Combined ACL and ALL reconstruction was found to be effective in improving subjective and objective outcomes. Nevertheless, these findings were not significantly superior to isolated ACL reconstruction except for the instrumented knee laxity testing results. This might indicate that ALL reconstruction should not be performed routinely for patients undergoing ACL reconstruction.

Acute traumatic patellar dislocation is a common injury, and spontaneous reduction may occur at the time of injury or may be reduced at the field of the accident by someone. It may be associated with osteochondral fractures and rupture of medial patellar stabilizers leading to recurrent patellar instability. The aim of this prospective study was to evaluate the outcomes of medial patellofemoral (PF) ligament (MPFL) reconstruction in recurrent traumatic patellar dislocation. Forty-five patients presented with PF instability as a result of traumatic rupture MPFL with normal patellar tracking underwent MPFL reconstruction without patellar fixation hardware through two parallel transpatellar tunnels and one screw in femoral tunnel. All patients were evaluated clinically preoperatively and at a minimum follow-up of 24 months, and International Knee Documentation Committee (IKDC) and Kujala scores were used to assess the clinical results. All patients were available for evaluation at a minimum of 24 months (up to 36 months). The mean age of these patients at the time of surgery was 22.82 years (range: 18–34 years). All patients gave history of trauma of their knees. Mean IKDC scale showed significant improvement as it rose from 47.17 preoperatively to 77.94 postoperatively, and mean Kujala score rose from 53.88 preoperatively to 86.24 postoperatively (p < 0.001). No recurrence of dislocation was recorded. Only three patients had mild atrophy of thigh and one patient had some difficulty in jumping. Reconstruction of MPFL by this method provides good clinical result in the treatment of PF instability by using autologous graft (semitendinosus and gracilis). Less hardware were used with less complications.

BackgroundDelayed graft function (DGF) is the most common early postoperative complication of renal transplantation. The occurrence of DGF can lead to both early and late devastating consequences on the allograft's survival. The risk of developing this complication can increase with certain factors that are related to both the donor and the recipient. In the present study, we aimed to detect the incidence rate of DGF among patients attending a tertiary care hospital in Riyadh, Saudi Arabia, and to investigate potential predictors of DGF. Materials and methodsThis retrospective chart review was conducted at King Abdulaziz Medical City (KAMC), a tertiary care hospital in Riyadh, Saudi Arabia. The inclusion criteria were all patients, 18 years or older, who had renal transplantation from January 1, 2016, to March 31, 2020. Patients who had a second renal transplant, or renal transplantation in a different hospital and were followed up at KAMC were excluded. Patients' medical records were accessed using the BESTCare electronic system to obtain the patients' demographic data. A Chisquare test was used to test for the association between a predictor and a delay in graft function. ResultsA total of 344 patients were enrolled in the present study, approximately half of whom were males (56.6%, n=189). Around one-half (49.4%) were aged between 40 and 64 years. The most common cause of renal failure was hypertension, which was found in 117 (35%) patients, followed by diabetes mellitus (DM) in 94 (28.1%) patients. Most organ donors 258 (77.2%) were alive. A total of 23 (6.9%) participants developed DGF. Mycophenolate mofetil (MMF) was found to be significantly associated with DGF (P < 0.001). Those who took MMF (5.9%) had a significantly lower rate of DGF compared to those who did not (36.4%). A significantly higher rate of DGF was seen in patients whose transplants were taken from deceased donors (15.5%) compared to living donor transplants (3.9%). Gender, age, body mass index (BMI), recipient blood type, donor blood type, and cause of renal failure were not associated with DGF. ConclusionsOnly 6.9% of the study's participants exhibited DGF. The observed rate was lower than the ones detected in the literature. Those who took MMF had a significantly lower rate of DGF compared to those who did not. Transplants of deceased donors (15.5%) showed a significantly higher rate of DGF. Larger multicenter studies are required to further investigate DGF in a region with a high prevalence of organ failure and a higher need for transplantations, such as Saudi Arabia.

Background: Urinary tract infection (UTI) is a common clinical problem in the primary care setting. Urine dipstick analysis is a quick, cheap and widely used test to predict UTI in clinically suspected patients. Objective: To evaluate the sensitivity of urine dipstick analysis as a screening test in predicting UTI in symptomatic adults in the primary care setting. Methods: A total of 420 culture-positive urine samples from patients with symptomatic UTI, who had dipstick urinalysis in a primary care center were the materials of this study from March to October 2015. The sensitivity of urine dipstick nitrites (NT), leukocyte esterase (LE) and blood was calculated and compared with positive culture samples either individually or in combination. Results: The sensitivity of dipstick NT alone was the lowest of all tests (20.7%), while LE alone was marginally higher than NT (31.42%), whereas dipstick blood test when considered alone was the highest sensitive (61.9%). In combination, NT and/ or LE were marginally higher than either test alone (41.2%), while NT and/or blood were (64.5%). The highest sensitivity of dipstick is obtained when all the three parameters were considered together (NT and/or LE and/or Blood, sensitivity 81.4%). Conclusion: Dipstick NT, LE, and blood are poor screening tests when used individually. Dipstick sensitivity significantly increases, and it could be considered a good screening test to predict UTI in symptomatic adults in the primary care setting when its three components are considered together. However, negative dipstick analysis should not rule out UTI in symptomatic adults, and urine culture is necessary for accurate diagnosis.
