The article introduces two clinical cases of successful treatment of Crohn’s disease (CD) of the upper gastrointestinal tract (UGT) with TNF inhibitors. Besides that, the article presentes the review, devoted to diagnostics and treat the same injury of patients with CD. It should be mentioned, that number of publications in worldwide and national literature is quite insignificant. One fact that is more important is the unavailability of unificational approach to treatment of such group of patients. We suppose that epidemiological research is necessary to define the true prevalence of lesions of the upper gastrointestinal tract in CD, risk factors and prognosis for this patient group, and to output the diagnostic and therapeutic tactics.
AIM: to evaluate the impact of dietary compliance in bowel cleansing for colonoscopy on the quality of the procedure.PATIENTS AND METHODS: one thousand patients who scheduled for diagnostic colonoscopy were included in the prospective observation study. The quality of bowel preparation was assessed using the Boston scale. Neoplasms were detected and endoscopically evaluated. The optical verification of tumors was used to calculate the indicators of identified adenomas and polyps (ADR and PDR).RESULTS: the quality of bowel cleansing by the Boston Scale was 6 (6; 8) points. One hundred eight (19.8%) patients did not follow the recommended diet. Poor preparation, which did not allow a total colonoscopy was found in 91 (9,1%) cases. The ADR was 37.4%, PDR — 43.4%. Logistic regression analysis showed that the noncompliance for diet recommendation was the only one significantly negative factor associated whit inadequate bowel cleansing.CONCLUSION: the leading factor worsening the quality of bowel cleansing was non-compliance with the prescribed diet before the colonoscopy.
AIM to show on the results of our study the influence of the quality of preparationfor videocapsule endoscopy (VCE) on the accuracy of diagnosis of diseases of the small and large intestine. MATERIALS AND METHOD. During the period from September 2014 to December 2016, a videocapsule study was performed on 100 patients of treatment at the State Scientific Center of Coloproctology. Final data processing was carried out based on the results of a survey of 96 patients (52 men and 44 women aged 18 to 78 years). To adequately prepare patients for the study, we used a specific scheme with the use of a «splitdose» of intestinal cleansers based on polyethylene glycol and stimulation solutions. The quality of preparation of the small and / or large intestine for the study was assessed according to the scale of Leighton J.A., Rex D.K. RESULTS. The completeness of the study and the full examination (the ability to visualize all the sections of the small and / or large intestine) are important criteria for conducting the VCE. In our study, a complete study of the small and large intestine was performed in 87 (91 %)patients, and a full study was performed in 86 (90 %) of 96 patients. In the study of the small and large intestine using small intestine or large intestinal videocapsules, in 87 (91 %) patients the intestinal preparation was rated «good» or «excellent». CONCLUSION. The experience of our study showed that in order to obtain reliable and informative results, careful follow-up of the methodology of preparation for the study should be done.
AIM: to evaluate the efficacy, safety and drug tolerance of «Colokit» for bowel cleansing before colonoscopy.PATIENTS AND METHODS: an open prospective non-randomised study of the quality of bowel cleansing was carried out in 30 patients (17 females, aged 26-72 years), who underwent diagnostic colonoscopy after using «Сolokit» (Mayoly Spindler, France) using two different regimes (recommended and alternative).RESULTS: the quality of the bowel cleansing was significantly better in patients after «Colokit» use in recommended regime vs alternative regime. No difference in subjective assessment of patients’ comfort during bowel preparation was found in both regimes. CONCLUSION: the «Colokit» agent provides good preparation and patients’ comfort and can be recommended for the bowel cleansing before colonoscopy.
