L'hallux valgus est depuis plus d'un siècle et demi, la déformation ostéoarticulaire de l'avant-pied ayant le plus stimulé l'imagination des chirurgiens qui se sont intéressés à cette pathologie et à son traitement. De nombreuses techniques et variantes chirurgicales ont été décrites au fil des années pour atteindre environ 150 procédures thérapeutiques. L'ostéotomie Scarf est une technique décrite par Burutaran en 1976, reprise par Weil et Borelli en 1990 et popularisée en France par Barouk. Nous utilisons pour le traitement de l'hallux valgus une ostéotomie de Scarf sans avoir recours à une ostéosynthèse. Cette technique a été décrite par Maestro en 1999. Dans notre série rétrospective de 35 pieds opérés dans notre service, nous avons étudié le résultat clinique et radiologique afin d'évaluer nos résultats et de démontrer la fiabilité et la reproductibilité de la technique que nous utilisons.Les modifications techniques apportées au Scarf classique par Maestro consistent à la réalisation d'un trait d'ostéotomie transversal proximal plus oblique, à la réalisation d'une encoche au niveau du versant médial de la tête méta-tarsienne où vient s'encastrer le coin latéral distal du fragment proximal, le remplacement de la vis distale par une ostéosuture et le remplacement de la vis proximale par l'enclavement du coin osseux, réséqué aux dépens du fragment proximal.L'ostéotomie de Scarf sans ostéosynthèse remplit toutes les conditions nécessaires pour être une technique de choix dans le traitement de l'hallux valgus offrant d'excellents résultats pour un taux faible de complications.Abstract Hallux valgus is the most frequent deformity of the forefoot pathology, especially among the female population.This deformation involves a medial deviation of the first metatarsal head and a lateral deflection, with often, a pronation to hallux.Hallux valgus is for more than a century and a half, a forefoot osteoarticular deformity, which has most fired the imagination of surgeons who were interested in this disease and its treatment.Many surgical techniques and variants have been described over the years reaching about 150 therapeutic procedures.Scarf osteotomy technique was first described by Burutaran in 1976, developped by Weil and Borelli in 1990 and popularized in France by Barouk.We use for the treatment of hallux valgus an osteotomy Scarf without using internal fixation (osteosynthesis). This technique has been described by Maestro in 1999.In our retrospective series of 35 feet operated in our department, we investigated the clinical and radiological results in order to evaluate our results and demonstrate the reliability and reproducibility of the technique we use.
