Summary. Clinical and epidemiological aspects of 41 cases of mycetoma seen in São Paulo city from January 1978 to December 1989 are presented. Twenty‐eight cases (68%) were caused by actinomycetes, while 13 (32%) were caused by true fungi. In only 22 (78%) actinomycetoma cases was it possible to identify the agent. Nocardia brasiliensis, by far the commonest actinomycete isolated, was responsible for 13 cases. Among eumycetomata, Madurella grisea was isolated from 3 cases, Scedosporium apiospermum from 2 cases, and Madurella mycetomatis from 1 case, and in 7 cases the agent was not identified. Distribution by sex and age was similar to that reported in the literature. Feet and legs were the commonest sites affected. The mean age of actinomycetoma patients was 33.2 years, while patients with eumycetoma were an average of 32.8 years. Mean duration of the disease was 6.8 and 9.8 years respectively. Most patients came from rural areas in the north‐eastern region of the country and worked as field labourers. Bone involvement was frequent for both actinomycetomata and eumycetomata and was characterized by osteoporosis, periostal reaction and cavitation. The authors' elected treatment consists of combining sulfamethoxazole (800 mg) and trimethoprim (100 mg) with prednisone (10 mg) orally, daily. Zusammenfassung. Es werden die klinischen und epidemiologischen Aspekte von 41 Myzetompatienten vorgestellt, die in São Paulo in der Zeit von Januar 1978 bis Dezember 1989 beobachtet wurden. Achtundzwanzig Fälle wurden durch Aktinomyzeten verursacht (68%), 13 durch Eumyzeten (32%). Nur bei 22 Aktinomyzetomen (78%) war es moglich, den Erreger zu identifizieren. Nocardia brasiliensis stellte mit 13 Fällen den größten Anteil. Bei den Eumyzetomen wurde Madurella grisea dreimal, Scedosporium apiospermum zweimal und Madurella mycetomatis einmal isoliert, in sieben Fällen konnte der Erreger nicht identifiziert werden. Die Geschlechts‐ und Altersverteilung entsprach den Literaturangaben. Die unteren Extremitäten waren die am häufigsten befallenen Bereiche. Das Durchschnittsalter der Aktinomyzetom‐Patienten lag bei 33.2 Jahren, das der EumyzetomPatienten bei 32.8 Jahren. Die mittlere Dauer der Krankheit lag bei 6.8 Bzw. 9.8 Jahren. Die meisten Patienten kamen von ländlichen Gegenden der Nordostregion des Landes, bei der Mehrzahl handelte es sich um Landarbeiter. Knochenbeteiligung war häufig und war charakterisiert durch Osteoporose, periostale Reaktionen und Kavitätenbildung. Die Autoren behandelten mit Sulfamethoxazol (800 mg) und Trimethoprim (100 mg) kombiniert mit Prednison (10 mg) täglich oral.
São registrados três casos de feo-hifornicose subcutânea em transplantados renais provocados pela Exophiala jeanselmei (Langeron) McGinnis et Padhye 1977, fungo demácio capaz, também, de produzir raramente eumicetoma de grãos pretos. Este fungo, segundo KWON-CHUNG & BENNETT, 1992(27) é antigenicamente muito heterogêneo, sendo identificados até o presente momento três sorotipos com subgrupos dentro de cada um deles. A feo-hifomicose subcutânea vem se tornando cada vez mais freqüente em transplantados renais, submetidos a terapêutica imunodepressora. Como a Exophiala jeanselmei já foi isolada do meio ambiente, torna-se dificil explicar a patogenia desses casos por um despertar ou reativação de processos quiescentes. Os Autores fizeram ampla revisão da literatura, registrando principalmente os casos de feo-hifomicose publicados no Brasil. Sugerem também, eventual ação fungistática da ciclosporina A sobre a Exophiala jeanselmei.
É descrito um caso de entomoftoromicose rinofacial por Conidiobolus coronatus, em paciente procedente de São Paulo (Capital), tratado com sucesso pelo fluconazol, por via oral, através terapêutica prolongada. Dados histopatológicos e micológicos confirmaram o diagnóstico.
