1) = -6,14; 95%CI: -11,63 to -0,73), sanitation systems, (Q4 vs Q1 = -25,58; 95%CI: -31,91 to -19,25), % measles vaccination coverage (Q4 vs Q1 = -7.35; 95%CI: -10,18 to -4,52), % of births attended by a healthcare professional (Q4 vs Q1 = -7,91; 95%CI: -11,36 to -4,52) and a % of the total health expenditure (Q3 vs Q1 = -2,85; 95%CI: -4,93 to -0,7). Ethnic fragmentation (Q4 vs Q1 = 9,93; 95%CI: -0.03 to 19.89) had a marginal effect. For child mortality<5 years, an association was found for these variables and democratization (not free vs free = 11,23; 95%CI: -0,82 to 23,29), out-of-pocket expenditure (Q1 vs Q4 = 17,71; 95%CI: 5,86 to 29,56). For MMR (Maternal mortality ratio), % of access to water for all the quartiles, % of access to sanitation systems, (Q3 vs Q1 = -171,15; 95%CI: -281,29 to -61), birth attention by a healthcare professional (Q4 vs Q1 = -231,23; 95%CI: -349,32 to -113,15), and having corrupt government (Q3 vs Q1 = 83,05; 95%CI: 33,10 to 133).ConclusionsImproving access to water and sanitation systems, decreasing corruption in the health sector must become priorities in health systems. The ethno-linguistic cultural fragmentation and the detriment of democracy turn out to be two factors related to health results.
OBJETIVO: Identificar evidencias y tendencias de pronóstico de la pandemia de COVID-19, que apoyen la toma decisiones sobre su mitigamiento. MATERIAL Y METODOS Investigación evaluativa basada en estimación de evidencias y tendencias de pronóstico a través de la simulación computacional del modelo S.E.I.R. que clasifica a la población en 4 grupos: los susceptibles al contagio (S); los expuestos al contagio (E), los infectados (I) y los Recuperados/Fallecidos (R). RESULTADOS El peor escenario, sin intervención, se daría a principios de junio con R0=3 afectando a un 73.2% de la población. Ante medidas de mitigación el mejor ajuste es con Ro=2.2 (mitigación 37.2%), iniciando a finales de julio, afectando un 58.9% de la población, CONCLUSIONES Se sugiere un monitoreo y adecuación permanente de medidas de mitigación en las fases de transmisión sostenida. Se requiere una estrategia de distanciamiento social intermitente, que se activaría con el incremento de la tasa básica de reproducción R0.
Objetivo: Identificar los determinantes de acceso a los servicios de atención primaria en adultos mayores residentes en áreas rurales y urbanas de diferentes regiones del mundo. Material y Método: Se realizó una revisión sistemática de la literatura en bases electrónicas a partir de publicaciones cualitativas y cuantitativas, de fuente primaria o secundaria, descritas entre 2015 y 2019. Se determinaron palabras clave enmarcadas bajo el Modelo de Dimensiones y habilidades de Levesque, Harris y Russell. El análisis se efectuó utilizando una lista de chequeo con cinco criterios técnicos de evaluación elegidos por conveniencia técnico-académica. Resultados: La búsqueda inicial identificó 118 manuscritos para revisión. Los estudios fueron analizados y clasificados en cinco segmentos que involucran a cinco regiones del mundo. Los principales determinantes de acceso encontrados fueron clasificados en habilidades personales de los adultos mayores y características propias del sistema de salud tanto para el área rural y urbana. Conclusiones: Se concluye que a nivel mundial el principal determinante de acceso a los servicios de atención primaria en adultos mayores residentes de áreas rurales es la falta de disponibilidad de servicios; mientras que los adultos mayores urbanos mantienen a las barreras financieras como el principal obstáculo para su acceso a la salud.
Objetivo: Analizar las aportaciones de incluir un marco analítico de gobernanza en el estudio de políticas en salud. Materiales y métodos: Se realizó una revisión de literatura publicada sobre gobernanza en salud en las bases de datos de EBSCO host. Durante un periodo que abarco de 2007 a 2017. Se usaron los descriptores de gobernanza y salud. Al final de la búsqueda se seleccionaron ocho artículos. Estos se clasificaron en metodológicos (n=5), estudios de gobernanza (n=2) y revisiones sistemáticas (n=1). Resultados: La revisión dio como resultado aportes en el ámbito de la definición, perspectiva, categorías de estudio e indicadores. Las definiciones incluían elementos como actores, normas, procesos y toma de decisión. Las perspectivas utilizadas de gobernanza fueron: analítica, prescriptiva y combinada. Las categorías de estudio son los problemas, actores, normas, puntos nodales y procesos. Los indicadores utilizados fueron objetos de estudio, Actores-poder, instrumentación y valores. Conclusiones: La aplicación de un marco analítico en gobernanza aporta información para identificar los espacios de toma de decisión. Un marco analítico de gobernanza complementa la información y permite una categorización que incluye la diversidad de actores y sus recursos de poder. Estudios en diversos ámbitos han demostrado la utilidad de usar un marco analítico de gobernanza en el estudio de políticas de salud.
RESUMEN El presente artículo incluye los principales planteamientos sobre las experiencias y balances de las Comisiones Nacionales en Macroeconomía y Salud (CMES), centrando su análisis en la pertinencia y relevancia para los países de América Latina. A manera de introducción, la primera parte plantea algunas premisas de las relaciones entre reforma en salud, inversión y desarrollo, como parte del eje central que abordan las CMES en los países. Posteriormente, se dan los principales antecedentes, lineamientos, conformación e implementación de tales comisiones en el mundo y en países de América Latina. La segunda parte del documento incluye el análisis de aportes y avances de metas y estrategias como objeto de análisis de tales comisiones: indicadores de mortalidad, indicadores de financiamiento/inversión e indicadores de generación de conocimiento. Finalmente, a manera de conclusión se plantean de manera explícita las principales reflexiones a partir del desarrollo de las CMES y de las tendencias de los indicadores revisados, así como una lista de sugerencias a manera de lecciones aprendidas que podrían ser retomadas para el redimensionamiento de las comisiones ya implementadas; o bien, para la implementación de nuevas comisiones en países donde aún se encuentran de manera incipiente. ABSTRACT This article covers the main approaches based on the experience and reports of the National Commissions on Macroeconomics and Health (CMES), focusing their analysis on the relevance and significance for the Latin American countries. As an introduction, the first part of this paper poses some premises of the relationship between health reform, investment, and development as part of the central axis that the CMES approach in the countries. After that, the main background, guidelines, creation, and implementation of such commissions in the world and in Latin America are given. The second part of this document includes an analysis of contributions and goals and strategies advancement as an object of analysis of such committees: mortality, finance/investment, and knowledge generation indicators. Finally, as a conclusion, there is a detailed explanation of the main reflections from the development of the CMES and trends of the revised indicators, as well as a list of suggestions that can be considered as learned lessons that could be retaken up for the remodeling of the already implemented commissions or to the implementation of new committees in countries that have them still in a fledgling way.
