Case report The present case series describes the clinical course and outcome of three cats diagnosed with pseudomembranous cystitis. This is an uncommon presentation of lower urinary tract obstruction but can be easily be identified by ultrasonography, revealing severe bladder wall thickening and thin hyperechoic luminal strips. The condition can be secondary to severe bacterial urinary tract infection. All cats were successfully treated with medical management only, mainly based on antimicrobials and individualised supportive therapy.Conclusion Further evaluation of this condition is necessary in order to determine potential underlying aetiologies, pathophysiological mechanisms and the most appropriate standardised treatment.
Background Antiviral prophylaxis is recommended in cytomegalovirus (CMV)-seropositive kidney transplant (KT) recipients receiving antithymocyte globulin (ATG) as induction. An alternative strategy of premature discontinuation of prophylaxis after CMV-specific cell-mediated immunity (CMV-CMI) recovery (immunoguided prevention) has not been studied. The aim of this study was to evaluate whether it is effective and safe to discontinue prophylaxis when CMV-CMI is detected and to continue with preemptive therapy. Methods In this open-label, non-inferiority clinical trial, patients were randomized 1:1 to follow immunoguided strategy, receiving prophylaxis (valganciclovir 900 mg daily) until CMV-CMI recovery or to receive fixed-duration prophylaxis until day +90. After prophylaxis, preemptive therapy (valganciclovir 900 mg twice daily) was indicated in both arms until month 6. The primary and secondary outcomes were incidence of CMV disease and replication, respectively, within the first 12 months. Desirability of outcome ranking (DOOR) assessed two deleterious events (CMV disease/replication and neutropenia). Results A total of 150 CMV-seropositive KT recipients were randomly assigned. There was no difference in the incidence of CMV disease (0% vs. 2.7%; P = 0.149) and replication (17.1% vs. 13.5%; log-rank test, P = 0.422) between both arms. Incidence of neutropenia was lower in the immunoguided arm (9.2% vs. 37.8%; OR, 6.0; P < 0.001). A total of 66.1% of patients in the immunoguided arm showed a better DOOR, indicating a greater likelihood of a better outcome. Conclusions Prophylaxis can be prematurely discontinued in CMV-seropositive KT patients receiving ATG when CMV-CMI is recovered since no significant increase in the incidence of CMV replication or disease is observed.
Leptospira is a widespread zoonosis that has been linked to transmission between dogs and humans. The main purposes were to determine the seroprevalence of anti- Leptospira serum antibody and to identify the most common serovars in dogs in Spain. This is a cross-sectional study with 1,310 records of canine Leptospira testing data from Spain since 2015 to 2017. Inclusion criteria were individual cases with MAT test results for 8 serovars (Bratislava, Icterohaemorrhagiae, Australis, Pomona, Grippotyphosa, Autumnalis, Canicola and Saxkoebing) and to have the zip code data. Three hundred and thirty-eight samples (25.8%; 95%CI 23.6–28.4) were seropositive (≥1:100). According to geographic areas, North had the highest seroprevalence (38.0%; 95%CI 28.9–47.1) followed by South (29.4%; 95%CI 20.1–38.8), Center (28.6%; 95%CI 24.3–33.0), Mediterranean (22.3%; 95%CI 19.1–25.6) and Northwest (22.2%; 95%CI 7.9–36.4). Seropositivity (MAT ≥1:100) was most common to serovars Icterohaemorrhagiae (19.4%; 95%CI 17.2–21.5) and Bratislava (8.5%; 95%CI 7.0–10.0), followed by Grippotyphosa (7.2%; 95%CI 5.8–8.6), Australis (6.4%; 95%CI 5.0–7.7), Autumnalis (5.0%; 95%CI 3.8–6.2), Pomona (4.5%; 95%CI 3.3–5.6), Canicola (3.4%; 95%CI 2.4–4.4) and Saxkoebing (0.8%; 95%CI 0.3–1.3). An association was found between positivity (MAT ≥1:100) and males (P = 0.003) and dogs that were 6 years old or older were at higher risk of exposure (P = 0.001; OR 4.61; 95%CI 1.86–11.43). This study has shown that dogs in Spain are commonly exposed to Leptospira infection and points out the necessity to control the prevalence of this severe widespread zoonosis in dogs and humans.

Western societies are facing a clear increase in the rate of obesity and overweight which are responsible for musculoskeletal pain. Some of the substances described in the environment of myofascial trigger points (MTrPs) are the same as those found in the skeletal muscle of obese people, such as cytokines. Furthermore, elevated neuromuscular neurotransmission has been associated with MTrPs. The main objective of this study is to assess whether obesity or overweight may be a facilitator of myofascial pain. The experiments were performed on male Swiss mice. One experimental group was given a typical “cafeteria” diet and another group a commercial high-fat diet for six weeks. Intramuscular adipocytes were assessed with Sudan III. The functional study was performed with electromyographic recording to determine the plaque noise and intracellular recording of miniature endplate potentials (MEPPs). The intake of a cafeteria diet showed the presence of more adipocytes in muscle tissue, but not with the fat-supplemented diet. Both experimental groups showed an increase in the plaque noise and an increase in the frequency of MEPPs that lasted several weeks after interrupting diets. In summary, the supply of a hypercaloric diet for six weeks in mice increases spontaneous neurotransmission, thus facilitating the development of MTrPs.
Computed tomography has been used in human medicine to evaluate the thyroid gland functional status by means of Hounsfield units (HU). Studies describing attenuation value abnormalities, other than those in thyroid neoplasia, lack in veterinary medicine. The authors have observed a subjective reduction in thyroid attenuation in some patients undergoing CT for reasons other than thyroid disease. This finding has been identified more frequently in brachycephalic breeds compared to non-brachycephalic breeds. In order to determine the thyroid gland attenuation variability on CT, a retrospective and prospective, analytical, cross-sectional study was performed. The thyroid attenuation values of 65 client-owned dogs were assessed using a 16-slice helical CT scanner. Differences in attenuation between brachycephalic and non-brachycephalic dogs were estimated by Welch's t-test. Serum TT4 and TSH levels were available in 26 patients. Statistically significant differences were observed in the pre-contrast attenuation value ranges between brachycephalic and non-brachycephalic dogs (P = .04). The pre-contrast attenuation value range for the brachycephalic group was 69.1-108 HU while being 75.8-121 HU for the non-brachycephalic group. No significant correlation was found between thyroid attenuation and serum thyroid hormone levels in our population (P > .6). All patients with hypoattenuating thyroid were brachycephalic and reported euthyroid. In conclusion, brachycephalic dogs seem to have a pre-contrast attenuation thyroid gland range lower than non-brachycephalic dogs. The present study contributes to the veterinary literature by providing a new thyroid attenuation value range for brachycephalic breeds. Additionally, hypoattenuating thyroid glands may be found in dogs with normal TT4 and TSH values.
