Introduction: Hospital beds, human resources, and medical equipment are the costliest elements in the health system and play an essential role at the time of treatment. In this paper, different phases of the NEDA 2026 project and its methodological approach were presented and its formulation process was analysed using the Kingdon model of policymaking. Methods: Iran Health Roadmap (NEDA 2026) project started in March 2016 and ended in March 2017. The main components of this project were hospital beds, clinical human resources, specialist personnel, capital medical equipment, laboratory facilities, emergency services, and service delivery model. Kingdon model of policymaking was used to evaluate NEDA 2026 development and implementation. In this study, all activities to accomplish each step in the Kingdon model was described. Results: The followings were done to accomplish the goals of each step: collecting experts’ viewpoint (problem identification and definition), systematic review of the literature, analysis of previous experiences, stakeholder analysis, economic analysis, and feasibility study (solution appropriateness analysis), three-round Delphi survey (policy survey and scrutinization), and intersectoral and interasectoral agreement (policy legislation). Conclusion: In the provision of an efficient health service, various components affect each other and the desired outcome, so they need to be considered as parts of an integrated system in developing a roadmap for the health system. Thus, this study demonstrated the cooperation process at different levels of Iran’s health system to formulate a roadmap to provide the necessary resources for the health sector for the next 10 years and to ensure its feasibility using the Kingdon policy framework.
Objective: Pre-hospital personnels (PHPs) who work in disasters under extreme pressure, uncertainty, and complex situations are victims of disasters themselves, and there is a link between experiencing such incidents and mental health problems. Because most studies focus on the injured and less on the psychological issues of PHPs, the present study aimed to develop a model to provide relief for PHPs in disasters from a psychological perspective.Methods: A grounded theory methodology recommended by Corbin and Strauss (2015) was employed. PHPs (n = 24) participated in a semi-structured interview between July 2018 to May 2020.Results: In the analysis of the pre-hospital staff interviews, three main themes were extracted, namely, providing relief with struggle (complexity of incident scenes, command-organizational and occupational challenges), psychological distress (psychological regression and psychological empowerment), and consequences (resilience and job burnout). Seven categories and 22 subcategories were explored from our data via the grounded theory approachConclusions: The PHPs managed psychological distress with two approaches: psychological self-empowerment and regression, which resulted in resilience and burnout, respectively. Due to the lack of enough support, the resilience of the PHPs was short-term, turned into burnout over time, and affected the structural factors again as a cycle.
Introduction: The organizational citizenship behavior in hospitals is of great importance since it enhances positive relationships between employees and their involvement in organizational activities. On the other hand, culture is one of the main and major components of the organization, which has a direct effect on promoting the organization's activities. In this regard, rich Islamic culture with its pure concepts has a great role in shaping and refining people's behaviors. The purpose of this study was to investigate the effect of Islamic culture on organizational citizenship behavior among nurses in hospitals of Yazd University of Medical Sciences. Methods: In this cross-sectional study, 264 staff of the selected hospitals affiliated with Yazd University of Medical Sciences was selected using two-stage sampling method. The data collection tool was a valid and reliable questionnaire consisting of three sections: demographic information, a 22-item organizational citizenship behavior questionnaire with 5 dimensions, and the Islamic culture questionnaire, which should be answered using a 5-point Likert scale. Data were analyzed using SPSS 21 and SmartPLS software. Results: Among the dimensions of organizational citizenship behavior, the highest mean and standard deviation were related to conscientiousness dimension (3.10±0.69). The total mean and standard deviation was calculated as 2.99±0.57 for the component of organizational citizenship behavior. The mean and standard deviation of Islamic culture was 2.89±0.53. According to the data analysis algorithm in PLS method, by examining the significant coefficients of t for each path, Islamic culture had significant effect on all components of the organizational citizenship behavior. Conclusion: Considering the impact of Islamic culture on organizational citizenship behavior, various cultural programs such as cultural training courses, cultural seminars, and cultural events are suggested to promote Islamic culture in hospitals
ObjectivesReliable and valid information on burden of road traffic injuries (RTIs) is essential for short-term and long-term planning. We designed the present study to describe the levels and trends of burden of RTIs in Iran from 1990 to 2019.MethodsThis is an observational epidemiological study. We used the Global Burden of Disease (GBD) 2019 estimates to report RTIs incidence, prevalence, mortality and disability-adjusted life-years (DALYs) by sex, age group and road user category in Iran and each of the 31 provinces from 1990 to 2019.ResultsAge-standardised incidence, prevalence, death and DALY rates of RTIs decreased by 31.7% (95% uncertainty interval (UI): 29.4 to 33.9), 34.9% (33.8 to 36.0), 57.7% (48.1 to 62.3) and 60.1% (51.7 to 65.2), respectively between 1990 and 2019. The 2019 age-standardised DALY rates varied from smallest value in Tehran 303.8 (216.9 to 667.2) per 100 000 to largest value in Sistan-Baluchistan 2286.8 (1978.1 to 2627.9) per 100 000. The burden of RTIs was mainly related to injuries sustained by drivers or passengers of motorised vehicles with three or more wheels and pedestrians’ injuries, mostly affected males aged 15–29 years and individuals aged ≥70 years.ConclusionThe reducing trend in the burden of RTIs in Iran possibly reflects the effectiveness of the intervention programmes. However, with regard to the Sustainable Development Goals the burden is still at an alarming level. Further reductions are necessary for specific road user groups such as adolescent and adult male drivers or passengers of motorised vehicles, also pedestrians aged ≥70 years.
