Background:Emergency Medical Services (EMS) provides out-of-hospital acute medical care to different types of serious emergencies. The team of EMS includes paramedics, besides many other staff working in the pre-hospital setting. Although the paramedics are the first responders to the patient in emergency, they face several barriers, which interfere with their efficiency.Aim:However, the magnitude of these obstacles is not known, hence; it was found worthwhile to evaluate the depth of these difficulties.Materials and Methods:A questionnaire that included eight different commonly experienced barriers and four different variables of strength was framed. The questionnaire was served to 140 paramedics in three different regions (Jeddah, Riyadh, and Dammam) of Saudi Arabia.Results:The positive response obtained was statistically significant for traffic congestion (P<0.01; P<0.001), nuisance by bystanders and family members (P<0.001), lack of competence of doctors and the administration in the EMS routines (P<0.05; P<0.01; P<0.001), lack of trust and confidence (P<0.001) lack of independence (P<0.001), patient's resistance (P<0.001), Interference of legal issues and litigation proceedings (P<0.05; P<0.001), paramedic's impression on general public and the family of the patients (P<0.001).Conclusions:The authorities in the hospitals and EMS administration should improve the barriers which interfere with the efficiency of a paramedic and cause gross humiliation to the patients, and the Health authorities should impart proper education and training to the paramedics.

Estrogen deficiency is a major risk factor for osteoporosis in postmenopausal women. Although hormone replacement therapy (HRT) has been rampantly used to recompense for the bone loss, but the procedure is coupled with severe adverse effects. Hence, there is a boost in the production of newer synthetic products to ward off the effects of menopause-related osteoporosis. As of today, there are several prescription products available for the treatment of postmenopause osteoporosis; most of these are estrogenic agents and combination products. Nevertheless, in view of the lack of effect and/or toxicity of these products, majority of the postmenopausal women are now fascinated by highly publicized natural products. This is an offshoot of the generalized consensus that these products are more effective and free from any adverse effects. Recently, certain plant-derived natural products, mostly phytoestrogens (isoflavones, lignans, coumestanes, stilbenes, flavonoids) and many more novel estrogen-like compounds in plants have been immensely used to prevent menopause-related depletion in bone mineral density (BMD). Although, a number of papers are published on menopause-related general symptoms, sexual dysfunction, cardiovascular diseases, Alzheimer's disease, diabetes, colon, and breast cancers, there is paucity of literature on the accompanying osteoporosis and its treatment. In view of the controversies on synthetic hormones and drugs and drift of a major population of patients toward natural drugs, it was found worthwhile to investigate if these drugs are suitable to be used in the treatment of postmenopausal osteoporosis. Preparation of this paper is an attempt to review the (a) epidemiology of postmenopausal osteoporosis, (b) treatment modalities of postmenopausal osteoporosis by hormones and synthetic drugs and the associated drawbacks and adverse effects, and (c) prevention and treatment of postmenopausal osteoporosis by phytoestrogens, their drawbacks and toxicity. It is apparent that both the categories of treatment are useful and both have adverse effects, but the plant products are nonscientific and hence are not advised to be used till more studies are undertaken to ensure that the benefits clearly outweigh the risk, in addition to recognition by Food and Drug Administration.

Background:Saudi Board of Emergency Medicine (SBEM) graduates are involved in a 1-month rotation in emergency medical services (EMSs) and disaster medicine. The purpose of this study was to evaluate change in knowledge and attitude of EM residents after the introduction of the EMS and disaster medicine rotation.Materials and Methods:The study included 32 3rd-year SBEM residents. A pretest/posttest design and a five-point Likert scale were used. The data included a response to a questionnaire developed by EMS and disaster experts. The questionnaire was distributed on the 1st day of the rotation and 45 days after. Satisfaction questionnaires were distributed after the rotation. The data were analyzed using SPSS 20.Results:Twenty-five residents responded to the satisfaction survey (75%). The overall satisfaction with the course modules was high; the course content showed the highest level of satisfaction (96%), and the lowest satisfaction was for the air ambulance ride outs (56%). The results of the pre-/post-test questionnaire showed an increase of 18.5% in the residents mean score (P < 0.001). In the open-ended section, the residents requested that the schedule is distributed before the course start date, to have more field and hands-on experience, and to present actual disaster incidents as discussion cases. The residents were impressed with the organization and diversity of the lectures, and to a lesser extent for the ambulance ride outs and the mass casualty incident drill l. Seventy-one percent indicated that they would recommend this course to other residents.Conclusion/Recommendation:This study showed that a structured course in EMS and disaster medicine had improved knowledge and had an overall high level of satisfaction among the residents of the SBEM. Although overall satisfaction and improvement in knowledge were significant, there are many areas in need of better organization.

Background: Emergency Medicine physicians are shown to be at increased risk of burnout. In this study, we aimed to assess the risk of burnout among emergency physicians working at one of the largest Emergency Departments in Saudi Arabia.Methods: This is an observational, cross-sectional study based on a structured questionnaire Maslach Burnout InventoryHuman Services Survey (MBI-HSS), which has been previously tested and validated extensively. The study targeted all physicians in the Emergency Department (ED) at a tertiary medical center in Riyadh. A total of 72 emergency physicians were included in the study.Results: Overall, 53 (74%) out of 72 subjects filled the questionnaire. Out of the 53 respondents, 45 (85%) were males and eight (15%) females. The years of practice experience ranged from six months to 24 years, with a median of seven years. Burnout Inventory-Human Services Survey subscale results:Emotional Exhaustion (EE): The mean EE score was 2.72 (SD 1.28), with 21 participants (40%) in the high-risk zone. Depersonalization (DP): The mean DP score was 1.86 (SD 1.31), with 21 participants (40%) in the high-risk zone. Personal Accomplishment (PA): The mean PA score was 4.5 (SD 0.9), with 17 participants (32%) in the high-risk zone. Conclusion:Our results are consistent with previous literature in showing that emergency physicians are at a moderate to high risk of burnout. Decision makers should take serious steps to address the threat, in order to minimize the risk of burnout and its impact on physicians as well as the patient they care for.
