Zusammenfassung Die Behandlung von Fingerfrakturen ist nach wie vor eine Herausforderung f?r den jeweiligen Behandler. Grunds?tzlich gilt, dass die fr?hzeitige Diagnosestellung und Behandlung von Fingerfrakturen Voraussetzungen f?r ein gutes funktionelles Ergebnis sind. Aufgrund der verschiedenen Frakturformen und Frakturlokalisationen sowie der unmittelbaren N?he zum komplexen Sehnenapparat und zu den Gef??-Nerven-B?ndeln gibt es zahlreiche konservative und operative Behandlungsverfahren. W?hrend stabile nichtdislozierte Frakturen eine Dom?ne der konservativen Bruchbehandlung darstellen, sind instabile dislozierte Frakturen der Phalangen operativ zu behandeln 4 8 33. Bei der Wahl des Osteosyntheseverfahrens sollte nicht nur die Frakturform und -lokalisation, sondern insbesondere auch die funktionelle Anatomie der Finger ber?cksichtigt werden. Ziel einer jeden Osteosynthese sollte neben der anatomischen Reposition auch eine stabile Retention sein, welche eine fr?hzeitige Mobilisation der Finger erm?glicht. Nur so kann in der ergo- und physiotherapeutischen Nachbehandlung, als weiterer zentraler Baustein in der Behandlung von Fingerfrakturen, ein gutes funktionelles Ergebnis erzielt werden 9. Die Kirschner-Draht-Schrauben und die Plattenosteosynthese sind derzeit die bevorzugten Verfahren. W?hrend die Kirschner-Dr?hte sehr weichteilschonend eingebracht werden k?nnen, weisen sie aufgrund unzureichender kortikaler Verankerung h?ufig eine zu geringe Frakturstabilit?t f?r eine fr?hfunktionelle Nachbehandlung auf. Platten- und Schraubenosteosynthesen dagegen f?hren insbesondere bei Frakturen ohne Knochenverlust zu einer sehr hohen Stabilit?t. Allerdings ist die f?r die Implantation zus?tzlich notwendige Sch?digung des vom Trauma selbst schon in Mitleidenschaft gezogenen Weichteilmantels ein nicht unerhebliches Problem, welches das funktionelle Ergebnis, insbesondere bei der Plattenosteosynthese, negativ beeinflussen kann. Es ist bisher nicht gekl?rt, wie viel Stabilit?t ?berhaupt f?r eine fr?hfunktionelle Nachbehandlung notwendig ist. Obwohl Fingerfrakturen noch lange im R?ntgenbild sichtbar sind, besteht im Allgemeinen schon nach 3???4 Wochen, unabh?ngig vom Behandlungsverfahren, eine ausreichende ?bungsstabilit?t. Anhaltspunkt hierf?r kann der Druckschmerz im Frakturbereich sein. Ist dieser verschwunden, kann man von einer ausreichenden Frakturstabilit?t f?r eine funktionelle Nachbehandlung ausgehen. Im Allgemeinen kann bei einfachen Frakturen ein gutes funktionelles Ergebnis erreicht werden. Bei Komplexverletzung dagegen, insbesondere mit Weichteilsch?digungen, ist in der Regel mit einem schlechteren funktionellen Ergebnis zu rechnen.

The Austrian Landfill Ordinance ( BGBL 169/96, 1996) sets standards for the types of waste which are allowed to be landfilled. There are limits for total organic compound (TOC), ignition loss (IL), calorific value and many other parameters describing the potential of harmful substances but no parameters checking the biological reactivity of waste. Experimental determination of gas‐generating potential is well suited for the assessment of this reactivity because such methods (an incubation test developed by the Austrian Department of Waste Management [ABF‐BOKU]), allow researchers to reproduce the majority of ‘natural conditions’ in the laboratory. The problem with anaerobic tests is that the period over which the tests run tends to be long, which is why aerobic tests were also carried out. To reduce gas generating potential to less than 10%, a gas generation GS90 ≤20 Nl kg–1 dry substance (DS) Nl kg–1 DS is to be maintained. This research shows that the correlating respiration activity AT7 is between 12 and 15 mg O2 g–1 DS.

It could be shown that the distal ulnar shortening osteotomy by a transverse osteotomy using a compression device and an ulnodorsal approach has a low complication rate. It is comparable to the oblique osteotomy in effectiveness and safety.

Vascularized bone transfer is increasingly recognized as a very useful and versatile technique for reconstructing large bone defects. It is especially indicated in patients with challenging conditions, such as nonunions, postoncologic resections, severe trauma, and congenital defects. With increasing experience of reconstructive surgeons, improved microsurgical techniques, and more enhanced flaps, the failure rate of free flaps has been reduced in the last decades. Especially, the early-postoperative period is decisive in the outcome of surgery. Several techniques have been described for monitoring the postoperative viability of microsurgical free vascularized graft, and their sensitivity and reliability continues to be the object of animal experiments and clinical trials. The qualities of an optimal monitoring device should be objective, continuous, noninvasive, safe, reproducible, easily managed, and interpretable for the nursing staff, inexpensive, and a clear indicator of changes in arterial and venous circulation. Presently, no one neither fulfills all of the criteria completely nor is uniformly accepted. In this article, relative advantages and disadvantages of these various postoperative monitoring techniques are discussed.
