The boundaries are the carpal bones and the flexor retinaculum. In addition to the medial nerve, the carpal tunnel contains nine tendons: the flexor pollicis longus, the four flexor digitorum superficialis and the four flexor digitorum profundus. Ultrasound (US) study of the carpal tunnel generally involves short-axis imaging of the tendons, and in the presence of disease, long-axis imaging and dynamic maneuvers are added. There are numerous reports of anatomical variants of the wrist involving vessels, nerves, tendons and muscles, and they can all be studied by US. Some are particularly relevant from a clinical point of view and will therefore be accurately described. The anatomy is complex, and the US operator should therefore be thoroughly familiar with the normal anatomy as well as the anatomical variants that may have a role in the pathogenesis of carpal tunnel syndrome or influence treatment.Sommario Il tunnel carpale è un canale osteo-fibroso localizzato nella regione volare del polso, delimitato profondamente, medialmente e lateralmente dalle ossa del carpo e superficialmente dal retinacolo dei flessori. Al suo interno decorrono nove tendini: il flessore lungo del pollice, i quattro flessori superficiali delle dita, i quattro flessori profondi delle dita ed un nervo, il mediano. Lo studio ecografico del tunnel carpale avviene generalmente per asse corto rispetto ai tendini ed è completato, almeno in presenza di patologia, da scansioni per asse lungo e manovre dinamiche. A livello del polso esistono poi numerose varianti anatomiche coinvolgenti vasi, nervi, tendini e muscoli, tutte ben valutabili ecograficamente. Alcune hanno una maggior rilevanza clinica e sono pertanto oggetto di una descrizione più accurata. L'anatomia normale è complessa, ma la sua conoscenza è essenziale per lo studio della patologia, come è essenziale la conoscenza delle varianti anatomiche che possono avere un ruolo nella patogenesi della sindrome del tunnel carpale o condizionarne le terapie. ª
Hamartomas, also known as fibroadenolipomas, are rare, benign formations that can develop in various organs, including the breast. They present clinically as a soft, mobile nodular lesions and are generally asymptomatic. They may be discovered incidentally during imaging studies performed for other reasons. Owing to the increasingly widespread use of mammographic screening, the diagnosis of breast hamartomas is on the rise. The masses are associated with specific mammographic and sonographic features that reflect their diverse tissue components. They also appear to present reproducible features on elastography. This article reviews the typical features of breast hamartomas seen on these three imaging modalities.Keywords Breast hamartoma Á Ultrasound Á Breast elastography Sommario L'amartoma, o fibroadenolipoma, è una rara formazione benigna che può insorgere in diversi organi, tra i quali la mammella. Clinicamente si presenta con aspetto nodulare soffice e mobile rispetto ai piani circostanti; generalmente asintomatico, può rappresentare un riscontro occasionale nel corso di esami strumentali, in particolare la diffusione dei programmi di screening mammografico ha comportato un maggiore riscontro di tale lesione. Presenta caratteristiche mammografiche ed ecografiche peculiari, legate alle differenti componenti tissutali che lo costituiscono. Anche l'elastonosografia mostra aspetti riproducibili.Vengono presentati gli aspetti tipici di questa lesione con ecografia, mammografia ed elastosonografia.
Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant disorder characterized by the presence of telangiectasias and arteriovenous malformations in various organs and organ systems, including the liver. The most frequent clinical manifestation of HHT is epistaxis. In 2010 (JanuaryeDecember inclusive) 51 patients with HHT were admitted to the ENT ward of our hospital for epistaxis, and in accordance with routine protocols, all 51 underwent abdominal ultrasonography in our department to detect hepatovascular lesions. They included 27 males (53%) and 24 (47%) females ranging in age from 11 to 86 years (mean 48.5 years). The sample was selected in an arbitrary manner to take maximum advantage of the hospital stay and monitor patients from regions other than our own. Retrospective analysis of the findings from these sonographic examinations revealed hepatic HHT in 27 (53%) of the 51 patients. Nineteen (70%) of these (age range 40e86 years, mean 63) had vascular malformations of various dimensions but no portal hypertension; the other eight (30%) (age range 39e81 years, mean 60) had vascular malformations plus portal hypertension. Our retrospective analysis indicates that a significant number of patients can have unrecognized hepatic involvement; that the appearance of hepatic lesions can be fairly unpredictable, even when the HHT has been diagnosed for years and the patients are already symptomatic; and that the hepatic lesions are frequently progressive. Therefore, regular sonographic follow-up is advisable for patients with HHT. The limitations of this study are related to the small number of patients examined and to the fact that all of them were symptomatic. Further study is therefore needed (especially in asymptomatic patients) to define the indications for hepatic sonography and the optimum examination schedule. Sommario La telangectasia emorragica ereditaria è una malattia autosomica dominante caratterizzata dalla presenza di telangectasie e di malformazioni artero-venose in vari organi e apparati. La sua manifestazione clinica più frequente è l'epistassi. Nel 2010 (gennaio-dicembre inclusi) 51 pazienti sono stati ricoverati nel reparto di otorinolaringoiatria (ORL) del nostro ospedale per epistassi e, secondo la loro prassi, tutti i 51 pazienti sono stati sottoposti a ecografia dell'addome nel nostro reparto per la ricerca di lesioni epatiche. Il campione era costituito da 27 maschi (53%) e 24 (47%) femmine, di età compresa tra 11 e 86 anni (media 48,5). È stato scelto questo campione in maniera arbitraria per sfruttare al meglio il tempo del ricovero e per monitorare pazienti provenienti anche da altre regioni.
