The gastric volvulus is defined as an abnormal rotation of all or part of the stomach around one of its axes, creating the conditions of an upper abdominal obstruction with gastric dilation and risk of strangulation. It is a rare entity that requires a surgical treatment, and its diagnosis is often delayed due to frequently aspecific symptoms. We will describe the observation of a 62 year old patient who presented to the emergency department for acute epigastric pain with dyspnea. The thoracoabdominal CT has demonstrated a stasis stomach on pyloric obstacle evoking a gastric torsion. An upper gastrointestinal endoscopy (EGD) and an upper gastrointestinal contrast made it possible to diagnose an acute gastric volvulus on hiatal hernia. A midline laparotomy was performed with detorsion of the stomach and repair of the hiatal hernia. The patient recovered gradually and was discharged on the sixth postoperative day. Three months after the operation, the patient remained asymptomatic.
Le pseudomyxome péritonéal (PMP) ou l'ascite gélatineuse est une entité rare, décrite pour la première fois par R. Wyerth en 1884. Il correspond à une atteinte péritonéale diffuse, composée d'une ascite mucineuse et d'implants épithéliaux mucineux multifocaux. Cette maladie touche essentiellement les femmes. Son incidence est estimée à 2 cas par un million d'habitants. Le pseudomyxome péritonéal peut rester asymptomatique, découvert lors d'une laparotomie. Le symptôme le plus fréquent est représenté par une distension abdominale associée à des douleurs abdominales diffuses. Le scanner abdominal constitue le moyen diagnostique le plus spécifique. Il met en évidence des signes pathognomoniques de l'ascite gélatineuse. Les tumeurs mucineuses de l'appendice sont la cause la plus fréquente du pseudomyxome péritonéal avec 90% des cas. L'origine ovarienne en constitue une cause très rare. Il existe essentiellement deux types de traitement du PMP: premièrement le « debulking » chirurgical multiple et deuxièmement la chirurgie de cytoréduction avec chimiothérapie intra-péritonéale péri-opératoire consistant en une chimiothérapie hyperthermique intrapéritonéale avec ou sans chimiothérapie intrapéritonéale post opératoire immédiate. Nous rapportons un cas de pseudomyxome péritonéal secondaire à une tumeur mucineuse de l'ovaire gauche.
Le cancer colorectal est classé parmi les cancers les plus fréquents au monde (3ème rang après le cancer du sein et celui de la prostate), deuxième cancer digestif au Maroc après celui de l'estomac. Cependant son incidence dans notre pays reste moins élevée que celle des pays occidentaux (2.5 à 3.3/100 000 ha) mais rejoint celle des pays du Maghreb dont ce cancer touche les sujets jeunes dans 27% des cas. Le cancer colorectal (CCR) est un des meilleurs exemples du processus multi étape de cancérogenèse. La connaissance des caractéristiques anatomopathologique du CCR conditionnera certainement nos attitudes thérapeutiques. Nous avons mené une étude rétrospective épidémiologique et anatomopathologique au service de chirurgie viscérale de l'hôpital militaire Moulay Ismail à Meknès sur une période de 5 ans allant du mois de janvier 2012 au mois de Décembre 2016 (36 cas: 14 femmes et 22 hommes). À la lumière de cette étude, l'objectif de ce travail est d'analyser le profil épidémiologique et les aspects anatomopathologiques des cancers colorectaux. L'analyse de nos résultats montre un profil épidémiologique particulier qui se caractérise par un âge plus jeune, une répartition selon le sexe en faveur des hommes, les carcinomes sporadiques sont largement prédominants avec une répartition topographique recto sigmoïdienne fréquemment observée. Sur le plan pronostique, le faible degré de différenciation des adénocarcinomes et le type mucineux sont corrélés à un stade TNM et d'Aster Coller avancés, avec un statut ganglionnaire péjoratif. Cette approche multidisciplinaire sera une première nationale, rendant ainsi notre structure de pratique et de recherche médicale l'une des plateformes de prise en charge multidisciplinaire du cancer colorectal au Maroc.
L'invagination intestinale est une affection très fréquente en pédiatrie, alors que chez l'adulte, elle reste une entité clinique rare et ne représente que 10% de l'ensemble des invaginations et 4% des occlusions intestinales. Un lymphome colique révélé par invagination intestinale est une entité très rare. Nous rapportons le cas d'une femme de 21 ans, admise aux urgences de l'hôpital Militaire Mouly Ismail à Meknes, Maroc pour une subocclusion intestinale. Le scanner abdominal a montré la présence du boudin d'invagination ainsi que la cause organique au niveau colique. La résection chirurgicale était le traitement choisi, avec un examen anatomopathologique de la pièce opératoire qui montre la présence d'un lymphome B de type diffus à grandes cellules. Après la chirurgie, une chimiothérapie est indiquée dans le but d'améliorer le pronostic et d'éviter une éventuelle rechute.
To perform an epidemiological review of the visceral surgery operative activity of the Juba Field Hospital (JFH). A total of 202 surgical procedures were retained. They represented 38.92% of operative activities. The emergencies were 12 (13.33%). The first condition was hernias 114 (60%) followed by proctologic conditions 45 (23.68%). The average age of patients was 38, 75 years with extremes of 12 and 86 years. There were 150 males (78, 94%) and 40 females (21.05%). The sex ratio is 3.75. The evolution was favorable in 95% of the cases, the postoperative complications were 9.9% dominated by the suppurations of the operating site. Conclusion:The visceral surgery activities are important in Juba with domination of the hernia pathology that must be treated early to prevent complications.
