Evidence in support of acellular dermal matrix use in head and neck reconstruction is generally poor. Prospective comparative studies are required to define the indications, safety and effectiveness of acellular dermal matrices in laryngotracheal and pharyngeal reconstruction.
Nine patients received systemic MTX and the other three underwent intracervical MTX treatment. Mean Serum hCG levels showed 97% (range, 83.0-99.9) drop within 1 months. Rebleeding after embolization occurred in three (25%), and the bleeding ceased after secondary embolization in all the rebleeding cases. There was no major procedure-related complication. Conclusions: UAE combined with MTX is a safe and effective modality for the treatment of ectopic pregnancy.
This syndrome has been described in association with a variety of stress situations. Once again, this may result in circulatory failure mimicking myocardial infarction or myocardial contusion as in this patient.Almost always this syndrome occurs in post-menopausal women and the aetiology is most probably due to the combination of catechol surge in conjunction with an oestrogen-deficient state causing vasoconstriction of the myocardial microvascular system with severe myocardial dysfunction and possibly cardiomyotoxicity. 2 The apex of the ventricle is predominately affected because of the density of beta receptors in this area. The injury is reversible, and when echocardiograms are repeated, they return to normal.Most reported patients require only conservative measures with temporary pharmacological support. 2 Patients with cardiogenic shock should be treated with non-adrenergic ionotropes (milrinone or amrinone) or calcium-sensitizing agents (levosimendan). If shock persists, the intra-aortic balloon pump should be employed. Some authors have even suggested that the balloon pump be used as a first-line method for haemodynamic support in such patients. 7 We believe that our patient is the first reported case of the Takotsubo syndrome associated with trauma and cardiogenic shock requiring intra-aortic counter pulsation and resulting in survival.
