When using ultrasound for detection of kidney enlargement, we found an acute abdominal aortic aneurysm secondary to aortitis following umbilical artery catheterisation in a premature neonate with systemic candidiasis. Aortography was performed to provide vascular details such as involvement of celiac, renal, iliac and femoral arteries.
ZusammenfassungHintergrund und Fragestellung: Die mit dem Aufenthalt ihres Kindes auf einer neonatologischen Intensivstation verbundenen emotionalen Belastungen und deren Folgen gelten als erhebliche Risikofaktoren nicht nur für die psychische Gesundheit der Eltern, sondern langfristig auch für die Eltern-Kind-Beziehung. Erhoben wird das Ausmaû der elterlichen Hilflosigkeit und Depression sowie Bewältigungsmechanismen und der Bedarf an psychologischer Unterstützung in dieser schwierigen Zeit. Selbsteinschätzung der Eltern und Fremdeinschätzung durch das Betreuungspersonal werden gegenübergestellt. Patientinnen und Methoden: Zur Einschätzung der elterlichen Befindlichkeit wurden den Eltern von 200 NICU-Patienten und dem betreuenden ärztlichen und pflegerischen Personal Fragebogen vorgelegt. Der Einfluss der kindlichen Krankheitsbelastung wurde durch die Bildung von ¹Risikogruppenª berücksichtigt. Ergebnisse: Zehn Prozent der Mütter und sechs Prozent der Väter fühlten sich hilflos und alleingelassen. Bei jeder dritten Mutter und jedem siebten Vater dominierten Traurigkeit und Depressivität. Das Ausmaû der psychischen Belastungen war unabhängig vom medizinischen Risiko des Kindes. Neben individuellen Ressourcen war auch psychologische Unterstützung, die 33% der Mütter und 17 % der Väter erhielten, hilfreich. Zwischen der elterlichen Selbstbeurteilung und der Fremdbeurteilung durch das Betreuungsteam bestand eine geringe Übereinstimmung.
AbstractBackground and Aims: The emotional stress caused by the stay of a newborn in the NICU and the consequences of such stress constitute risk factors not only for the psychological health of parents but, in the long term, also for the parent-infant relationship. We evaluated the extent of parental helplessness and depression, coping strategies of parents and the need for psychological support during this difficult period. Self-assessment of parents and external assessment by the medical and nursing staff were compared. Patients and Methods: To evaluate the psychological and emotional state of parents, the parents of 200 NICU patients as well as the medical and nursing staff were given questionnaires. The severity of neonatal illness and its influence were taken into account by forming so-called risk groups. Results: Ten percent of the mothers and six percent of the fathers felt helpless and isolated. Every third mother and seventh father experienced predominant feelings of sadness and depression. The extent of psychological stress was independent of the medical risk for the infant. In addition to individual resources, psychological support (received by 33% of the mothers and 17 % of the fathers) was helpful. The self-assessment of parents did not concur with external assessment by the staff. Conclusions: In view of the high burden on parents, there evidently is a significant need for psychological support. An initial psychological assessment of all parents is recommended. Given
