Доказано, что пациенты с тромбозами и высоким риском системных тромбоэмболических осложнений (ТЭО), включая пациентов с неклапанной фибрилляцией предсердий (ФП), должны получать эффективную и безопасную антитромботическую терапию [1][2][3]. Персонифицированный подход к продленной терапии ПОАК и АВК ассоциирован с улучшением гемостазиологических маркеров тромбинемии и клинических исходов у пациентов с ВТЭ и неклапанной ФП [4][5][6][7][8]. Материалы и методы. Проведено проспективное исследование на базе регионального центра антитромботической терапии (РЦАТТ) Архангельской области «Первой городской клинической больницы им. Е. Е. Волосевич» (ГБУЗ АО ПГКБ). Объект исследования -пациенты, входящие в регистр терапии АВК, обратившиеся в РЦАТТ (n = 107), которым проведено фармакогенетическое тестирование (определение генотипа CYP2C9, CYP4F2, VKORC1) с расчетом индивидуальных доз варфарина, оценкой качества жизни с использованием валидизированных опросников. Проведен анализ эффективности и безопасности варфаринотерапии. Результаты. Установлено, что осложнения варфаринотерапии развились вне зависимости от присутствия генотипа (СУР2С9, CYP4F2, VKORC1) и алгоритма дозирования варфарина в условиях наблюдения в АК. Показано, что при условии наблюдения в АК фармакогенетическое тестирование у пациентов с ВТЭ и ФП является дополнительным методом. Заключение. Персонифицированный подход к продленной терапии АВК с использованием фармакогенетического тестирования целесообразен для пациентов старшей возрастной группы, пациентов с отягощенным геморрагическим анамнезом, частыми малыми кровотечениями при целевых значениях МНО и наличии сопутствующей терапии ингибиторами цитохромов Р450.
