Objective. To share the experience of endovascular interventions in pregnant women and women in labor, gained by national team “obstetric cardiology”. Materials and methods. The algorithm of medical support was presented, including the cases of cardiosurgical interventions performance , for pregnant women with critically severe pathology of the heart, which was based on actual recommendations and own experience. Into the clinic of Amosov National Institute of Cardiovascular Surgery during 6.5 yrs (12.2013 - 05.2020) 145 women-patients were admitted to hospital, in 79 of them cardiosurgical intervention was done. Endovascular cardiosurgical operations were performed in 36 patients, and the hybrid - in 2. Urgent and emergency endovascular interventions were performed in 17 women-patients with various terms of pregnancy (15 - 35 weeks) and in 2 women in labor. Conditionally-elective cardiosurgical interventions were performed in 17 women-patients on various terms of pregnancy (as a rule, in II trimester) and in 2 women in labor. In 34 patients the interventions were performed with fetus in utero. Early and late maternal mortality was absent. One «programmed» loss of the fetus have occurred on 12-th week of pregnancy with the inborn heart failure. Modern views on impact of ionized radiation and contrast substances on fetus were presented, and the methods of lowering of their potential harm. Conclusion. There was trusted, that while applying strict indications for performance of endovascular interventions during pregnancy, these methods of diagnosis and treatment are more secure in comparison with «open» operative interventions and may be performed under supervision of experienced multidisciplinary team, taking into account the pregnancy term and the fetus defense.
GUCH-пацієнти – окрема когорта пацієнтів, які зазвичай мають певний ступінь серцевої недостатності, підвищений ризик розвитку інфекційного ендокардиту і синдрому раптової серцевої смерті та потребують додаткових втручань на серці. Експерти Європейського товариства кардіологів (ESC) розглядають стосовно них також питання медичного супроводу вагітності, пологів і післяпологового періоду. В Україні з 2013 року функціонує мультидисциплінарна команда з фахівців НІССХ імені М. М. Амосова та Інституту ПАГ НАМН України. Консультативну допомогу за цей час надано 2815 вагітним, 116 проліковано в стаціонарі. До групи GUCH-пацієнток увійшли 64 жінки, з яких 61 – вагітна, 3 – породіллі. У 53 прооперованих жінок загалом було проведено 42 втручання на серці та судинах, 18 кесарських розтинів. Серед операцій на серці та судинах були «закриті» та ендоваскулярні втручання (n=23), операції зі штучним кровообігом (n=19). У плановому порядку проведено 23 кардіохірургічні операції, в екстреному та невідкладному – 19. Одна материнська і дві фетальні втрати спостерігалися в групі вагітних (породілей), що підлягали екстреним втручанням зі штучним кровообігом. Більше ні материнських втрат, ні фетальних втрат не було. В усіх інших прооперованих жінок (n=47) вагітність закінчилася без ускладнень. Сучасний рівень надання спеціалізованої допомоги GUCH-вагітним значно знижує ризик смертності та інвалідизації матері та дитини. Першою лінією захисту GUCH-жінки фертильного віку є первинна профілактика великих кардіальних подій – її предгравідарна підготовка до вагітності, мультидисциплінарний супровід такої жінки протягом усієї вагітності, пологів і післяпологового періоду.
Objective: to increase the safety of a patients with cardiovascular pathology during the implementation of their reproductive function, by presenting a modern strategy of medical support for such women based on the assessment of the maternal and perinatal risks.Cardiac pathology is the leading cause of morbidity in pregnant women and an indirect cause of maternal mortality. In recent decades, the focus on the problem of safety of pregnant women with the pathology has been steadily increasing. Stratification of patients by cardiovascular risk (CVR) consists of assessing the functional capacity of the patient, cardiac medications needs, history of previous cardiac events, diagnosis of structural, anatomical, and physiological changes and parameters of heart remodeling, and, if necessary, genetic counseling. An important point is to inform the patient about the potential risks and make a consensus decision with her about the possibility of safe implementation of her reproductive function. The authors of the paper analyze the CVR assessment scales used in leading medical practices and the national health care system as well, discuss current clinical guidelines for risk stratification and registered strategies for pregnancy and delivery management depending on the class of CVR in such patients. This paper presents the basic principles of medical support for women of reproductive age with pathology of the heart and great vessels in the form of an algorithm developed by the National pregnancy heart team. Determination of the degree of risk allows to prepare a woman with cardiovascular pathology for the pregnancy and delivery (medical correction or preventive cardiac surgery). Research on the identification and categorization of maternal and perinatal CVR factors and stratification of pregnant women by risk classes are extremely relevant and continue in the framework of the international study ROPAC (Registry Of Pregnancy And Cardiac disease).Ukrainian multidisciplinary team (on the base of two academic State Institutions – “National Amosov Institute of Cardiovascular Surgery” and “Institute of Pediatrics, Obstetrics and Gynecology named after acad. O.M. Lukianova”) takes an active part in this international collaboration.
