Душанбе.Республика Таджикистан В статье представлен обзор современной литературы, поcвященный осложнениям лапароскопиче-ской холецистэктомии (ЛХЭ). По литературным данным, различные осложнения при ЛХЭ встречаются в 0,1-0,6% случаев. Наиболее частыми осложнениями являются: пересечение желчевыводящих путей, ране-ние крупных сосудов брюшной полости и ворот печени. Факторами риска развития осложнений являются: хронический воспалительный инфильтрат в области желчного пузыря и гепатодуоденальной связки, ожи-рение, наличие избыточного отложения жировой ткани в области lig. hepatoduodenale, плохая экспозиция и визуализация в области треугольника Кало. Частота повреждений желчных протоков при холецистэктомии варьирует от 0,22 до 0,86%. Повреждение артерий при ЛХЭ является одним из сложных видов травм. Наи-более часто повреждается правая печеночная артерия, частота достигает 7%.Наибольшее применение при повреждение желчевыводящих путей (ЖПВ) находят классификации Bismuth-Corlette и Э.И. Гальперина. При повреждении ЖПВ во многих случаях операцией выбора считается наложение билиодигистивного анастомоза по Ру, гепатикоеюностомия с чреспеченочным каркасным дре-нированием по Pradery-Smith или Goetze-Saypol-Kurian и гепатикоеюностомия по Hepp-Couinaud. Частота хороших и удовлетворительных результатов лечения пациентов с повреждением ЖВП при применении би-лиодигистивных анастомозов составляет от 46 до 94%. Восстановление же холедоха на Т-образном дренаже по типу «конец в конец» в 50-60% случаев приводит к неудовлетворительным результатам. According to the literature data various complications during LCE occur in 0,1-0,6 % of cases. The most frequent complications are: the intersection of the biliary ducts, injuries of large abdominal vessels and the hepatic porta. Risk factors of intraoperative complications are chronic inflammatory infiltrate in the gall bladder and hepato-duodenal ligament, obesity, fat deposits in the lig. hepatoduodenale zone, a poor exposure and visualization in the Calot triangle. The frequency of bile duct (BD) injury during cholecystectomy ranges from 0,22 to 0,86 %. Damaging of arteries during the operation is one of the most difficult types of injuries. The right hepatic artery is damaged more often, the incidence of damage reaches 7%.The classification of Bismuth-Corlette and E.E. Galperin finds the largest application in case of bile ducts damage. When bile ducts are damaged in many cases Roux biliary-enteric anastomosis, hepaticojejunostomy with transhepatic drainage by Pradery-Smith or Goetze-Saypol-Kurian, and hepatico-jejunostomy by Hepp-Couinaud are considered to be the operation of choice.
