The article presents an analysis of literature data on the changes in structure and frequency of eye injuries among the adult population of the Russian Federation over the recent years. An estimation of various socio-economic and medical factors influencing these indicators was derived. The article also reviews current methods of treatment of eye injuries, and provides an evaluation of their effectiveness.
Цель. Оценить гипотензивный эффект факоэмульсификации катаракты (ФЭК) у пациентов с ранее не оперированной открытоугольной глаукомой различных стадий. Материал и методы. Проведен анализ результатов ФЭК у 136 пациентов (136 глаз). Средний возраст пациентов 72±8,9 лет. ФЭК с имплантацией гибкой ИОЛ выполнялась по стандартной технологии тоннельным разрезом 2,0 мм на факоэмульсификаторах Infinity и Centurion Vision System.Пациенты были распределены на 3 группы: 1-ягруппа – I стадия, 2-я группа – II стадия, 3-я группа – III стадия глаукомы. Срок наблюдения 1 год. Результаты. В 1-й группе отмечалось снижение ВГД на срок наблюдения 1 год на 10% от исходного уровня: с 18,5±4,3 до 16,7±2,4 мм рт.ст. Во 2-й группе пациентов выявлено снижение ВГД с 18,4±4,5 до17,4±3,5 мм рт.ст. (на 7,6%). В 3-й группе отмечалось снижение ВГД с 19,8±4,1 до 18,3±2,3 мм рт.ст.(на 7,5%). Выводы. Выявлено достоверное снижение ВГД после выполнения ФЭК на глазах с начальной глаукомой. У пациентов с развитой и далекозашедшей стадией глаукомы гипотензивный эффект достоверно ниже.
Purpose. Analysis of the effectiveness of surgical treatment of patients with primary open-angle glaucoma associated with cataract the first stage realization of cataract phacoemulsification with IOL implantation (PHACO), and the second stage - microinvasive nonpenetrating deep sclerectomy (MNPDS). Material and methods. The results of 234 cases of performing PHACO and MNPDS at various stages have been analyzed. The main group included patients who underwent PHACO, followed by MNPDS – 39 persons, 40 eyes. The group was divided into a subgroup based on the date of the second intervention. 1 subgroup – patients who underwent MNPDS 1 month after PHACO (11 cases); 2 subgroup – 3 months after PHACO (29 cases). The control group included patients who underwent MNPDS followed by PHACO – 180 persons, 195 eyes. Also, subgroups of patients were identified for whom PHACO + IOL was performed in the second stage after 1 month – 1 subgroup (99 cases) and after 3 months – 2 subgroup (96 cases). Results. Patients of the main group showed a significant decrease in intraocular pressure after each stage of treatment. in the control group, intraocular pressure decreased after the first stage of treatment, but increased 6 months after surgery. Conclusions. According to the study, in primary open-angle glaucoma associated with cataract, the most persistent hypotensive effect is observed when the first stage of cataract phacoemulsification is performed, and the second stage is antiglaucoma surgical intervention.