Summary: This paper reports a case of disseminate sporotrichosis with cutaneous, mucous, osseous, and ocular lesions, a fact rarely registered in the literature, mainly because it happened in Brazil where this mycosis is frequent. The cutaneous and mucous lesions were severe, causing irreversible sequelae. Of special gravity was the ocular lesion resulting in blindness of the affected eye. The study showed no altered delayed immunity. The possibility of an aereal inoculation of the agent is discussed. The experiments with test animals do not demonstrate special virulence of the strain isolated from the patient. The response to the iodine treatment was unsatisfactory, as was the response to ketoconazole. The response to amphotericin B was considered satisfactory. Zusammenfassung: Die Arbeit bezieht sich auf einen Fall von Sporotrichosis disseminata mit Haut‐, Schleimhaut‐, Knochen‐ und Augenläsionen. Ein Vorfall selten in der Literatur registriert, bemerkenswert, da er in einem Land, Brasilien, vorkam, wo Mykose eine alltägliche Erscheinung ist. Die Haut‐ und Schleimhautläsionen waren heftig mit unabänderlichen Folgeerscheinungen. Von besonderer Schwere war die Augenläsion, die zur Erblindung des befallenen Auges führte. Der Befund verzögerter Immunität war normal. Diskutiert wurde die Möglichkeit einer Einimpfung des Erregers. Die Teste mit Versuchstieren zeigen keine spezielle Virulenz des vom Patienten isolierten Erregers. Die Reaktion auf Jodbehandlung war nicht zufriedenstellend, wie auch die Reaktion auf Ketoconazole nicht den Erwartungen entsprach. Die Behandlung mit Amphotericin konnte als zufriedenstellend angesehen werden.
FUNDAMENTOS: A criocirurgia com nitrogênio líquido (C-N2L) é um método terapêutico que vem sendo usado com freqüência crescente no tratamento da cromomicose. Acreditava-se anteriormente que as temperaturas baixas poderiam destruir o agente infectante, mas foi demonstrado que as culturas fúngicas em temperaturas tão baixas como -196°C não causaria a morte do mesmo. Apesar da comprovada eficácia, ainda não se conhece o exato mecanismo de cura. OBJETIVO: Avaliar o período de persistência de fungos viáveis em lesões de cromomicose tratadas pela C-N2L. PACIENTES E MÉTODOS: Cinco pacientes com cromomicose tiveram suas lesões tratadas pela C-N2L. Foram colhidos, em diferentes intervalos de tempo após a criocirurgia, fragmentos do tecido tratado. A coleta, realizada com punch de 4mm, foi feita em três períodos diferentes: de 0 a 48h, de cinco a sete dias e de 10 a 14 dias após a realização da criocirurgia. Os fragmentos obtidos foram inoculados em Agar Sabouraud para verificação de crescimento de colônias fúngicas. Cada paciente teve um total de três amostras colhidas, uma em cada um dos três períodos mencionados. RESULTADOS: O crescimento de colônias foi maior nas coletas mais precoces, enquanto nas amostras colhidas entre o pós-operatório imediato e o quinto dia de pós-operatório o índice de viabilidade foi de 7/8 (87,5%), e naquelas colhidas a partir do sexto dia de pós-operatório foi de apenas 2/7 (28,5%). O maior período de pós-operatório que demonstrou positividade foi de 12 dias. CONCLUSÃO: Os resultados confirmam os achados anteriores, os quais demonstraram que as baixas temperaturas alcançadas pela C-N2L não são responsáveis pela destruição dos fungos nas lesões de cromomicose. Os autores acreditam que fenômenos biológicos tardios, como necrose ou imunoestimulação sejam os verdadeiros responsáveis pela erradicação dos fungos nas lesões de cromomicose.
Summary. Black piedra or nodular tricho‐mycosis is a superficial mycosis of the hair shaft, prevalent in the tropical areas of South America and Asia and caused by the fungus Piedraia hortai. By scanning electron microscopy the invasion of the hair keratin and the elimination of spores through canals was seen. Transmission electron microscopy showed a complex, well‐organized microsystem, in which cement‐producing hyphae constitute the pseudoparenchyma. Both sexual and asexual reproduction were found. Sexual reproduction gives rise to ascospores and asexual reproduction to arthrospores. The ascospores were found in thick‐walled bags, the asci; the arthrospores, that undergo gemmation, are eliminated through canals, disseminating the disease. Zusammenfassung. Die schwarze Piedra, Piedra nigra oder noduläre Trichomykose ist eine oberflächliche Mykose des Haarschaftes, die in den tropischen Gebieten Südamerikas und Asiens vorkommt. Der Erreger ist Piedraia hortai. Die rasterelektronenmikroskopische Untersuchung belegt die Invasion des Haarkeratins und die Freisetzung von Sporen an der Knötchenober‐fläche. Transmissionselektronenmikroskopisch ließ sich ein komplexes, gut organisiertes Mikro‐system darstellen: Eine vom Pilz sezernierte Kittsubstanz zementiert die Hyphen zu einem Pseudoparenchym zusammen. Sexuelle und asexuelle Reproduktion tritt nebeneinander auf. Die sexuelle Vermehrung führt zur Askosporenbildung, die asexuelle zur Arthrosporenbildung. Die Askosporen finden sich in dickwandigen, sackförmigen Asci. Arthrosporen sind zur Gemmenbildung befähigt und verbreiten die Infektion durch ihre Streuung.
