IL TEST DI LIDDLE IN SOGGETTI AFFETTI DA DIABETE MELLITO ANTONIO TORRI MASSIMO BONI PREMESSELo studio della funzionalitä surrenalica nel diabete, da noi intrapreso da qualche anno con modalitä metodologiche diverse s, 9, nonchè l'accostamento dei nostri dati con quelli della letteratura, ci hanno portato a questi convincimenti: 1 ) il diabete florido tardivo o senile e il diabete magro precoce o giovanile presentano espressioni ormonali totalmente diverse (ToRRI e coll, S); 2) il surrene, in corso di malattia diabetica, appare funzionalmente interessato soprattutto quando essa sia complicata da scompenso acidosico (con atteggiamento iperfunzionale 8) o quando richieda un protratto trattamento insulinico (con evidente ipofunzione 9); 3) la compromissione funzionale del surrene nel diabetico sembra secondaria a fattori molteplici, quali lo squilibrio metabolico, le complicanze infettive o vascolari, la terapia insulinica, e, probabilmente, ad altri fattori, capaci di agire come stress atti a sovrastimolare, entro certi limiti di tempo e gravitä, il surrene, o di portarlo ad una sorta di esaurimento dopo un decorso di anni con particolare gravitä. Con il presente contributo segnaliamo i risultati del test di LIDDtE (risposta corticotropinica al metopirone) da noi ottenuti in 8 soggetti diabetici.
METODOLOGIAAbbiamo distinto la nostra casistica in due gruppi in rapporto allo stato di equilibrio o squilibrio metabolico della malattia diabetica in atto.
