According to results of the molecular methods, we thought that a C. albicans outbreak had occurred in the neonatal pediatric unit, due to contamination of TPN solution.
Amaç: Çalışmada, Escherichia coli ve Klebsiella pneumoniae suşlarının genişlemiş spektrumlu beta-laktamaz (GSBL) üretimi ve antibiyotik direnç oranlarındaki değişimin belirlenmesi amaçlandı. Gereç ve Yöntem: Beş yıllık (Ocak 2006-Eylül 2010) bir süreçte laboratuvarımıza gönderilen klinik örneklerden izole edilen 3488 E.coli, 994 K.pneumoniae suşunun GSBL üretimi ve antibiyotiklere direnç oranları retrospektif olarak araştırıldı. Konvansiyonel yöntemlerle tanımlanan tüm suşların kesin identifikasyonu ve antibiyotik duyarlılık testi için BD Phoenix otomatize sisteminden (Becton Dickinson, USA) yararlanıldı.Bulgular: E.coli suşlarında %48, K.pneumoniae suşlarında ise %67 oranında GSBL pozitifliği tespit edildi. İzole edilen suşlara en etkili antibiyotiklerin karbapenem grubu olduğu ve bu grup antibiyotikleri E.coli suşlarında amikasin, nitrofurantoin ve sefoksitinin, K.pneumoniae suşlarında ise amikasin, sefoksitin ve siprofloksasinin izlediği görüldü. Yıllara göre antibiyotik direnç oranlarında da artışlar meydana geldiği gözlendi. Sonuç: Bölgemizde enfeksiyon etkeni olarak izole edilen E.coli ve K.pneumoniae suşlarında GSBL oranları hayli yüksektir. Bunun bir sonucu olarak antibiyotik direnç oranlarında gözlenen artış, her hastanenin periyodik olarak kendi verilerini değerlendirmesi ve buna paralel antibiyotik kullanım politikası belirlemesi gerektiğini ortaya koymaktadır.
