We studied 151 patients undergoing total hip or knee arthroplasty, or cruciate ligament reconstruction in a multicentre study in Australia and New Zealand. Patients were openly allocated randomly to one of five treatment groups or to a control group. General anaesthesia was induced after introduction of extradural block with 0.5% ropivacaine. After surgery, patients received an extradural infusion of 0.2% ropivacaine at 6, 8, 10, 12 or 14 ml h-1 or received no postoperative extradural infusion (control group). All patients had access to i.v. PCA morphine for supplementary analgesia. Morphine consumption was lower in all treatment groups compared with the control group, decreasing with increasing ropivacaine infusion rate. Median VAS scores were lower in all ropivacaine infusion groups compared with the control group for the first 10 h of the study; however by the end of the study, VAS scores were similar in all groups. The higher ropivacaine infusion rates caused a slower convergence of spread of the initial sensory block and a higher degree of motor block. The overall incidence of side effects was similar, with the exception of a higher incidence of urinary retention and hypotension in the groups receiving the higher rates of ropivacaine. The quality of treatment scores were similar for all treatment groups (Br. J. Anaesth. 1996; 76: 606-610).
Objective:
In developing countries, such as Bangladesh, the live bird market (LBM) is a vital location for the trading of live poultry. The study was carried out in nine LBMs located around Bangladesh to ascertain the present regulations and procedures governing their operation. Additionally, the responsibilities and levels of engagement of the stakeholders were determined.
Materials and Methods:
The data were gathered through the use of a semi-structured interview guide. Thematic analysis was used to code the interview transcripts iteratively.
Results:
The findings indicated that the government was directly and indirectly involved in the leasing process of the markets. A market in this country is divided into numerous sectors, including LBM, fish market, vegetable market, and grocery stores. A market’s hygienic condition is highly dependent on market authority’s decisions. In some markets, market officials conducted routine sanitary inspections. Veterinarians played a little role in the inspection procedure.
Conclusion:
There is no adequate, functional monitoring system to ensure that LBMs adhere to cleanliness and adequate and functional biosecurity. Biosecurity enhancements, effective cleaning programs, and regular monitoring by relevant authorities are critical for LBMs in Bangladesh.
Following the spread of COVID-19, online education/distance learning (OE/DL) was opted by the governments across the world. However, many challenges remained unexplored as OE/DL was quite an unfamiliar teaching approach, particularly in Bangladesh. This study was aimed at identifying the major challenges of OE/DL in Bangladesh from the perspectives of students. Data for this web-based cross-sectional study were collected using an e-questionnaire based on Google Form. Out of 451 initial responses, 419 were retained based on some specifications. The univariate analyses identified the financial, technical, and pedagogical issues as major problems. Exploratory factor analysis and confirmatory factor analysis suggest a two-factor solution – ‘financial-technical obstacles’ and ‘pedagogical obstacles’ – with overall good reliability (Cronbach’s α = 0.881; McDonald’s ω = 0.880). The two-factors, with strong intercorrelation (r = 0.552), meet the criteria of discriminant validity. To overcome the shortfalls of OE/DL in Bangladesh, inclusiveness with sustainable educational and infrastructural approaches was recommended.
Background: Propofol is an intravenous (IV) anesthetic agent, can irritate the skin, mucous membrane and venous intima. The main drawback is the pain at injection site following its intravenous injection.
Objectives: This study was performed to evaluate the effect of intravenous nitroglycerine on pain in patients following propofol injection.
Materials and Methods: Eighty adult patients of both sexes, aged 20-50 years, according to American Society of Anesthesiologists (ASA) physical status were divided into two equal groups (n=40) to receive 200 mcg intravenous nitroglycerine diluted in 10 ml saline (group A) and 10 ml normal saline as placebo (group B) at an ambient operating room temperature in a randomized and double blinded fashion to compare the pain-relieving effects of the drugs during propofol injection before the patients lost consciousness. The pain on propofol injection was assessed according to the Mc Crirrick and Hunter scale.
Results: The overall incidence and severity of pain were significantly less in Groups A (nitroglycerine group) than group B (placebo group) (p< 0.05). The incidence of mild and moderate pain in Group A versus group B was 25% vs 45% and 15% vs 30% respectively (p<0.05). The incidence of score '0' (no pain) was higher in Group A (60%) than Group B (25%) (p<0.05).
Conclusion: Pretreatment with 200 mcg nitroglycerine with venous occlusion for one minute is effective pretreatment in alleviating propofol injection pain when compared to placebo.

A systematic literature review was conducted to summarize the overall thermal performance of different gasified cooking stoves from the available literature. For this purpose, available studies from the last 14 years (2008 to 2022) were searched using different search strings. After screening, a total of 28 articles were selected for this literature review. Scopus, Google Scholar, and Web of Science databases were used as search strings by applying “Gasifier cooking stove” AND “producer gas cooking stove” AND “thermal performance” keywords. This review uncovers different gasified cooking stoves, cooking fuels, and fabrication materials besides overall thermal performances. The result shows that the overall thermal performance of different gasified cooking stoves was 5.88% to 91% depending on the design and burning fuels. The premixed producer gas burner with a swirl vane stove provided the highest overall thermal performance range, which was 84% to 91%, and the updraft gasified stove provided the lowest performance, which was 5.88% to 8.79%. The result also demonstrates that the wood pellets cooking fuel provided the highest thermal performance and corn straw briquette fuel provided the lowest for gasified cooking stoves. The overall thermal performance of wood pellets was 38.5% and corn straw briquette was 10.86%.
