Schon vor Jahren war uns eino unter den Friihgeburten hs Pneumonieform aufgefallen, die sieh fast symptomlos entwickelte, allen therapeutischen Ma~nahmen zu trotzen schien und sehlieBlieh den Tod der Kinder verursachte. Bei der Sektion fanden sich regelmiBig beide Lungen bis auf geringe ~berreste infiltrier~. Dieses Krankheitsbfld land dureh Benecke eine iiberrasehende Aufklirung. Benecke sah, dab es sich hier nicht um die gew6hnliehen Bronchopneumonien des S~uglings handelt, sondern um Verinderungen, die fast ausschlieBlieh im !nterstitium ihren Sitz hatten. Das interstitielle Gewebe war miehtig vermehrt und enthielt reichlieh Plasmazellen. Die Alveolen land er teils komprimiert und luftleer, teils mit einem stark fs zellarmen Exsudat. geffillt.Naehdem wir auf diese histologische Besonderheit aufmerksam gemach~ waren, versuehten wir, das uns sehon in manchen Punkten vertraute klinisehe Bfld in seinen Symptomen so deutlich nachzuzeichne n, dab sich auch im Krankheitsbfld die Abgrenzung yon den fibrigen Formen der Pneumonie ermSgliehen lieB. Zugleich waren wir bestrebt, die Entstehung, die Verlaufsformen und Komplikationen der Krankheit kennen zu lernen. SchlieBlich strebten wir danaeh, eine wirksame Behandlung der Erkr~nl~ung zu finden. Nach Iangen und vergeblichen Bemiihungen glauben w/r aueh hiefffir eine im wesentliehen befriedigende LSsung mitteilen zu kSnnen.
Das klinische Bild.Das roll entwiekelte klinisehe Bild wird yon den Erseheinungen hoehgradigster Atemnot beherrseht. Der Ausdruck angstvoller Qual im Gesicht der Kinder wird yon Zeiten g/inzlieher Ersch6pfung und Teilnahmslosigkeit abgel6s~. Die Augen schauen in die Ferne, als ob sic die Leiden des K6rpers vergessen hgt~en, l~ur die besehleunigte, st6hnende und miihsame Atmung zeigt begngstigend die schwere Not, die Nasenfliigel sind weir gebls aueh der Mund sehnappt mit jedem Atemzug naeh Luft. Auf der bliulich blassen Haut des Gesiehtes steht der SchweiI]. Manehe dieser Kinder sind yon einem anhaltenden Husten-
